4 Key features and benefits What is the Critical Five Policy? This policy, which is underwritten by ACE European Group Limited, trading as Combined Insurance, pays out a cash lump-sum if you are diagnosed with one of the critical illnesses that are covered under this policy. Your policy must be in force for 90 days and you must survive for at least 28 days after diagnosis in order for the claim to pay out. What conditions are covered? The complete list of conditions we cover is set out below. Cancer - certain exclusions apply; Coronary artery bypass grafts which requires surgery to divide the breastbone; Heart attack - depending how severe it is; Kidney failure - where dialysis or transplant is needed; or failure of both kidneys; Stroke that results in permanent symptoms. 5 Please remember that the heading of each critical illness is only a guide to what we cover. For example: we do not cover certain types of cancer; and to make a claim for some illnesses, you need to have permanent symptoms. Please refer to your original Policy Document for more information and detailed descriptions on each of the covered critical illnesses. Other important facts about your Critical Five Policy You decide the amount of cover you want that best suits your needs and budget. There are eleven units of cover available. Cover is sold in units. The maximum number of units you can buy dep ends on your income and is subject to an overall maximum of eleven units. This policy is available if you are between 16 and 64 years old on the date we issue the policy, and will continue until you reach the age of 70, or until you make a claim. After we pay the cash lump sum, your policy ends and you pay no more premiums. You must have been living in Ireland for tax purposes for at least 12 months before you take out your cover. You must then continue to live in Ireland for at least 40 weeks of any 52-week period after that. You must pay regular premiums to keep your cover going. Children aged 3-15 years old inclusive are eligible for cover, at no extra cost, under your policy. To cover eligible children, they must be named in the policy either at the time of purchase or later by notifying Combined Insurance in writing. Children s cover ends on their 16 th birthday. Claims in respect of your children do not affect your cover. Key Features /03
6 Key Terms Here is an explanation of some of the key terms used in this Product Summary and your Policy Document: 1. Accidental bodily injury: Physical bodily injury caused by a sudden, unexpected and violent event outside of your control. This injury must not be linked with any illness, disease, or physical disorder that you may have. It must not be contributed to by any condition which happens naturally or gets worse with time which you experience within three months of the date of the accident. Accidental bodily injury does not include post-traumatic stress. 2. Activities of daily living: Activities you carry out as part of daily living. These are moving around, dressing, eating, using the toilet and washing and bathing. 3. Convalescence: This is a term used to describe the period of recovery immediately following discharge from hospital when you are still unable to do every duty of your normal occupation and under regular care from your doctor. 4. Endorsement: An agreed amendment to your policy written especially to cover items just for you. An endorsement is also a change to your policy that we make during the term of the policy. We add an endorsement to your policy to add to or change the terms of the original contract. 5. Fracture Benefit: Minor Fracture: This is a fracture of one or more of the following: fingers, thumbs, toes or nose. Major Fracture: This is a fracture of one or more bones of the body (other than fingers, thumbs, toes or nose). 6. Hospital In-Patient Benefit: This is paid for each overnight stay following admission to a hospital. 7. Indexation: Each year on the anniversary date of the issue of the policy, the premiums and benefits increase as shown in your policy document under Schedule of Benefits. If Key Features /04
7 you make a claim, the amount of benefits payable will be those applicable on the date that your accident occurred or your accident began. 8. Insured: The person named in the Application Form as covered by the policy. 9. Material information: This is any important fact or information about you and particularly about your earnings and medical history that may affect our decision to accept you for insurance or our decision to accept or reject any claim you make. 10. Occupation: All jobs or professions from which you receive an income or profit. 11. Partial Disability: Benefits are paid for Partial Disability when the insured is unable to perform one or more important duties of their normal business or occupation (or usual activities if currently unemployed). 12. Pre-Existing Condition: This is a sickness condition for which the insured has received advice or treatment in the 12 months preceding the date of issue of the policy or has shown clear symptoms which would have caused an ordinarily prudent person to seek advice or treatment. 13. Premium: This is the amount you pay for your policy. 