Extend Health benefit advisors are available weekdays from 8 a.m. to 9 p.m., Eastern Time. Identify yourself as a DuPont retiree. You may also access information online at Extend Health Enrollment Period: October 22 December 31, 2012 DID YOU KNOW? Extend Health has helped more than 500,000 Medicare-eligible individuals enroll in health care plans. Extend Health benefit advisors will provide personal support and guidance during each step to help you choose appropriate health care plans and enroll for coverage. 2

5 NEW HEALTH CARE CHOICES FOR 2013 LEARN ABOUT What s Changing and Why Effective January 1, 2013, DuPont will offer Medicare-eligible retirees and dependents a new way to access health care coverage that will provide plan choices and greater flexibility. We are partnering with a company called Extend Health. They offer a choice of Medicare plans so Medicare-eligible participants may choose a plan that works best for them. Extend Health s phone support and enrollment services are available at no cost to you as a DuPont retiree or eligible dependent. The plans offered through Extend Health provide a greater choice than the single option of the past. The Extend Health plan types include: Medigap/supplemental medical coverage that supplements or fills the gaps in your existing Medicare Parts A and B Medicare Advantage Plans (Medicare HMOs, PPOs, etc.) that replace your existing Medicare Part A and B coverages Part D prescription drug coverage Dental and Vision plans See page 6 for more detail about the plan types available. The insurance coverage you elect during the Extend Health enrollment period will take effect January 1, DuPont will provide a tax-free Health Reimbursement Arrangement (HRA) to help you cover some or all of the cost of the health insurance plan you select. Extend Health will administer the HRA accounts for DuPont participants. The HRA funds can be used to reimburse your eligible health care expenses such as monthly premiums for your new plan, deductibles and copayments, vision and even dental. We believe this arrangement with Extend Health will benefit you and DuPont because it: Provides you with a number of plan options so you can choose the best health care plan for your situation. Gives you flexibility with a new HRA from DuPont to reimburse yourself for eligible health care expenses. May save you money, depending on which plans you choose to enroll in. Simplifies the DuPont plan administration and stabilizes DuPont plan costs. 3

6 NEW HEALTH CARE CHOICES FOR 2013 Who is eligible? Medicare-eligible retirees, survivors and dependents who participate in the DuPont retiree medical and/or dental plan must contact Extend Health to select health insurance plans to replace their DuPont coverage. How you are affected by this change depends on your eligibility for Medicare: Eligible for Medicare (or over age 65) If you are a Medicare-eligible retiree Coverage currently available under the DuPont medical and dental plans will end on December 31, You must enroll in individual coverage through Extend Health to receive an HRA from DuPont. You may use the HRA to be reimbursed for health insurance premiums and other qualifying expenses. NOT eligible for Medicare If you are a retiree who is not eligible for Medicare Your DuPont group medical and or dental coverage will continue. As in the past, you will receive a newsletter in the fall informing you of any changes in coverage, benefits or premiums. When you become eligible for Medicare, you will then be eligible for the HRA and support through Extend Health You will receive information from Extend Health approximately four months before you reach age 65 to begin the enrollment process and establish your HRA. If you become eligible for Medicare due to a disability, before age 65, you must notify MyInfo at You may not continue DuPont group medical and dental coverage once you are eligible for Medicare. MyInfo will notify Extend Health to begin the enrollment process and establish your HRA. If you are a Medicare-eligible spouse of a retiree or a Medicare-eligible child of a retiree Coverage currently available under the DuPont medical and dental plans will end on December 31, You must enroll in individual coverage through Extend Health to receive an HRA contribution from DuPont. All HRA contributions are made into a joint account with the DuPont retiree or survivor as the account owner. The HRA may be used to reimburse the retiree for the cost of your health insurance premiums and other qualifying expenses. If you are a spouse who is not eligible for Medicare You will continue to be eligible for DuPont group medical and dental coverage as a dependent of an eligible DuPont retiree or survivor. As in the past, a newsletter will be sent in the fall informing you of any changes in coverage, benefits or premiums. When you become eligible for Medicare, you will then be eligible for the HRA and support from Extend Health. You will receive information from Extend Health approximately four months before you reach age 65 to begin the enrollment process and establish your HRA. If you become eligible for Medicare due to a disability, before age 65, you must notify MyInfo at You may not continue DuPont group medical and dental coverage once you are eligible for Medicare. 7 Important Note What happens if you decline coverage or don t enroll. MEDICAL PLAN PARTICIPANTS: If you are a Medicare-eligible individual participating in the DuPont medical plan, you must purchase an individual medical insurance plan through Extend Health in order to receive HRA contributions from DuPont. If in the future you cancel the medical insurance you selected through Extend Health, your HRA will end on the date your coverage ends. Purchasing dental insurance or Medicare Part D prescription coverage is optional and may be dropped or added without impacting the amount of or eligibility for your DuPont HRA contribution. DENTAL ONLY PARTICIPANTS: If you are a Medicare-eligible individual, you must purchase an individual dental insurance plan through Extend Health in order to receive HRA contributions from DuPont. If in the future you cancel the dental insurance you selected through Extend Health, your HRA will end on the date your coverage ends. Retirees and survivors must continue coverage for themselves through Extend Health in order for their dependents to be eligible for the HRA or DuPont coverage. All declinations of coverage are permanent and irrevocable. Once your HRA or DuPont health coverage ends, you will only be allowed to re-enroll if you notify MyInfo at within 60 days of losing continuous eligibility for other group health coverage, such as through another employer s group plan. Voluntarily termination of other group health coverage or termination for failure to pay premiums does not qualify as a reason for reinstatement into the DuPont plan. What resources are available through Extend Health? DuPont chose Extend Health because of their depth of experience in helping people navigate change and assisting with health care enrollment. When you are ready to enroll, their benefit advisors will become advocates for you and your Medicare-eligible dependents. They can help you choose and enroll in individual health care coverage that serves your needs and fits your budget. Don t be surprised if your Extend Health benefit advisor asks you some questions. They will need certain information to help you select the plan that s right for you and will be required to respect the confidentiality of your personal information. 8 NEW HEALTH CARE CHOICES FOR 2013 Will I have more plan choices under the Extend Health options than I do now? Review the chart below to familiarize yourself with how the various Medicare components work and take inventory of your own medical needs. Extend Health offers health care options from leading national and regional insurance companies. They can provide you with more specific information when you decide to enroll. Your Current Medicare and DuPont Choices 1. Medicare Part A: Hospital Insurance Provides you with inpatient care and covers inpatient hospital stays, home healthcare, stays in skilled nursing facilities and hospice care. Enrollment in Part A is automatic and requires no premiums. 2. Medicare Part B: Medical Insurance Provides you with outpatient care and covers physician fees and other medical services not requiring hospitalization. You must enroll in Part B and pay the required premiums. 3. DuPont Retiree Medical Coverage Provides medical coverage (including prescription drugs) secondary to your Medicare Part A and B. Your New Medical Coverage Choices for 2013 When you call to enroll, Extend Health will assist you in making a choice among these options and then enroll you in a private insurance plan as appropriate. Your choice of plans will depend on your personal health care needs, coverage preferences and financial resources. 1. Medicare Part A: Hospital Insurance, Medicare Part B: Medical Insurance, Medigap/Supplemental Coverage and Medicare Part D: Prescription Drug Plans This option starts with Parts A and B, and adds Medigap medical coverage, which is offered by private insurance companies to supplement or fill gaps in Parts A and B., In addition, you get outpatient prescription drug coverage offered through private insurance companies. 2. Medicare Part A: Hospital Insurance, Medicare Part B: Medical Insurance and Medigap/Supplemental Coverage This option starts with Parts A and B, and adds Medigap medical coverage, which is offered by private insurance companies to supplement or fill gaps in Parts A and B. Prescription drugs are not provided under this coverage. 3. Medicare Advantage Part C This plan type is offered by private insurance companies to provide your Medicare Part A and Part B coverage, plus additional coverage based on the plan you choose. There are two versions of this coverage: a. Medicare Advantage Prescription Drug (MAPD): includes prescription drug coverage. b. Medicare Advantage (MA): does not include prescription drug coverage. 6

