As part of Hospital Corporation of America s North Florida Division, both Fort Walton Beach Medical Center and Twin Cities Hospital are dedicated to providing compassionate, accessible, high quality, cost effective healthcare. Our commitment is to provide our patients with the highest level of care right here in our communities. In addition to being accredited by The Joint Commission, we are proud to be recognized by Healthgrades, the leading independent healthcare ratings organization, in their recent thirteenth annual Hospital Quality in America Study as detailed on page five of this report. While these recognitions speak well of some of our major achievements, we are also focused on promoting patient safety and quality as detailed throughout this report. As the area s largest employer with 1,278 employees, we are also proud of our contribution to the communities we serve. During 2010, we treated approximately 148,000 patients, paid over $84,000,000 in salaries and benefits, provided over $16,000,000 in uncompensated care and paid nearly $9,000,000 in taxes. In addition, due to our strong commitment to serving the needs of our community, our hospitals and employees generously supported our community through donations and volunteer time to health related organizations, United Way agencies and other non-profits, charities, schools, youth programs, and the arts. 7 Awards and Recognition Primary Stroke Center Certification Fort Walton Beach Medical Center and Twin Cities Hospital are accredited by The Joint Commission. Joint Commission accredited hospitals have demonstrated that they meet the highest standards in their field. Fort Walton Beach Medical Center and Twin Cities Hospital are proud to display The Joint Commission s Gold Seal of Approval, a visible symbol of our commitment to providing the highest level of care. Fort Walton Beach Medical Center is also the only Joint Commission accredited Advanced Primary Stroke Center in the tri-county area. This accreditation is awarded on the basis that quality care is effectively managed to meet the unique and specialized needs of stroke patients and the national standards for improvement of outcomes of stroke. HealthGrades, the leading independent healthcare ratings organization, also recognized Fort Walton Beach Medical Center in their most recent study which annually assesses patient outcomes mortality and complication rates at virtually all of the nation s nearly 5,000 non-federal hospitals. According to the thirteenth annual HealthGrades Hospital Quality in America Study, Fort Walton Beach Medical Center is ranked among the top 5% in the nation in Spine Surgery and 10% in Cardiac Surgery and Overall Orthopedics care in the nation. In addition, Fort Walton Beach Medical Center has received the Cardiac Care Excellence Award and Orthopedic Surgery Excellence Award, and is also a recipient of 5-Star Ratings in Coronary Bypass Surgery, Spine Surgery, Hip Fracture Repair and Back & Neck Surgery. Twin Cities Hospital s commitment to high quality and personalized patient care is continuously reflected in the awards it earns and the excellent rankings achieved in patient satisfaction surveys. In 2011, HealthGrades awarded Twin Cities Hospital 5-Star Ratings for the Quality Treatment of Chronic Obstructive Pulmonary Disease for the 4 th year in a row and the Quality Treatment of Pneumonia for the 9 th year in a row. Twin Cities Hospital has achieved the highest rank in the HCA Eastern Group of hospitals and was number three overall in HCA for the 4 th Quarter of 2010 on the Hospital Consumer Assessment of Healthcare Providers and Systems Survey (HCAHPS). This national survey focuses on patient s perception of customer service and quality related activities in each hospital. For the second consecutive year, Twin Cities Hospital has been selected as a Hospital of Choice Award recipient by the American Alliance of Healthcare Providers (AAHCP). Since the awards inception in 2002, AAHCP evaluates approximately 400 hospitals for consideration of this award each year. The Hospital of Choice Award is designed to find America s most customer-friendly hospitals based either on an extensive application process, or by a secret review of a facility s communication and interaction with customers. The application process requires a review of six principle areas of consideration including Standards of Conduct, Performance Management & Improvement, Staff Development & Training, Systems of Communication, Good Citizenship, and Educational & Promotional material for the community. 5
