Currently, healthcare executives are under a mandate to make information more accessible and more complete. 8 LEADERS IN ICA: The Interoperability Experts Come as you are: A continuum for moving toward full interoperability from any baseline The healthcare industry still has much to do to achieve full interoperability. Nevertheless, hospitals can begin to leverage existing data, tools, networks and technology to create a baseline of participation that sets the stage for effective interoperability to support care coordination. Whether the initial goal is simply to transfer information from one point to another, or to perform sophisticated data analysis for population health initiatives, ICA suggests that there are key steps hospitals must take to make tangible progress. Step One: Identify and prioritize your medical trading partners It is impossible to create population health management or other collaborative care initiatives without knowing which other organizations share patients with the hospital. After all, the most effective connections are going to be with those facilities and practices that provide care for patients seen in common. Because most healthcare is delivered locally, determining where patients receive care can involve a relatively straightforward, low-tech evaluation of the surrounding healthcare community. However, organizations that prefer more analytical data might want to work directly with payers to get a more detailed snapshot of the patient population to identify where they can achieve the greatest economic impact. Step Two: Determine the processes needed for efficient workflow and care coordination It is important to identify workflow processes across your organization and, if they are currently supporting care coordination, to expand that review to the extended enterprise. Step Three: Assess the technical landscape among partner organizations After identifying and prioritizing who to share information with, hospitals must assess the technical landscape that exists across those partners. Gather information about the systems they use and their level of adoption. Questions to ask include: n Do you have an electronic health record (EHR)? n Where are you in the adoption of your EHR? Is your EHR Direct enabled? n Have you qualified for Meaningful Use Stage 1? If so, are you applying for Stage 2? n Are you participating in a health information exchange (HIE)? Step Four: Determine which data is of the highest value for sharing Until technological hurdles are overcome and full interoperability is achieved within the healthcare industry (in the absence of perfect data 100% of the time), actionable data delivered to the right person, at the right time, is better than no progress. The key is to start early to get to a functional state, set realistic expectations and not let perfection be the enemy of good enough.