14. Review date: The date on which any change to the premium, after we have reviewed your premium, will apply from. 15. Start date: The day you complete and sign your Application Form and pay your first premium. 16. Total disability: This means you cannot carry out all duties of your usual business or occupation if in paid work, or your activities of daily living, if you are not in paid work. 17. 8 How much does the policy pay out? The policy pays out a cash lump-sum. You decide how much you would like this to be when you take out the policy. The summary of benefits table below shows all the possible levels of cover. Summary of benefits Important conditions for all claims under all sections The covered condition, its diagnosis and any initial consultation with symptoms that lead to it being diagnosed, must take place after your policy has been in force for at least 90 days. There are eleven benefit levels. 9 Who is covered under the plan? You can apply for the plan to cover: just you or; you and any eligible children as described below. Can children have cover? Yes. We can cover your children natural and adopted from age three up to their 16th birthday at no extra cost. You can include your children on your policy when you take it out. Please see the Policy Document for instructions on how to add children after you have taken out your cover. The amount we pay for a child diagnosed with a critical illness that we cover is 50% of your benefit. We only pay one claim for each child, after which, cover for that child will end. However, cover will continue for you and any other children covered under your policy. After the policy starts, is there anything I need to do? We will send you confirmation of your policy in the post. This will include some details about your personal circumstances which you will need to check. If the details are not correct, you must tell us immediately, as failure to do so could invalidate your policy or impact our ability to service it properly. Key Features /07
10 What this policy does not cover When will the policy not pay out? We will not pay for any claim for you or your children resulting directly or indirectly from the following: An illness that does not meet our definition of one of the critical illnesses we cover. For example, we do not cover some types of cancer; Inappropriate use of alcohol or drugs, including: - over consumption of alcohol - taking an overdose of drugs, whether legally prescribed drugs or not; and - taking controlled drugs (as defined by the Misuse of Drugs Act 1971) other than in line with a legal prescription. Bodily injuries; Conditions excluded by name or specific description on an endorsement to your policy; Human immunodeficiency virus (HIV); Conditions due to acquired immune deficiency syndrome (AIDS); Pre-existing conditions; Unreasonable failure to ask for or follow medical advice. As well as these exclusions, we will also not pay if: the condition or any initial consultation with symptoms that leads to its diagnosis happen: - before your policy has been in force for 90 days or more; or - within 10 days of reinstating a policy that had previously ended; death occurs within 28 days of the diagnosis of the critical illness; you live outside of Ireland for more than 12 weeks in any 52-week period; you do not tell us about your claim within six months of your diagnosis or surgery, or as soon as possible after the six months; we do not receive medical reports and evidence from a suitably qualified medical practitioner or hospital in Europe, the United States Key Features /08
11 of America, Canada, Australia or New Zealand (this evidence must be provided in a form which our Chief Medical Adviser approves); you do not provide us with the information we ask for when you take out your policy. You should not assume that we will write to your doctor. It is your responsibility to fill in the Application Form accurately; the claim is for children s cover and the condition is familial or congenital or if symptoms first arose before the start of the cover for the child. Full details of what is covered, and any limits to the cover, are given in the Policy Document. We may apply specific exclusions to your policy. You will be advised of any such exclusion one your application has been underwritten at our Head Office. All these exclusions also apply to any child named on your cover. Paying your premiums How much does the policy cost? You pay a regular premium to keep your cover in force. The amount you pay depends on: personal details for example, your age and whether you smoke; the level of cover and payment option you choose. You can pay your premiums every month, every three months, every six months or every year. You will have to pay Government Stamp Duty on all premiums. This is currently 5%, but may change. If you stop paying your premiums, your cover will end 31 days after the due date of the unpaid premium. The policy has no cash-in value at any time. Key Features /09