8 Quality & Patient Safety In addition to our achievements, HCA North Florida recognizes the importance of patient safety and continues to make it paramount. Our initiatives ensure the safety and quality of care given to our patients each day. Accredited Chest Pain Center Designation Fort Walton Beach Medical Center received designation as an Accredited Chest Pain Center by the Society of Chest Pain Centers (SCPC) in The accreditation involved intense review of all processes, procedures and outcomes related to heart care -- ensuring that patients get the treatment they need during the critical early stage of a heart attack. SCPC cited the hospital s strong relationship and teamwork with area emergency medical services staff in achieving this designation. PatientKeeper (PK) Vitals Capture TM PK Vitals Capture is a computer software application that streamlines the process of capturing and recording patient vital signs and intake/output data. Nurses and patient care technicians (PCTs) enter this information into a handheld device right at the patient s bedside, saving time while also helping to avoid possible errors due to duplicated efforts. The information is transferred into Meditech MIS the hospital s information system wirelessly within a few minutes of being entered into the handheld device. This allows for faster, more accurate information for both nurses and physicians and less time spent documenting by the PCTs. Door to Balloon Initiative Reducing the treatment time for heart attack patients continued as a major initiative at Fort Walton Beach Medical Center in Door to balloon is the time from when a patient arrives at the hospital to when the blocked artery is cleared in the cardiac catheterization laboratory. The teamwork of EMS, the emergency department and cardiac catheterization laboratory has results in an average of door to balloon time of 53 minutes in 2010, compared to the national benchmark of 90 minutes, and a 2009 average of 61 minutes, dramatically improving the lives of our patients experiencing a heart attack. Pediatric Medication Management Enhancement in the ED Fort Walton Beach Medical Center and Twin Cities Hospital emergency departments added the Artemis system for the treatment of children. Artemis is a computerized system that ensures children get the correct dosage of medication quickly and safely. MedDirect The MedDirect System allows nursing to scan medication orders from the units directly to our pharmacy department. This system allows pharmacists to enter orders in a timely manner into the medical record system. Flu Prevention Fort Walton Beach Medical Center and Twin Cities Hospital were leaders in flu immunizations last year in the community and healthcare setting. Community immunization clinics were held and flu shots were also offered to all inpatients. The hospital required all healthcare providers to either obtain the seasonal flu vaccine or wear a mask throughout flu season and achieved 100% compliance. 6
9 Quality & Patient Safety (continued) Electronic Health Record (EHR)/hCare Clinician Portal With EHR, access to patient health information is simplified and patient safety and quality improved. Our hospitals have brought numerous clinical components for the EHR on-line through major investments in technology. The hcare Clinician Portal is an electronic gateway for a physician to view meaningful patient information, clinical data and systems to meet HITECH federal requirements. Physicians can access EKG images, images and reports from cardiology and radiology picture archive communication systems, via one tool from their hospital or remotely via the Internet. Picture Archiving and Communication System (PACS) The PACS system immediately digitizes images such as X-rays and other scans including CT, MRI and PET, so they can be stored and electronically viewed on screens anywhere. This enables doctors and other healthcare professionals to access the images, even remotely, and to compare new images with previous images at the touch of a button. Patients are able to obtain their images on disk for easier management and referral. Voice Recognition To improve report turnaround time and patient care, our hospitals use voice recognition that allows radiologists to report their interpretation of a study directly from the PACS imaging system by speaking instead of dictating. The physician then reviews the report on screen, makes any adjustments, signs the report and it posts immediately for other care providers to view as necessary. MUSE Fort Walton Beach Medical Center and Twin Cities Hospital use MUSE, a system that digitizes EKG tracings so physicians can quickly review EKGs from any computer for the diagnosis and treatment of heart patients. Lifenet Fort Walton Beach Medical Center, Twin Cities Hospital, Okaloosa County EMS and area hospitals partner to use a hightech communications system to enhance the timeliness of heart diagnosis and treatment. The upgraded computerized upgraded Lifenet system allows paramedics to transmit vital cardiac patient information to the closest hospital for an emergency department physician to view while they are on scene or transporting a patient. The physician directs the EMS team to proceed to the nearest hospital, or, in the case of an abnormal EKG reading that is consistent with a heart attack, transport the patient to Fort Walton Beach Medical Center for acute intervention to open a blocked artery. Methicillin-Resistant Staphylococcus Aureus (MRSA) Staph infections are a growing health issue. Fort Walton Beach Medical Center and Twin Cities Hospital implemented a number of proactive initiatives to prevent these infections from occurring in the hospital. We have expanded our testing procedures for MRSA beyond screening only high-risk patients and precautions are taken to assure the spread of the infection is prevented. Infant Hearing Program All newborns at Fort Walton Beach Medical Center receive a hearing screening shortly after birth. Newborns that do not pass the screening are referred to an audiologist for further evaluation. 7
