1 Distribution Business Electrical Safety Performance Reporting Guidelines

2 1. General The Electricity Safety Act 1998 (the Act) specifies that Energy Safe Victoria (ESV) has eleven functions. Two of the critical functions are to investigate events or incidents which have implications for electrical safety and advise the electricity industry and the community in relation to electricity safety. To support the activities that deliver these functions, ESV keeps a record of all reported electrical incidents and events. The information acquired from these reports, and any subsequent investigations, is used to determine root cause, take enforcement action, and to consider actions to mitigate future risks. Remedial action may include the revision of regulations, providing information and guidance to special interest groups or the public in general, or the establishment of special committees to set rules for improving safety. These Guidelines provides distribution businesses (DBs), who are major electricity companies (MECs) as defined in the Act, with guidance on the application of the Act and regulations 27 and 28 of the Electricity Safety (Management) Regulations 2009 (the Regulations) when reporting incidents involving their assets. ESV must be notified of serious electrical incidents as a requirement of the Act. Failure to report such an incident is a serious breach of the Act. Fatal or serious electrical incidents are to be reported to ESV immediately after making any necessary contact with emergency services such as police, ambulance services providers, Metropolitan Fire and Emergency Services Board, or Country Fire Authority. 7 3. Reporting by distribution business (MEC) to ESV 3.1. Actions to be taken by a Distribution Business regarding relevant details within its knowledge of any serious electrical incident associated with its network (section 142(1) & (2) of the Act and regulation 27(2) of the Electricity Safety (Management) Regulations) Incident Notification and report by distribution business If as a result of any serious electrical incident; which the death of a person, other than MEC Worker, is directly caused by electricity (excluding suicide); the death of a MEC Worker is directly caused by electricity (excluding suicide); or the death of a person is indirectly caused by or involving electricity (excluding suicide); a person takes his or her own life (suicide); or a person, other than MEC Worker, is injured; or a MEC Worker is injured; or significant damage to property has occurred; or significant disruption to the community has occurred; or a transmission line is involved; or an incident is serious enough to warrant on site action to mitigate risk to the public by a statutory body or emergency service providers (excludes attendance by a single emergency unit to keep the public away from a potentially dangerous situation eg wire down); or a reverse polarity is resulted when the electrical circuit in question has been in service or deemed to be in service, ; or (a) As soon as practicable report to ESV by telephone all relevant details; and (b) Follow up with a completed Electrical Incident Notification form (refer to appendix A: Schedule 1) sent to ESV by within 2 business days of the telephone report; and (c) Send by a completed Electrical Incident Report form (refer to appendix B: Schedule 2) to ESV within 20 business days of the telephone report Uncontrolled copy when printed Version 4.0 July 2014, DOC/13/4099 Page 6 of 23

8 Incident (continued from previous page) accidental contact with live high voltage bare overhead conductors or exposed live high voltage underground cables has occurred; or tree or tree branch has been caused to contact a live bare high voltage conductor by MEC Worker; or tree or tree branch has been caused to contact live bare high voltage conductor by a person other than MEC worker, or ground or vegetation fires in HBRA (irrespective of area burnt) are occurred; or property damage by high voltage injection (ac or dc) occurred; or a person receives electrical shock due to any electrical work or other activities undertaken by MEC worker within the 12 month period prior to the occurrence of the electrical shock. Actions to be taken by a Distribution Business within its Network regarding relevant details within its knowledge of any other serious electrical incident not reported in section 3.1 above (Section 142(1) & (2) and Regulation 27(2)) Incident If as a result of any other serious electrical incident; which any person receives an electric shock as a result of direct or indirect contact with any network assets or an electrical installation; or live high or low voltage overhead bare conductors are on the ground or at reduced clearances less than 4.3 metres to the ground; or Report by distribution business Send by a completed Electrical Incident Report Form (refer to appendix B: Schedule 2) to ESV within 20 business days of becoming aware of the incident there is uncontrolled release of live bare HV or LV conductor by MEC Worker; or any electrical asset failure resulting in asset fire (Pole Fire, Cross-arm fire, Transformer failure, EDO fuse failure, Street-light, etc); or any act of vandalism, damage or interference on MEC s electrical asset that results in reduction of electrical safety Uncontrolled copy when printed Version 4.0 July 2014, DOC/13/4099 Page 8 of 23

22 7. ESV Annual Public Report For the purposes of creating an annual report, ESV also requests the following information by 30 January of each year. Statistics based on calendar year (Jan Dec).
