1 General Information Koraal Groep works for people with an intellectual and/or psycho-social disability in the provinces of Limburg and Noord-Brabant. Koraal Groep offers integrated care, education, work integration, observation, diagnostics and treatment according to a unique full service concept. Koraal Groep has work foundations, each with its own director, in Limburg and Noord-Brabant. The concept s added value is that the client can find support in every possible combination. If a work foundation cannot meet the client s needs, in full or otherwise, Koraal Groep will seek a solution at the group level.
2 De Hondsberg is an Observation Centre and Pedological Institute which includes a VG Child and Adolescent Psychiatry Unit. Both intramural and extramural help (Day Centres for Early Intervention, General Policlinic, Eating Disorders Policlinic) is offered according to the user s needs. The target group consists of children and young people (0-18 years) who have mild, medium or severe intellectual disabilities that include complex behavioural problems and/or psychiatric symptoms. The team uses multi-disciplinary research to arrive at an integrated diagnosis, prognosis and advice with regard to living, school or work. De Hondsberg provides specialist treatment for eating disorders, autism and social-emotional issues. T E [email protected] De La Salle is a medical pedagogy treatment institute that specialises in treating young people with mild intellectual disabilities in combination with severe behavioural and/or psychiatric problems. Parents and other family members are closely involved in the treatment where possible. The guiding treatment principles are: dialoguecentred, solution-oriented and empowerment. The institute offers ambulatory and/or after-school day treatment or 24-hour treatment. The treatment centres with treatment groups are located in Boxtel, Rosmalen, s-hertogenbosch, Boxmeer, Uden en Oss. T E [email protected]
3 Gastenhof is a medical pedagogical treatment institute for young people with mild intellectual disabilities and is recognised by the Dutch Exceptional Medical Expenses Act (AWBZ). Gastenhof offers parents help in raising their child, either at home or by temporary admission in one of its facilities. Gastenhof aims to offer young people a new perspective on the future through intensive treatment. Our guiding principle is individualised care. The treatment centres with treatment groups are located in Urmond, Simpelveld and Venlo, while the re-orientation houses are located in Bunde, Landgraaf and Maasniel. Ambulatory support consists of: day centres for day treatment and after-school treatment, intensive home treatment, polyclinic (medical pedagogy and psychiatry) and families first. T E [email protected] Maashorst is an organisation that provides youth and family support and specialist education for young children up to 12 years of age, who have normal intelligence but (severe) developmental, behavioural and intellectual difficulties as well. Maashorst offers ambulatory support, (after-school) day treatment (MKD De Kleine Cauw, MKD De Kleine Hoogen, Boddaert), treatment as part of a family group (Kinderhuis Reek), lodging and special education (LZK School De Vlinder ). Maashorst s support is aimed at the child and his or her family and environment, and is tailored to the needs of both the parents and the child. 4 Maasveld is an organisation providing support for people with an intellectual disability in the Heuvelland region in Limburg. Maasveld offers support in different areas: nursing and care, education and teaching selfreliance, stabilisation and/or treatment of people with an intellectual disability, but also support for the client s social network. The support can take various forms: living on location or living in the area (both 24-hour care), support with independent living, specialised home care, daily activities, research, advice and treatment, and leisure activities. T E [email protected] Op de Bies offers care and support for people with an intellectual disability, mainly in the Eastern part of South Limburg. Support is offered based on the client s needs as everyone has their particular strengths and limitations. Care, support and service provision should be aimed at facilitating each person s strengths and removing their limitations as far as possible. Creating opportunities and making the most of them, that is what it s all about. In order to be able to meet an individual s needs Op de Bies not only offers an extensive range of care products, but is also specifically set up to offer individually tailored support. The support can be in the area of living, working and daily activities, leisure activities and treatment and support. The support St. Anna offers is intended for permanent residents in one of our many housing accommodations between Nieuwstadt and Weert, as well as for people who live in their own homes (home support, lodging in various accommodations throughout the entire region, day care in various forms and times, including holidays, evening hours and weekends). 6 Sterk in Werk (Strong at Work) achieves valuable labour market participation for teenagers, young adults and adults with developmental disorders, learning disabilities and/ or behavioural problems. Sterk in Werk offers the following services: Work diagnosis, labour specialists, work education and training (incl. work placements), work (re-)integration (higher education bridging and reintegration programmes), job finding and job coaching. Our underlying principle is: intensive and individual care based on an integrated approach. Sterk in Werk provides the programmes - or combinations of programmes - in collaboration with the young person, parent/carer, care and education partners, and service and work organisations. Thirteen schools in Oosterhout, Breda, Rijsbergen, Roosendaal, Bergen op Zoom and Halsteren provide the education. Each location has a specific education profile. This allows the schools to tune the education to each individual student s needs as closely as possible. To this end, we work on the basic principles of providing prospective-rich education and our pupil s strengths.