10 SPECIALIZED CARE Emergency Excellence Highly skilled emergency physicians and nurses combined with best practices and the latest technology benefitted 65,705 residents last year. Our protocol driven care, that begins in the field through direct communications with local EMS teams, meets stringent standards ensuring emergent medical needs are provided timely. Our strong focus on enhancing care and improving ER processes has also dramatically reduced average wait times to below the national average. In fact, we take so much pride in our reduced ER wait times, our current Average ER Wait Times are posted on electronic billboards throughout our communities, available on our websites at com and by texting ER to 23000, and through itriage smartphone technology for iphone and Android. Both Fort Walton Beach Medical Center and Twin Cities Hospital are leading the way in making this important time decision criteria available to residents and visitors of our communities through these innovative technologies. Destin Emergency Care Center Destin Emergency Care Center (DECC) is located at 996 Airport Road, less than one mile north of Highway 98, across from the Destin Airport. The DECC consists of 6 exam/treatment rooms. The DECC assesses and treats all ages, from neonatal to geriatric patients. With an emphasis on timely assessment and treatment, the DECC staff are skilled in the specific age-related needs of these diverse populations in this clinical setting. Every effort is made to include the patient s family or significant others in treatment, education, and discharge planning needs. The DECC has developed mechanisms for triage, treatment, and stabilization as defined in department policies and procedures. The DECC is equipped to handle traumatic, medical, pediatric, and/or psychiatric emergent health care needs. Patients requiring an advanced level of care will be stabilized and transported to the facility of their choice which has the required services available. Heart Care The Heart Center at Fort Walton Beach Medical Center is the Tri-County s only comprehensive cardiovascular program. In HealthGrades recent annual Hospital Quality in America Study, Fort Walton Beach Medical Center was recognized with its Cardiac Surgery Excellence Award, ranking the Medical Center in the Top 10% in the Nation in Cardiac Surgery. HealthGrades also named Twin Cities Hospital as a recipient of the Five Star Rating for the Treatment of Heart Failure for the fourth year in a row. The Heart Center at Fort Walton Beach Medical Center provides the latest and most advanced treatments in cardiac care including: cardiac catheterization with coronary angioplasty and stents. These advanced cardiac treatments ensure that life-saving technology and treatments are available right here in our community. The American College of Cardiology and the Centers for Medicare and Medicaid Services have established guidelines for how soon a hospital must treat a patient having a heart attack once they arrive at the hospital. The guidelines call for a patient to be taken to the catheterization lab to have their artery opened within 90 minutes of arrival to the hospital. This is often referred to as the door to balloon time. Fort Walton Beach Medical Center s Emergency Department and 8
11 SPECIALIZED CARE (continued) Catheterization Lab Team work closely together in meeting this standard of care. During 2010, the Medical Center s Heart Attack Response Team s average door to balloon time was 53 minutes, an improvement of 27 minutes over the 2008 average time of 80 minutes. For patients recovering from cardiovascular surgery and interventional cardiology procedures, both Fort Walton Beach Medical Center and Twin Cities Hospital offer cardiac rehabilitation programs which assist the cardiac patient in achieving and maintaining a state of optimal health and daily living activities through education, regular exercise, dietary counseling and lifestyle changes. Stroke Care Fort Walton Beach Medical Center is the Tri-County s only Advanced Primary Stroke Center certified by the Joint Commission. In June 2009, the Medical Center also received the American Stroke Association s Get With The Guidelines Silver Performance Achievement Award recognizing the Medical Center