9 Health Reimbursement Arrangement Information A Health Reimbursement Arrangement is a DuPont-funded account that you can use to reimburse yourself for qualified health care expenses, on a tax-free basis. An HRA will be established for you and your covered spouse and dependents if you each are: Eligible for Medicare, Enroll through Extend Health and Eligible participants in the DuPont medical and/or dental plans. You receive an annual HRA contribution from DuPont at the beginning of each year. The HRA funds are not taxable! You can use them to reimburse yourself for health care plan premiums and/or your share of qualified health care expenses during the year. Extend Health manages the account on your behalf. You cannot make additional contributions to this account. Any HRA funds not used during the year remain in the account for your use in later years, as long as you continue to be eligible for the HRA. Getting Reimbursed From Your HRA When You Enroll Through Extend Health 1 You Pay Premiums and Expenses 2 You Submit Out-Of Pocket Expenses 3 Extend Health Reimburses You You pay the full premiums directly to your insurance provider. You pay your provider any required out-of-pocket expenses. You submit your claim to Extend Health for your premiums and outof-pocket health care expenses. Extend Health administers your account and will reimburse you from your HRA to the extent that funds are available. HRA-Eligible Expenses Premiums for Medicare Supplement, Medicare Part D, or Medicare Advantage coverage Out-of-pocket medical expenses like deductibles, copayments, your share of coinsurance Out-of-pocket prescription drug expenses Dental and vision plan premiums Dental and vision plan out-of-pocket expenses Note that your DuPont HRA cannot be used to reimburse Medicare Part B premiums, group medical/ dental plan premiums (including Tricare), or Long-Term Care expenses. If you have additional questions about qualifying expenses covered by your HRA, please contact Extend Health toll-free at Important! You and your Medicare-eligible spouse and dependents must enroll separately in an individual insurance plan this fall through Extend Health in order to receive an HRA contribution from DuPont. 7

Provides helpful information about how to create your Extend Health profile and what you need to do. Includes a worksheet about your health history. Completing this worksheet will help the benefit advisors verify your information and recommend plan options that may work well for you. Education sessions In-person and telephone meetings to learn more about the enrollment process and plan options available in your area. Your opportunity to get your questions answered. Enrollment Guide mailed to your home. Includes reminder of the date and time of the appointment you scheduled during the Getting Started process. Explains the types of plans available for Provides definitions for important Medicare terms. Describes how to enroll.
