3 Dear friends of St. David s Center, Thanks to you and your belief in our work, together, we did it! We raised more than $14 million to fund our renovation and expansion; We stayed open during 12 months of construction, serving even more children while remaining on schedule; We launched new community partnerships to increase access to mental health services in several new childcare centers, schools and school districts, and we collaborated with Hennepin County Child Protection and the Juvenile Court to pilot a new Infant Court Team; We completed a new strategic plan, guiding our path for growth into 2020; And we did it all while closing with a balanced budget! This past year, our reach increased to 2,872 children, teens and adults. Commitment to our mission building relationships that nurture the development of every child and family is the foundation of all we do. 4 In October 2015, we exceeded the $13.7 million goal, raising $14.1 million to complete our Building Our Children s Future capital project. In May 2016, construction was completed and the building was named a Peter J. King Family Foundation Center of Excellence in Child Development. 2

5 Governor Dayton declared July 20, 2016, the day of the Grand Opening Celebration, St. David s Center Day in the State of Minnesota. Showcasing our state-of-the-art Minnetonka facility: October 20, 2015 was proclaimed Inclusive Playground Day by the City of Minnetonka at our new playground ribbon-cutting ceremony. AUTISM DAY TREATMENT SUITES Three unique suites pairing sensory-calming classrooms with connected therapy gyms. CHILDREN S MENTAL HEALTH CLINIC Seven assessment and treatment rooms. EARLY CHILDHOOD EDUCATION CLASSROOMS 12 enlarged, light-filled classrooms. MARA S WING, PEDIATRIC THERAPY CLINIC Five therapy gyms including a 1,500 square-foot Super Gym and five speech therapy treatment rooms. 7 SUCCESSES & ACCOMPLISHMENTS EARLY CHILDHOOD EDUCATION Our Early Childhood Education program achieved NAEYC accreditation renewal for five years, earning our highest scores to date at least 96% across all ten criteria areas. 51 children with special needs were active participants in preschool, learning side by side with typically developing children. 5

8 WHO WE SERVED IN SUCCESSES & ACCOMPLISHMENTS SPECIAL NEEDS SUPPORT SERVICES Enrollment in our community-based Therapeutic Recreation program increased by 15% thereby expanding access to out-of-school programming that builds community and encourages physical fitness for teens and young adults with disabilities. 468 clients developed life skills through in-home and community-based one-on-one personal support. 6

9 SUCCESSES & ACCOMPLISHMENTS EARLY INTERVENTION AND TREATMENT Early intervention and treatment access was expanded to nearly 2,000 children and families, 40% of whom were living in poverty. Our Mental Health services provided a continuum of care to 775 children and teens, an increase of almost 30%. More than 480 children were served in our Pediatric Therapy services with remarkable results: 89% demonstrated improved skills. We increased enrollment in our pediatric therapy clinic by 20% in the first five months of operation. More than 220 children in Osseo and Hopkins schools participated in our School-Linked Mental Health program with outstanding results: 94% of students improved social-emotional functioning. 81% of surveyed teachers indicated that our therapists helped reduce the number of children referred for a special education assessment. Launched the Infant Court Team to reach 60 children over the three-year collaborative pilot with Hennepin County Child Protection and Juvenile Court, aiming to reduce the rate of re-entry into foster care and improve child outcomes by intervening in all caregiver relationships, cooperating among the team of providers, and making clinical recommendations to the Court. 7

11 IT S A COMPLEX STORY, BUT IT S ALSO A WONDERFUL STORY. When Matthew was two days old, he entered foster care. His birth mother gave him to hospital staff through the Safe Haven program. She disclosed that she hadn t received prenatal care and had used drugs and alcohol during her pregnancy. Meanwhile, Sue and her husband, Rick, had been providing foster care for infants and were hoping to adopt. When Sue heard about Matthew, she didn t hesitate. Knowing that Matthew might be affected by Fetal Alcohol Spectrum Disorders, Sue was proactive about his treatment. As suspected, before long, Matthew was diagnosed with Alcohol-Related Neurodevelopmental Disorder. As Matthew grew and developed, it became clear that his intellect and IQ were unaffected, but he suffered from anxiety, exhibited behavioral challenges, and found sensory input upsetting. Sue began searching for occupational therapy options, and that s when she found St. David s Center. At St. David s Center, Matthew began preschool, speech and occupational therapy. Despite making forward strides, he struggled with the environment, and his teachers and Sue agreed that a traditional preschool setting wasn t the best for him. Staff suggested St. David s Center s Family Place. Family Place is a classroom- and home-based program where mental health professionals nurture the emotional and social development of young children who struggle in school settings and at home. Sue said that once Matthew started in Family Place, there was no looking back. It was very clear he did not stand out. He fit right in. And it was one of the best things we ever did. Matthew started kindergarten this fall, a transition Sue says would have been overwhelming without Family Place. Now, he goes to school wearing a backpack something he couldn t tolerate previously. Matthew used to find being around his peers difficult. Not anymore. He loves to be with other children now, said Sue. Before he wasn t able to do that. Sue credits the programs at St. David s Center for pointing them in the right direction and allowing Matthew to move toward his full potential as an individual, a friend, a classmate, and a member of his family. 9
