Every day health expenses For a regular monthly premium, you and your family can claim cash back against some of the costs of every day health expenses annual limits. 3 The way it works is simple Bupa Cash Plan 100 has been designed to help with your everyday healthcare expenses and those of your family. Cash back you can rely on Receive cash back your annual benefit limits on a wide range of common health expenses. Dental Even if you are registered at a NHS Dental surgery, just two check ups a year will cost you 37 (based on NHS band 1 courses of treatment cost correct as at April 2014). Further information on NHS dental charges can be found on the NHS Choices website. With our cash plan you re able to claim cash back on dental fees up to the maximum set out for your membership level. Optical With our cash plan, you are able to claim cash back of between 50 and 0 annum, depending on your level of cover, towards the cost of routine sight tests, glasses and contact lenses when provided by a qualified ophthalmic practitioner. Regular eye examinations can help to identify some conditions before symptoms occur 1. Prescriptions The current price of an NHS prescription in England is 8.05 item (cost correct as at April 2014). You can also choose to buy an annual Prescription Payment Certificate which costs 104 (cost correct as at April 2014). With our cash plan you are able to claim cash back of between 20 to annum against these charges depending on your level of cover. Hospital in-patient stay There are costs involved in hospital stays if you are staying overnight. 4 Claiming is easy It s simple to make a claim with your cash plan. Here s exactly how it works: o To make a claim, you must first submit a claim form. If you d like us to send you one, please call * or [email protected], remembering to quote your membership number. o As soon as possible, send us the original receipts for your treatment remembering to quote your membership number via: post Bupa Health Funding, Bupa Cash Plan, 1st Floor, Tower Court, Courtaulds Way, Coventry, CV6 5NX or go online cashplanclaims.bhwcloud.com o Once your claim has been assessed, we ll work out how much money you are due and make payment directly into your bank account.
