3 Application Process Applicant/HR,/Admin/PRO to submit the details of the educational qualification, health license and work experience, in addition to the supporting documents as per SCH requirements DataFlow will verify the received documents and information and submit the result to SCH. Application to be completed online or by downloading the Dataflow application form, filling it manually or completing the available PDF form electronically and submitting it along with supporting documents directly SCH at DataFlow desk located inside Application payment could be done online or at SCH

10 Read the instructions, fill the Details in all Sections as requested Scan and Upload the required Documents Click Submit

11 Select your preferred payment method Fill the Details in all Sections Submit payment

12 A confirmatory will be received The includes the applicant s details, DataFlow and SCH references

Last Five years of experience QAR 880 Allied Health Care Professionals/ Nurses/ Pharmacist 1. One Educational Degree 2. One Professional License 3. Last Three years of experience QAR 660 Physicians and Dentists working in Qatar for more than FIVE Years Retrospective Applications 1. Two Educational Degrees 2. One Professional License QAR 550 Allied Health Care Professionals/ Nurse / Pharmacists working in Qatar for more than THREE Years 1. One Educational Degrees 2. One Professional License QAR 550 Additional Documents Additional Verification Additional Educational Degree QAR 330 Additional Professional License QAR 110 Additional Three Year employment if applicants have more than Two Employers QAR 110 Important Information Applications must provide proof of receipt when they submit the license application. Additional QAR 35 will be charged for onsite applications The above prices and discounted packages are only applicable if documents are submitted for verification at the same time. Service Provider shall charge the applicable rate of Additional Document for any additional documents submitted post receiving the initial package of documents
