Place: Ministry Conference Center; Living Stream Ministry Campus 2431 W. La Palma Avenue, Anaheim, California B. Dates: Monday, July 2, 2012, 4:00pm Saturday, July 7, 2012, 9:30pm C. Registration donation: There is a registration donation of $ USD per trainee for both domestic and international trainees. D. The cost of the outlines for the live training is $5.00 USD. II. Due Date: All items required for registration must be turned in before or on May 1, All requested information on the forms must be provided. If partial registrations are received, your registration will be considered incomplete and you will not be registered until we receive all the required items and information. All registration requests after the deadline, including additions and substitutions, will be considered late and pending approval. III. Registration Information: A. Trainees must be between the ages of If an exception is made for anyone over 60, please do not request hospitality for that trainee. Young People: Attendance by young people under the age of 17 is discouraged. Exceptional cases will be considered; however, a Young People s Registration Form from the elders in the sending locality (including an elder s letter of recommendation) in addition to prior attendance in a minimum of one video training is required for each applicant under the age of 17. Young people who are 15 years old and under will not be accepted to the live training, please do not apply. All those who are under 17 years old and who have been accepted to the training will be expected to adhere to the same training requirements as all other trainees. All underage trainees must be registered for the training full time; part time and shared seats will not be allowed. Please note: The required Release of Liability Agreement must be signed by a parent or legal guardian and returned to LSM for each registrant under 18 years old. For those under 17 years old, the Young People s Registration Form and the Release of Liability must accompany their registration and they must be received on the due date or they will be considered late. B. There is a hospitality quota for those registering from the churches in the USA, Canada, and Mexico. This quota corresponds with the number of hospitality spaces your locality may occupy during this training. Borrowed spaces for hospitality will not be accepted. The nearby churches have a seating quota. C. Hospitality: 1. No additional hospitality spaces will be given above the number of your quota. 2. All those requesting hospitality must complete an enclosed hospitality card. 3. Hospitality will only be provided for domestic trainees (USA and Canada) who are 25 years of age and under. Hospitality for international trainees will be provided for those who are 60 years of age and under. INFO1-Summer

2 4. For domestic trainees (USA and Canada), do not request hospitality for trainees over 25 years of age. 5. For international trainees, do not request hospitality for trainees over 60 years of age. 6. Do not request hospitality for any trainee who needs special care (including diet or transportation). 7. Do not request hospitality for children. 8. Keep the host updated on any hospitality changes. 9. Do not attempt to switch hospitality assignments. 10. Do not lend nor borrow hospitality spaces to or from other localities. 11. If, due to a cancellation, a hospitality substitution becomes necessary, be sure it is brother for brother, or sister for sister. The substitute should know whom he/she is replacing. Be sure to notify the host directly of a substitute or cancellation and how this will affect car availability. It is best not to substitute for couples. 12. In making arrangements for air travel to Los Angeles Airport (LAX) or the Orange County Airport (SNA) please plan your arrival during daylight hours. D. If translation is needed, trainees must bring their own FM radio, earphones, and batteries. E. To limit confusion, please assign one person to coordinate training registration from your locality. F. Please use the LSM online registration system to register. If a computer is not available, please complete and return the enclosed forms by May 1, Mail, fax, or the forms early to ensure that they are received by the due date. G. The registration donations ($ USD per trainee) are to be sent in by May 1, H. The outline donations for live trainees ($5.00 USD per trainee) should also be sent in by May 1, INFO1-Summer

