2 Philosophy of Nursing We believe that nursing is dedicated to: Service to one s fellow human beings Service to the care of the ill with a fostering of the return of health, providing independent living to the maximum degree possible Service to the maintenance of life and, where this is not possible, continuing skilled and dedicated nursing care which may allow the dying patient dignity and peace. Nursing is an art as well as a science in which the holistic needs of the patient are met through the utilization of the nursing process in a variety of clinical settings. The practice of nursing incorporates scientific principles, interpersonal and psychomotor skills and takes place in an ever changing health care setting that requires caring, critical thinking, competency, legal/ethical accountability, lifelong learning and patient advocacy.

3 Philosophy of Nursing (cont.) Practical nursing education is a continuous process and is accomplished through a planned program of competency based instruction using curriculum developed to proceed from the simple to complex with an emphasis upon the correlation between nursing theory and practice. For learning to occur, both student and instructor must be actively involved: ideally, the teacher acts as a facilitator and analyst, the student enthusiastically participates and studies until the program objectives are accomplished. Upon completion of the practical nursing program, qualified students can function as an essential member of the health care team under the supervision of a licensed medical practitioner, registered nurse and/or other approved licensed health professional. Practical nursing instruction prepares the graduate to function in a variety of settings with patients in all stages of the life cycle.

4 Guide/Ethical Practices 1. Adherence to provisions of the Civil Rights Act shall be strictly enforced. 2. All instruction and clerical personnel with access to confidential information shall respect the nature of that information. 3. The Student Handbook shall contain policies regarding promotion and graduation. It shall also include written policies regarding grounds for dismissal of a student. Students shall be informed of dismissal from school, promotion and graduation policies. Students facing dismissal, and parents or guardians of students who are minors, shall be afforded the opportunity to discuss reasons for this action with the coordinator and/or faculty representative according to the school s due process procedure for student grievances. 4. No student shall be prohibited from graduating and applying for a licensing examination, providing that he/she has met all the requirements of the school and all qualifications specified by the New York State Licensing Division. 5. The faculty shall endorse and teach ethical practices in keeping with the nursing profession.

5 Curriculum The HFM BOCES Career & Technical Center s Practical Nursing Program is registered by the New York State Education Department. The program is based upon a minimum of 1200 clock hours that all students must complete in two school years.

7 LPN Nursing Faculty The strength of any educational institution depends upon the caliber of its faculty members, their depth of educational preparation, their experience, general spirit of dedication, sincerity and devotion to the purposes of the school, and their desire to guide individuals to become all they are capable of being. The faculty and administration share a genuine interest in the success of their students. 8 Program Objectives The curriculum will develop the student s ability to: 1. Assess and implement comprehensive nursing care by employing the nursing process. 2. Plan and evaluate health care under the guidance of a registered professional nurse. 3. Administer nursing care based on scientific principles to clients of all age groups based upon scientific principles. 4. Utilize principles of communications to meet the affiliation needs. 5. Promote safety and security as a part of patient care. 6. Promote efforts of the community in meeting its health care goals. 7. Function as a contributing member of professional organizations. 8. Change and grow professionally.

9 Clinical Sites The LPN academic program is taught at the HFM BOCES Career & Technical Center. The clinical classes are conducted at the following health care facilities: Hospice of Fulton County Nathan Littauer Hospital and Nursing Home St. Mary s Hospital St. Mary s at Amsterdam Memorial Campus Mayer & Cope Family Practice

10 Application Process Application information may be obtained by calling, visiting, or writing to the Program of Practical Nursing, Monday Friday 7:30am 3pm. Students are admitted annually. A pre-entrance TEAS test for Practical Nursing is required before an application will be distributed. The test will take approximately 2 ½ hours and the cost is $35 and is payable by debit or credit card only. NO CASH The TEAS test was developed to measure basic essential skills in the academic areas of Reading, Math, Science, English and Language Usage. Interviews are with the LPN Advisor after successful completion of the TEAS test. Upon acceptance to the LPN Program, the student will be required to submit a commitment fee of $ payable by check/money order to HFM BOCES or cash. Deposit may be paid in payments.

Susan Roth, LPN Advisor HFM BOCES 2755 State Highway 67 Johnstown, NY At the interview, the coordinator will discuss the course content, program calendar, references, physical exam, attendance requirements, satisfactory academic progress and financial aid process. Students must obtain three (3) written references. Please do not include relatives. One reference must be from a professional (i.e. Doctor, Nurse, Minister, Priest, or Rabbi, Lawyer, Law Enforcement Officer or Teacher). Students shall be considered for admission based upon: Pre-test scores Educational background Previous experience in the health care field (not essential) Work history Interview Physical examination with required immunizations Reference checks

12 Articulation Agreement between Fulton-Montgomery Community College and Hamilton-Fulton-Montgomery Career Technical Center Objectives 1. To attract qualified candidates to FMCC and HFM CTC. 2. To facilitate educational and career mobility in the Nursing programs by increasing accessibility to associate education for qualified individuals. 3. To promote efficient transfer of qualified graduates from HFM Career Technical Center to FMCC. 4. To disseminate program specific information to students who wish to obtain a nursing degree. 5. To provide students with advisement in academic and career planning throughout their program of studies. 6. To reduce unnecessary repetition of general education and curricular content. 7. To facilitate communication and academic coordination between faculty, students, and administrators at each institution.

