22 COMMUNITY HEALTH NEEDS ASSESSMENT (CHNA) o Tanner Health System CHNA (completed in June 2013): a phased approach with both primary and secondary research Phase one: Quantitative data from existing health sources and reports. Phase two: Primary, qualitative data gathered through formal surveys, focus groups and key informant interviews from all three counties (More than 1,500 surveys were collected or returned throughout Carroll, Haralson and Heard counties between Nov and Feb ) Key issues identified through CHNA process to be addressed in Implementation Strategy (FY 2014-FY 2016): o o o Access to care Chronic disease education, prevention and management Obesity, heart disease, diabetes and cancer Mental health services