3 Instructions for Completing the Trainee List Please complete and return the enclosed Trainee List using the following instructions. Please provide all the information requested so your registration can be completed. All Churches: Name: Last name, First name. (Alphabetize by last name) Bro/Sis: (Brother/Sister). Age: Write in age for each trainee. Translation: If translation is needed during the meetings write C (Chinese), S (Spanish), J (Japanese), K (Korean), P (Portuguese), etc. Nearby Churches: FT/FT-SH/PT (Seat status: Full-Time/ Full-Time-Shared Seat/ Part-Time): Check the appropriate column for each trainee. Shared seats must be occupied full time. Please use only one line on the trainee list for each shared seat. Distant Churches: Hospitality Needed: Please mark Y in this column for each trainee who is requesting hospitality. Hospitality/Other: Write NO if hospitality is not needed and fill in alternate arrangement if known. Leave blank if hospitality is requested. The Agreement shall be legally binding upon me, the registered trainee, the parents or legal guardians thereof if a minor, my heirs, estate, successors, or assigns and shall be interpreted in accordance with the laws of the State of California. 2. As a trainee of the Living Stream Ministry (hereafter referred to as LSM ), I will be attending meetings in Southern California, where I realize there are potential dangers arising from, among other things, traffic, gangs, and weather conditions. In addition, I acknowledge there are other physical, emotional, and psychological stresses associated with such training. I understand that there are risks in traveling to and from the meetings, attending meetings for extended periods of time, and in participating in rigorous study sessions. I also understand that individuals from numerous other countries or regions will attend the training and that LSM is unable to completely screen all of these individuals as to their backgrounds and character. 3. In consideration of LSM allowing my participation in this training, I, the trainee, and the parents or legal guardians thereof if a minor, do agree to hold harmless and release LSM, its owners, agents, employees, officers, members, and affiliated organizations from any liability for such ordinary negligence, which may be alleged against them. I do further agree that except in the event of LSM s gross and willful negligence, I shall bring no claims, demands, actions, and/or litigation against LSM and associated individuals or organizations, for any economic or non-economic losses due to bodily injury, death, or emotional distress sustained by me and/or my minor child or legal ward in relation to the training and the premises and operations of LSM. The semi-annual trainings are recorded and made available to churches that apply for a video training. Your locality can carry out the video training by webcast or by requesting DVDs. B. You must agree to abide by the rules and regulations of the training (see enclosed). C. If your video training begins on or before Tuesday, July 10, 2012 you must take the video training by webcast. If your video training begins after Tuesday, July 10, 2012 you may take the video training by webcast or DVD. Due Date: Please adhere to the due date. All items required for registration must be turned in before or on the due date. III. Registration Information: A. Online Registration (preferred method): Living Stream Ministry offers online registration for video trainings. Please see the Online Training Registration Information Sheet (pink paper). Please complete the online forms by May 1. B. Registration by mail: Please complete and return all the enclosed application forms by May 1. IV. DVDs: A. DVD Format: Please see the accompanying information sheet on yellow paper. B. DVD pickup or shipping options: In order to facilitate obtaining your DVDs, LSM offers shipping directly to your locality. This means no designees and no DVD pickup during the training. If you choose this service option, DVDs of all messages will be shipped by the most economical method in order to arrive one day before the start of your video training. Allow a few days for international deliveries. If your local video training will start after the completion of the live training, the church can save shipping costs that will otherwise be billed to the churches if a designee can carry the DVDs home directly. Please complete the Video Training Media Information form according to your video training needs. Note: DVDs will only be available for pickup on Saturday afternoon, Saturday evening after the last meeting, and Lord s Day afternoon between 12:00pm and 1:00pm. There will not be daily DVD pickup. All remaining DVDs will be shipped by the most economical method in order to arrive one day before the start of your video training. Video-Summer 2012-OIE-1

7 V. Webcast: A. Accounts: Every church that registers to receive a video training by webcast must go to and create an account for their church. You must do this every training. Once the account has been setup, the church will not be able to enter the site until their account has been approved. Webcast accounts will be activated approximately two weeks before the live training begins. All training registration forms and donations must be received before an account will be activated. LSM will send an to the training coordinator for each church to notify them that their account has been activated and will include instructions on the use of the video training webcast. B. Languages: The messages will be available in English, Chinese, Spanish, Korean, Portuguese, Japanese, and Russian. In addition to this, we will also provide a download only file that contains Eng/Chin, Eng/Span, Eng/Kor, and Eng/Jap stereo audio tracks for churches that want to play two languages at the same time. C. Availability: The messages of the training will be available to view both live and by