13 Terms of Agreement 1. To be eligible for acceptance into the FMCC Nursing Program as a second semester freshman, HFM Career Technical Center graduate must meet the following conditions: a. Application for admission to FMCC will be the responsibility of the student. b. Applicants must meet requirements of FMCC s Advanced Placement Credit for Applied Technology Education. To be recommended for advance placement credit, the individual must have completed the following pre-requisites: c. High school or college level biology d. High school or college level Algebra or its equivalency e. High school or college level Chemistry f. Four (4) credits of college level Anatomy/Physiology taken within the last seven (7) years (before entrance into NU106) g. Three (3) credits of college level General Psychology taken within the last seven (7) years (before entrance into NU106) 2. The individual must also meet the following contingencies: a. The individual must have completed the HFM Career Technical Center s Program of Practical Nursing with an average of 85% or better. b. The individual must hold a current NYS License for Practical Nurses. c. The individual must successfully complete 15 non-credit contact hours of a Nursing Process Course offered at FMCC before admittance into NU Students who meet all the pre-requisites and qualifications stated in Terms of Agreement in this document will be awarded seven (7) advanced placement credits for NU105.

14 4. FMCC and HFM Career Technical Center will disseminate information regarding this articulation agreement in appropriate publications and/or media. 5. Changes in this articulation agreement can be made at any time by mutual consent. 6. This agreement will remain in effect until terminated by either party. Termination would require no less than six (6) months notification by either party. 7. This articulation agreement will be reviewed annually by the designated person(s) at FMCC and HFM Career Technical Center.

15 Satisfactory Progress/Academic Standing A Student must receive a grade of 75% or higher in each unit of study & each clinical experience to maintain satisfactory academic progress. Students who have a failing grade or grades will be notified. Failure of one learning unit per ten (10) weeks will result in immediate academic probation. This means the student s cumulative performance falls below the minimum requirements for successful completion of the the LPN Program. Students who fail to successfully complete their work, or have a failing grade(s) must meet with the advisor. The student s progress will be reviewed. Retention, transfer or dismissal of a student is determined by the advisor with recommendation from the faculty. A contract may be developed to outline and specify requirements the student must meet for successful completion of the program. Failure of a clinical rotation will place a student on clinical probation. Failure of two clinical rotations is reason for removal from the program. Students must maintain a 75% clinical average to graduate. A student will be terminated from the program for unsafe practice in the clinical class. Unsafe practice consists of endangering or harming the emotional or physical well-being of a client.

16 Attendance Policy It is the responsibility of each student to understand the attendance policy (in student handbook) and related procedures. Students must call the HFM BOCES LPN Office prior to any absence. Students must contact the instructor for make up work after each absence. Students must call if unable to attend classroom or clinical class prior to the start of the school day. Failure to call will result in a zero for that day s participation. In order to be eligible for the NYS LPN Licensing, diligent attendance is required. Sporadic attendance or a block of absences will result in failure and/or dismissal from the LPN Program. The advisor reserves the right to dismiss any LPN student who disregards the strict attendance policy. More than seven (7) absences (combined clinical and classroom) may result in dismissal from the program.

17 Requirements for Graduation The graduate pin of the Program of Practical Nursing and the diploma bearing the authorized signatures of HFM BOCES are awarded when the student has satisfactorily completed the educational program in accordance with the curriculum and attendance requirements. Completion at this level includes a demonstrated level of performance and of meeting the curriculum and program objectives which will qualify the graduate to practice safely and satisfactorily as an entry level practical nurse in a general hospital or geriatric setting. All financial obligations of the program must also have been met.

18 State Board Licensure Practical Nurses who apply for Licensure through the NYS Education Department will be asked the following questions. A student who will be answering yes to any of the questions must make an appointment to meet the advisor within the first week of classes. 1. Have you ever been convicted of a crime (felony or misdemeanor) in any state or country? 2. Have you ever been charged with a crime (felony or misdemeanor) in any state or country, the disposition of which was other than acquittal or dismissal? 3. Have you ever surrendered your license or been found guilty of professional misconduct, unprofessional conduct, incompetence or negligence in any state or country? 4. Are charges pending against you for professional misconduct, unprofessional conduct, incompetence or negligence in any state or country? 5. Has any hospital or licensed facility restricted or terminated your professional training, employment or privileges, or have you ever voluntarily resigned or withdrawn from such association to avoid imposition of such measures? In addition, students will be asked about the status of any student loans they may have. If a student answers yes to either having an outstanding student loan or a loan in default, their name will be forwarded to the financial aid account clerk. A high school diploma or GED & completion & graduation from an approved school of nursing are required in order to sit for state boards.

19 Estimated Cost of LPN Program LPN is a two-year program, classes begin in September and end in June. First year runs from 8:30am-11:30 am on Mondays, Wednesdays, Thursdays and Fridays and Tuesdays from 7:30am 2:30pm for clinical days. Second year runs from 12:00pm-3:00 pm on Mondays, Tuesday, Wednesdays and Fridays and Thursdays from 7:30am 2:30pm for clinical days. 21 Financial Aid HFM BOCES conducts an student orientation day in August to provide information regarding obtaining loans, entrance loan counseling, rights and responsibilities of students and default prevention. Students can fill out federal student aid (FAFSA) applications online at After individual student aid reports (sars) are received from the federal government, award letters are sent to the students, detailing allocation of awards and award schedule HFM BOCES accepts the following aid: PELL, Stafford loans, GI Bill, VA benefits, Scholarships, Employer Tuition Assistance, and Workforce Development support. It is the student s responsibility to apply in a timely basis. Questions regarding financial aid information are referred to the LPN Office at , ext Refund Policy Students who officially withdraw from the LPN program by giving written notice to the Coordinator may be eligible for a tuition refund. Tuition and aid refunds are based upon federal guidelines which calculate the completion of clock hours of instruction. Students will be responsible for a tuition bill equivalent to the percentage of clock hours completed for the year.