s success in implementing a higher standard of stroke care. In acheiving these designations, the Medical Center has demonstrated that its stroke program follows national standards and guidelines that can significantly improve outcomes of stroke patients. When a patient is suffering from a stroke, the time a hospital takes to administer the drug tpa (clot dissolving drug) is critical! Fort Walton Beach Medical Center, in cooperation with EMS (Emergency Medical Services), has demonstrated they can rapidly diagnose stroke victims and administer treatment within three hours of when the symptoms begin; the maximum time period for administration of tpa, a proven life saving intervention. Twin Cities Hospital provides state of the art teleneurology services for the evaluation and stabilization of stroke patients. When necessary, patients are transferred to Fort Walton Beach Medical Center for further treatment. Imaging Services Our hospitals provide the most comprehensive array of outpatient imaging services in the Tri-County area. Our facilities are equipped with a broad range of diagnostic technology. We are committed to offering imaging services needed across the full continuum of a patients care. Each patient receives the right services for their condition in a comfortable environment. Diagnostic Services available at our facilities include: n 64-slice CT n Bone Density/Dexa n Cardiac Cath n Digital Mammography n Echocardiogram n EEG n EKG n Fluoroscopy n MRA n Nuclear Medicine n Open and High Field MRI n PET/CT Scan n Stress Tests n Ultrasound n X-Ray Pediatric Emergency Room Fort Walton Beach Medical Center opened its New Pediatric Emergency Room in April It is the only one of its kind in the tri-county area including Okaloosa, Walton and Santa Rosa Counties. The Pediatric ER is a separate emergency room open 24/7 that is dedicated solely to treating the emergency health care needs of infants, children and adolescents from birth to 15 years of age. 9
12 In partnership with the Gulfarium and its staff of artists, the Pediatric ER provides a childfriendly ocean environment of vibrant sea creatures. Each of the ER treatment areas has a theme supported by Dolphins, Sea Lions, Sea Horses, Turtles, Puffer Fish, Eels, and Crabs. We are truly appreciative of the artwork the Gulfarium and its artists have done to make our Pediatric ER so pleasant for the children in our community. It will certainly reduce the stress level of children and parents as soon as they come into the ER. In addition to the aquarium-like décor that creates a fun and calming environment for patients, the Pediatric ER recognizes that children s medical emergencies require a special level of care. Our child friendly clinicians are trained to help children cope with medical emergencies. With the new Pediatric ER, we have dedicated space and staff to work with patients and families in providing them with specialized pediatric care in a more comfortable environment. Spine Surgery Program Fort Walton Beach Medical Center is among the top 5% in the nation for Spine Surgery as recognized by Healthgrades, the leading independent healthcare rating organization. Our team of highly trained Neurosurgeons, operating room staff, nurses and rehabilitation specialists work collaboratively to ensure spine surgery patients receive the high caliber of care and outcomes that have ranked us in the top 5% for the 4 th straight year in a row. Twin Cities Hospital spine surgeon performs one of the most advanced, minimally invasive procedures available today. The XLIF procedure dramatically reduces blood loss and surgical risk with outstanding outcomes. Dedicated specialists ensure that our spine patients receive the highest quality before and after care. Orthopedic Surgery Program In addition to general orthopedic surgery, our orthopedic surgeons offer advanced procedures including minimally invasive knee and hip resurfacing to relieve pain and help patients return to normal activities. Fort Walton Beach Medical Center s Hip and Joint Program and Twin Cities Hospital s Joint Venture Program offers prospective hip and knee surgical patients a comprehensive preoperative education program where patients and a caregiver are informed of pre, concurrent and post operative surgical and rehabilitative aspects of care. Fort Walton Beach Medical Center also offers the Tri-County s only Acute Inpatient Rehabilitation Center for those who need this level of post operative rehabilitation. Additionally, both Fort Walton Beach Medical Center and Twin Cities Hospital offer comprehensive outpatient rehabilitation services for recovering orthopedic surgical patients. Delle Diamon Niceville, Florida 10 I chose Twin Cities Hospital because I knew they would treat me with extraordinary care. They have the best of everything: wonderful physicians and nurses, all private rooms, and a state-of-the-art surgery center. It s a choice I made confidently and one I would make again in a heartbeat.