12 What are the charges? The premium may increase when we review our premium rates (which we do every year). Your premium will only increase if we apply a general increase to all policyholders of similiar policies. We will give you atleast 30 days notice if we plan to change your premium. Please see the Policy Document for full details. Further information is available in our Terms of Business document. Ending your cover When will the policy end? Your policy will end when: you reach age 70; you stop paying premiums; we pay you one of the benefits listed under sections A1, A2 or B on the summary of benefits (on page 14); you choose to cancel the policy; or in the event of your death; whichever happens first. An eligible child s cover will end when: your cover ends; they no longer qualify as an eligible child; we pay one of the benefits listed under sections C1, C2 or D on the summary of benefits (on page 14); you choose to cancel the policy; or in the event of their death; whichever happens first. 13 Best Doctors services Best Doctors While your Critical Five Policy is in force, you have access to leading medical advisory service provider Best Doctors if you suffer from one of the following conditions: Acquired Immune Deficiency Syndrome (AIDS); Alzheimer s disease; Blindness; Cancer; Cardiovascular conditions; Coma; Deafness; Kidney failure; Loss of speech; Major organ transplant; Motor neurone disease; Multiple sclerosis; Paralysis; Parkinson s disease; Severe burns; or Stroke. Best Doctors FindBestDoc and FindBestCare services These services which are provided by Best Doctors, not by Combined Insurance can find and recommend the top medical specialists and most experienced centres worldwide to treat your condition. 14 arrangements with Best Doctors, we will write to you with a full explanation, giving as much notice as is possible. We will then do our best to find a suitable replacement service. Making a claim If you have any questions or wish to make a claim, please contact our Customer Services Department on You can also contact us by letter or . You can find full contact details on the back page of this Product Summary. To make a claim, you must notify us within 6 months of the first day of loss occuring. Otherwise your claim may not be paid. Once you notify us, we will send you a claim form to complete. Written proof of the claims muct be recieved by us within 12 months of loss occuring. 15 Complaints procedure We want you to be fully satisfied with your Critical Five Policy. If you are not satisfied, please contact our Customer Services Department on You can also contact us by letter or . You can find full contact details on the back page. If you contact us by phone, it may be helpful to note the name of the person who deals with your enquiry. When you contact us, please quote your policy number or claim number. We aim to resolve complaints in a timely manner. Whilst we make every effort to resolve your complaint to your satisfaction, if you find our final decision is unacceptable, you can refer it to the Financial Services Ombudsman s Bureau of Ireland for review. Alternatively, you may contact the Irish Insurance Federation. Referring your complaint to either organisation will not affect your right to take legal action at a later stage. 16 Other important information Terms and conditions This Product Summary gives an overview of the key features of the Critical Five Policy. For a full list of all the definitions, exclusions, terms and conditions, please refer to your Policy Document. If we change some of the terms and conditions, we will write to you and explain these changes in advance of their taking effect. Tax Current Irish law for this type of policy, means that you cannot claim tax relief on premiums. However, benefit payments are free from income tax and capital gains. Law Irish law governs this Critical Five Policy and the Irish courts are the only courts that are entitled to settle disputes relationg to it. Key Features /14
17 Use of personal information The Data Protection Acts 1988 and 2003 provide you with rights in relation to any of your personal data held or controlled by Combined Insurance. We treat all of our customers data strictly in accordance with these Acts and the requirements of the Consumer Protection Code. You have agreed in your application form that any information which you supply to us may be used by us and other companies with the ACE group of companies to identify you when you contact us and deal with the products and services we provide to you. This may require us to share information with regulatory authorities, our reinsurers, company agents and certain other providers. If we pass your information to any entity outside of the European Economic Area (the European Union, Lichtenstein, Iceland and Norway), we will do so in line with the protections provided for by the Data Protection Acts. If you provide us with your express permission to do so, we may also occasionally contact you with details of other products provided by our group that may be of interest to you [and certain other companies see comment below]; and allow our representatives to show your name to prospective customers for marketing purposes. We are also obliged to ensure that all information held about you is kept up-to-date and accurate. If you discover that this is not the case (e.g. phone number or address is incorrect), please let us know in writing and we will update our records accordingly. You are entitled to a copy of all personal information which we hold on you.
