2 FY2013-FY2015 Community Health Needs Implementation Strategy Overview Located in Alabaster, Alabama, Shelby Baptist Medical Center is part of Baptist Health System, a faithbased, not-for-profit health care system consisting of four hospitals and a network of health centers serving north and central Alabama. With 212 beds, Shelby Baptist Medical Center is a major provider of health care services in Shelby County and the surrounding area and is dedicated to the Baptist Health System s faith-based ministry, focusing on whole-body wellness for all patients. This report includes the FY2013-FY2015 plans for Shelby Baptist Medical Center to address the prioritized health needs identified in the Community Health Needs Assessment report. The full Community Health Needs Assessment report is provided in a separate companion document. Target Areas and Population As described in the Community Health Needs Assessment report, Shelby Baptist Medical Center serves the broader Shelby County region, which includes a population of over 165,000. The majority of the population is White (77%), followed by African American (11.4%), and Hispanic (8.4%). Current estimates project an increase in number of older adults, with those aged 65+ years expected to grow 32.1% over the next five years. Identified Health Priorities The FY 2013 Community Health Needs Assessment for Shelby Baptist Medical Center was conducted using both statistical data on the current health status of the community as well as interviews from individuals who provided key insights into the broader health interests of those served by Shelby Baptist Medical Center. The resulting health needs were complied and ranked using a system that incorporated comparative methods with those health needs meeting high or medium criteria deemed as of greatest priority. Subsequently, 6 health priorities were identified as meeting high or medium criteria and are listed with a brief description below: 1. Cardiovascular Disease: Heart disease is the leading cause of death in Shelby County and health education relating to chronic disease was mentioned as a topic of interest by members of the community. 2. Health Care: Members of the community expressed concerns over access in rural areas in the community SBMC Version 2 - July 2013 Page 2 of 7
3 FY2013-FY2015 Community Health Needs Implementation Strategy 3. Maternal and Child Health: The infant mortality and low birth rates in Shelby County are slightly higher than HP 2020 goals. 4. Nutrition: Members of the community mentioned health education as it relates to health lifestyle and the relationship between nutrition and other chronic conditions were commonly mentioned by member of the community. 5. Overweight/Obesity: Obesity was discussed as a contributing factor to other chronic health condition an the importance of educational efforts to combat obesity was commonly mentioned as important to member of the community. 6. Smoking: Members of the community mentioned smoking as a health concern and the importance of effective education to reduce smoking was also discussed by members of the community. Development of Implementation Strategy Following a comprehensive review of the Community Health Needs Assessment report, the Shelby Baptist Medical Center leadership team was responsible for determining the best approach to address the identified health priorities. The resulting Implementation Strategy links specific activities to achieving targeted outcomes for each priority s specified goals and objectives. The activities in the Implementation Strategy including existing programs and partnerships as well as new activities aimed at enhancing the benefits Shelby Baptist Medical Center provides to the community. Additionally, rationale was provided for any health priorities that will not be address at this time. Implementation Strategy FY FY2015 The following Implementation Strategy details the plans for Shelby Baptist Medical Center to address the health priorities and over the next 3 years: SBMC Version 2 - July 2013 Page 3 of 7
4 FY FY2015 Community Health Needs Implementation Strategy Health Priority Goals, Objectives, Strategies, and Partnerships Activities Performance Outcomes Cardiovascular Disease To provide community education on the early signs and symptoms of cardiac distress and the need to seek immediate medical attention Rotary Club; Leadership Shelby County program Promote the adoption and training of the Early Heart Attack Care (EHAC) program to Hospital employees and promote the benefits to area health care providers Continue to promote the Baptist Health System Heart Aware program Utilize the Baptist Health Minute (TV segment) to provide information about the signs and symptoms of cardiac distress and when to seek medical care Conduct community educational sessions through the Rotary Club and Leadership Shelby County program on the importance of cardiovascular health Support the program initiatives aimed at decreasing cardiovascular disease including Smoking cessation classes, discounted memberships to Weight Watchers, enhanced access to gym/physical fitness activities, and health eating/nutritional education Number of educational sessions on cardiovascular disease and the importance of cardiovascular health Number of Baptist Health Minute segments on cardiac distress participating in Baptist Health System Workforce Wellness program screenings and activities aimed at reducing cardiovascular disease Increased awareness of efforts to increase cardiovascular screenings and prevention activities in the community Health Care To support and improve access to primary health care for uninsured and underinsured individuals in the community Community of Hope Health Clinic; Baptist Health Centers (Faith Clinic) Partner with the community Hope Health Clinic to provide primary care to uninsured residents of Shelby County Assist with the recruitment of clinic volunteers and provide continued support of clinic operations Support the establishment of the Faith Clinic which will provide primary care services for the underinsured (Medicaid) residents