1 ANZTADC Web based anaesthetic incident recording system (WebAIRS) The Australian and New Zealand Anaesthetic Data Committee have developed a web based incident recording system called WebAIRS. The system was in the pilot phase from October and has been released for registration by fellows of ANZCA, members of the ASA and members of the NZSA since October 2010.

2 In order to register your hospital for WebAIRS follow this link The first screen requires the user to enter the randomly generated number seen in the image. The purpose of this page is to make it more difficult for automated web programs (web bots) to attempt to obtain a valid login for the site. Enter the number in the box below the image and press Submit Number. If the number in the generated image is too difficult to read then press Select another number which will generate a new random number. This is the new page that is shown where the user has the option of registering with a hospital that is already registered or if the user s hospital is not already registered then they can select the option to register a new hospital.

3 Registering as a User in a Hospital that is already registered If you select that you are a member of a hospital that is already registered you will see this screen. Complete the form by selecting your hospital from the drop down list and completing the form (all fields are mandatory). Then press Register Account. If this action is successful the above message will be displayed and you will be sent an automated to confirm that the address was correct and that ANZTADC can send messages through your hospital security system to your box.

If your program prevents you from clicking on a link then copy the link and paste it to URL box in your web browser (see above). You do this by right clicking (or equivalent on Mac) the blue confirm link and then selecting Copy Hyperlink. Then move your mouse to the area arrowed in red above, then right click (or equivalent on a Mac) and Paste, then press the enter button on your keyboard. All being well you should see this message. Once your local administrator for ANZTADC authorises your account you will be sent a username and password. This normally takes 1-3 days. If you need the password more quickly or wish to immediately enter an incident then your Local Administrator will usually be your Quality and Safety Officer or Head of Department. Ask them to authorise (so they are aware of the ).

5 Registering a New Hospital When the page opens Select I wish to register a new hospital in the ANZTADC system. Complete the form and then click Register Account. All fields are mandatory. The information will be saved and then an automated sent to you to confirm that this is a genuine request. When you receive the then click on the word in blue Confirm. This is a security measure to confirm that a genuine address has been used. When your confirmation is received by ANZTADC the application will be reviewed and then authorised. This should only take a few days in most instances. If you have problems with this link see the instructions on page 4 above. The method for confirming is the same but the messages relate to registering a hospital rather than registering as a user. When your account is authorised you will receive a template to apply for ethics exemption. Use the template to apply to your local ethics committee and request exemption on the basis that this is a quality assurance activity. You will also receive a Memorandum of Agreement that describes the protection given to this quality assurance data by the Australian and The New Zealand governments. It also describes the need to keep any downloaded data or printouts from the system confidential. This form needs to be signed on behalf of your hospital by the Director of Anaesthesia or the Medical Administration. These forms can then either be uploaded online or can be sent to Giselle Collins at ANZCA. When both forms have been received your hospital is ready to commence incident reporting.