13 SPECIALIZED CARE (continued) Comprehensive Cancer Care Cancer is the second leading cause of death in this country. The Cancer Care Center at Fort Walton Beach Medical Center opened in February, 2005 and was formally recognized in 2006 as the first and only Community Hospital Cancer Program in the Tri-County area approved by The Commission on Cancer of the American College of Surgeons (ACoS). Recently, The Cancer Care Center earned recertification approval with commendation from the ACoS. According to the ACoS, receiving care through a cancer program approved by the Commission on Cancer ensures that a patient will have access to: n n n n n n Quality care close to home Comprehensive care offering to a range of state-of-the- art services and equipment A multi-specialty team approach to coordinate the best treatment options available to cancer patients Information about cancer research trials, education, and support Lifelong patient follow-up through a cancer registry that collects data on type and stage of cancers and treatment results Ongoing monitoring and improvement of cancer care The Cancer Care Center at Fort Walton Beach Medical Center received seven commendations from its recent recertification from the Commission on Cancer including: prevention and early detection, 100% compliance with patient guidelines, timely abstracting, compliance with American Joint Committee on Cancer staging, cancer related improvements, outcome analysis and cancer registry staff education. Currently approximately 1,400 cancer programs in the United States have earned approval from the Commission on Cancer of the ACoS. This group, representing roughly 25 percent of all hospitals in the United States, diagnoses and treats 80 percent of newly diagnosed cancer patients each year. 14 SPECIALIZED CARE (continued) Women s Services The Women s Suite at Fort Walton Beach Medical Center is designed to meet the personal needs of the female patient from Childbirth through menopause. The friendly, attentive Labor and Delivery staff nurture the patient and family as their baby makes its debut into the world. The warm, homey environment of the Labor, Delivery, and Recovery rooms provide a comfortable, supportive milieu and encourages family involvement in this very special occasion. Nursery staff furnishes comprehensive care to the neonates and instructs the new parents in childcare. A complete range of gynecological services is provided including specialized care after surgery. Breast diagnostics, with accredited mammography unit and instructions on breast exams help provide early detection of breast cancer. Many different types of classes are present; these include childbirth education, sibling class and various health forums all designed to provide the best care for the women of this community. The Level II Neonatal Intensive Care Unit (NICU) at The Center for New Life opened in August, This specialized unit provides for the special needs of moderately ill newborns 32 week gestations or later, birth weight of 1000 grams (2 lbs 3 oz) or more, and the more high-risk newborns born here and those transferred in from another facility. The Level II NICU is staffed 24/7 by Board Certified Neonatologists and highly skilled and compassionate registered nurses whose goal is to provide the most attentive and safest care your baby deserves right here in your community. In addition to having highly trained staff, the Level II NICU has the latest and most advanced neonatal equipment to meet the treatment needs of your premature or special needs baby. The Level II NICU also stabilizes severely ill newborns before transferring them to the appropriate tertiary care centers.