archive. The live meetings are at 23:00 Greenwich Mean Time (GMT) and 02:30 GMT from July 2 to July 7, The archived messages will be available at approximately 02:30 GMT and 06:00 GMT. D. Licenses: All training messages employ Digital Rights Management (DRM). The first time a church watches a message on a computer, a License Acquisition Window will be opened and the church will be prompted to enter a user name and password. They must use the same user name and password they used to log into the website. Once a message has been played, they have seven days to complete that message. The license only allows messages to be watched on a maximum of two computers. E. Study Questions: Study questions will be available online for each message with the exception of the last two messages. Outlines will not be available online. VI. Donations, name tags, and training materials: A. Donations: DVD or webcast trainings must be controlled by strict registration with a donation. The donation is one-tenth of the average individual monthly income in your country. Donation money is to be kept in a video training account for Living Stream Ministry until further instruction or authorization for use is given. Expenses directly connected with the video training may be deducted from the donations. You must submit your statement of account at the end of each training. B. Name tags: Name tags are sent out according to donations received three weeks before the training. Late applicants may be added up to two weeks before your training begins. These late donations should be sent to the LSM Registration Section before the start of your training; name tags will be either mailed or given to someone attending the training from your locality. There is one tag per seat; for shared seats type both names on one tag. C. Outlines: Training outlines are mandatory for each trainee. Training outlines are mailed out beforehand so they are received by your training dates. Video-Summer 2012-OIE-2

8 D. Control Sheets: Please find the enclosed Video Training Control Sheets helpful in your preparation for the video training registration. (These are for church use only; do not return to Living Stream Ministry.) E. Any registrant under the age of 17 must provide an elder s recommendation to the local training coordinator. II. The donation: A. The donation is the same whether full or part time. B. Those sharing a seat count as one person and give one donation. C. Those who attend the live training may attend that same local video training without an additional donation. III. Trainees may register full time or part time. They must attend all sessions for which they are registered. Those attending part time should sign up in advance for the sessions they will attend; they may attend other sessions also when they are free. IV. Shared seats must be occupied full time. V. Only those who are registered or who attended the live training may attend: A. Be in assigned seat five minutes before starting time. B. No admission to any session without a name tag. C. No unexcused absences; no more than three excused absences. D. Testing and sharing on each message. No more than three failures in testing. E. Permission is needed for leaving any session early. VI. Training Materials: A. Training outlines are mandatory for each trainee. B. All materials from this training, in both print and electronic formats, including outlines, audio materials, and video materials, must be obtained directly from Living Stream Ministry. C. We would ask the saints not to create copies of the training materials in any format or to accept copies that have been produced illegally. This applies to illegal copies for so-called personal use. 12 Online Training Registration We strongly encourage the churches to register for the semi-annual trainings using the LSM online registration system at The online registration system includes a Registration Status link on the left hand side of the page. This will show whether you are registered or not, what you have submitted, and what is still lacking in your registration. Once all necessary information is entered and training donations are received, please click on the Registration Status page to verify that the registration is complete. Once all of the necessary registration information has been received, the Registration Status page will display, Registration Complete. When you register online there is no need to mail or fax anything to LSM. When registering for a video training, please be sure to use the links on the left hand side of the page to not only indicate what type of media you will use but also to order the video training printed material. You can submit your donation online by credit card (Visa, MasterCard, Discover, and American Express are all accepted) or electronic check if you choose. Or as before, you can mail a check or call the LSM Registration Section ( ) with your credit card details. In the Forms and Reports section, you can view your completed registration list. You can proofread this report to confirm that all entries are correct. You can also view this list at any time to keep track of your changes, substitutions, cancellations, or additions.