of Shelby County Provide ongoing support of clinic operations and promote clinic services Number of volunteers recruited to Hope Health Clinic Resources used to support Hope Health Clinic operations Educational material developed for New Faith Clinic Improved perception and access to health care services in Shelby County Maternal and Child Health To improve access to health care resources for expectant mothers and to support the improvement of childhood health Local OB/GYN physicians Utilize Shelby Baptist Medical Center physicians and medical staff to provide educational sessions on prenatal health and promote available resources for expectant mothers Partner with local OB/GYN physicians to support expanded access to OB/GYN services in the community Number of prenatal educational sessions Increased efforts to support prenatal and maternal health in the community SBMC Version 2 - July 2013 Page 4 of 7
5 Health Priority Goals, Objectives, Strategies, and Partnerships Activities Performance Outcomes Nutrition To increase community education on the importance of nutritional health and to increase access to appropriate specialize health care services Shelby County Board of Education; Shelby County Food Connect; Middle Alabama Area Agency (M4A) and the Alabama Department of Senior Services (ADSS) Support the program initiatives for Hospital employees and community partners aimed at identifying potential health risk factors and encouraging physical activity including Glucose screening, BMI testing, smoking cessation classes, discounted memberships to Weight Watchers, access to Hospital gym, and health eating/nutritional education Provide educational classes and material on the health risks associated with diabetes and diabetes prevention to high risk individuals in community Continue to provide nutritional consults for Hospital patients through Hospital dieticians Partner with the Middle Alabama Area Agency (M4A) to provide seniors (60+ yrs old) with educational material on nutrition and healthy living SBMC will not directly provide childhood nutritional programs since the Shelby County Board of Education currently provides Child Nutrition Program through local schools participating in the Baptist Health System Workforce Wellness program screenings and activities (nutrition focused) Number of educational classes on diabetes and diabetes prevention Number of senior focused nutritional educational material and resources provided to M4A Improved efforts to promote diabetes education and prevention and the importance of proper nutrition in the community Overweight/ Obesity To educate the community on the health risks associated with obesity and encourage better nutrition and more physical activity Alabama Department of Public Health, Alabama Hospital Association, and Blue Cross and Blue Shield of Alabama Support and promote the program initiatives for Hospital employees and community partners aimed at improving nutrition and health eating including - Participation in the Freshfully program which provides employees access to fresh fruits and vegetables and encourages health eating Continue to partner with the Alabama Department of Public Health, the Alabama Hospital Association, and Blue Cross and Blue Shield of Alabama to support and promote Hospital employee and community participation in the Scale Back Alabama program - Scale Back Alabama is a statewide program to encourage through weight loss and healthy living participating in Baptist Health System Workforce Wellness screenings and activities Promotional resources utilized for Scale Back Alabama Number of Hospital employees participating in Scale Back Alabama Increased community access and participation in physical fitness activity SBMC Version 2 - July 2013 Page 5 of 7
6 Health Priority Goals, Objectives, Strategies, and Partnerships Activities Performance Outcomes Smoking To enhance access to resources aimed at reducing smoking and to support to smokers who want to quit Baptist Health System; YMCA; American Cancer Association; Investigate potential opportunity to expand Faithfully Fit program into Shelby County To promote and provide smoking cessation classes and community education on the risk associated with smoking Continue to partner with the American Cancer Society on efforts to reduce smoking Support the program to provide smoking cessation education and testing for Hospital employees and community partners Support the Baptist Health System non-smoking hiring policy by screening potential employees for smoking and only hiring nonsmoking employees Investigate potential opportunities to expand the Faithfully Fit program (in partnership with local churches and the United Way of Central Alabama) in Shelby County Successful pilot of coordinated health conference for residents of Jefferson County (2012) with emphasis on promoting healthy living, smoking cessation, and tobacco prevention through targeted educational and ministry focused activities Number of smoking cessation classes participating or utilizing smoking cessation classes, resources and testing Improved awareness of smoking cessation resources available in the community SBMC Version 2 - July 2013 Page 6 of 7
7 FY2013-FY2015 Community Health Needs Implementation Strategy Ongoing Monitoring To ensure the health priorities are being addressed, the Shelby Baptist Medical Center leadership team will review and update the Implementation Strategy in an ongoing manner beginning with its formal adoption through FY2015. Updates to the plan may include revisions to existing activities as well as additional programs and partnerships that further the goals to address each health priority. The next scheduled Community Health Needs Assessment will be conducted for FY2016. Approval The Community Health Needs Assessment report and FY2013-FY2015 Implementation Strategy were reviewed and approved by the Baptist Health System Quality and Missions Committee. Any changes to the Implementation Strategy will be provided for review to the Quality and Missions Committee and incorporated into this report annually.
