1 Advanced Practitioner Residency Application Welcome to the Reliant Medical Group-MCPHS University Advanced Practitioner Residency program! Our program is one of the first of its kind in Massachusetts offering training in primary care for both new graduate Family Nurse Practitioners and Physician Assistants. We offer a 12-month didactic program focused on preparation of competent, confident and effective primary care providers to function both autonomously and as part of a collaborative team to provide safe, evidence-based, cost-effective health care to our patient population. Reliant Medical Group is a longstanding multi-specialty medical group practice, serving Central Mass. since Reliant has 13 free-standing primary care practices located throughout Worcester County and more than 30 medical/surgical specialties. We employ 2,100 employees, including 260 physicians and 102 advanced practitioners and serve more than 200,000 adults and children. As a Patient Centered Medical Home (PCMH), Reliant Medical Group is committed to the integration of the entire care team in order to provide the best care possible for our patients. Clinically, Reliant performs above national standards on key quality measures. As a fully integrated healthcare delivery system, Reliant provides comprehensive radiology services, a range of same-day urgent care services, as well as endoscopy, physical therapy/rehabilitation, occupational health and eye surgery programs. Our robust Care Management department assists care providers in ensuring that our patients care is wellmanaged and coordinated. MCPHS University (formerly known as Massachusetts College of Pharmacy and Health Sciences) was founded in 1823 and is Boston's oldest college or university. This accredited, private, coeducational institution prepares students for success in the ever-expanding health care industry by offering excellence in teaching, scholarship, research, professional service, and community engagement. The university currently enrolls more than 6,500 students in Boston, Worcester, and Manchester, New Hampshire.
2 Our Program Mission: To maximize the growth and development of highly qualified primary care nurse practitioners and physician assistants through the use of a structured program providing didactic lectures and trainings in a safe and nurturing environment. Vision: We will be the premier Advanced Practitioner residency program of our service area. We will achieve this through providing coordinated health care that is supportive, patient and residentcentered and team-based while building on the strengths of our faculty providers. We will recruit highly qualified professionals who desire to build on their educational experiences to become extraordinary primary care providers who will provide exceptional care to our patients. Goals: 1. Become the first Advanced Practitioner primary care residency program for both Nurse Practitioners and Physician Assistants in Massachusetts. 2. To provide access to quality care for patients in the community through the betterment of clinical skills and competencies of new graduate Nurse Practitioners and Physician Assistants. 3. Train new Nurse Practitioners and Physician Assistants to a model of primary care consistent with the goals of the Patient Centered Medical Home model. 4. Become the residency program of choice through experience and exposures to local providers who are recognized as experts in their field. Program Structure: One-year intensive didactic program including curriculum based on the core competencies developed for both nurse practitioners and physician assistants: 1. Evidence-based patient-focused care 2. Professionalism and leadership 3. Technology driven 4. Health care policy 5. Managed care Continuity Clinics (6 sessions/week) 1. Resident will assist preceptors with panel management, assuming more independence as confidence and experience improves. 2. Resident will see patients independently with formal precepting for consultations 3. Self-evaluations and preceptor evaluations 3 x year to measure clinical growth 4. Expected visit productivity will start out at 6/day and will gradually ramp up to expected goal of 18/day by completion of program.
3 Partnered Clinics (specialty rotations, 2-4 sessions/week) 1. Clinic occurs at specialty site(s) 2. Resident will both observe and participate in patient care. 3. Patients chosen by specialty preceptor and resident from preceptor s schedule 4. Self-evaluation and preceptor evaluations at end of individual preceptor rotation to measure clinical growth. Sample of Specialty Rotations Dermatology Ophthalmology Pulmonary Urology Orthopedics Rheumatology Primary care Care Management Didactic Lectures (1-2 sessions/week) Formal learning sessions provided by our own knowledgeable physicians and advanced practitioners as well as MCPHS University faculty to complement and accentuate the hands-on learning experiences from the specialty rotations. Resident Expectations The residency program is a tightly scheduled, intense program with wonderful opportunities and exposure. In order to accommodate for these opportunities, you will be scheduled to work 40 hours per week. Any scheduled time off will need to be provided in writing no less than 3 months prior to the requested time off. Residents will also be rolled into the family practice oncall rotation after approximately 3 months of participation in program. Appropriate supervision and back-up will be provided for this experience. Personal and professional growth can only happen with an open-mind and reception to constructive criticism. As such, residents will be expected to complete and review selfevaluations pre- and post-residency, post-didactic sessions, and post-specialty rotations. In addition to self-evaluations, residents will be expected to complete reflective journaling every two weeks. These evaluations and journals will allow the resident to see his/her own growth over time as well as provide invaluable feedback and insight to the program and the needs of the residents. Journals and self-evaluations will be included as part of your clinical portfolio, which you will receive upon completion of the program. Residents will also receive a Certificate of Completion sponsored by both organizations.
4 Resident Compensation and Benefits Residents will be hired as full time employees of Reliant Medical Group and as such will be eligible for a pro-rated benefits package including: Paid time off (vacation, sick time) Health, medical and dental insurance 403B retirement plan Short-term and long-term disability and life insurance License, DEA and Massachusetts Controlled Substances reimbursement benefit Student loan repayment eligibility at the completion of second year, if hired into permanent position Student Loan Forbearance-Graduates participating in this program may be eligible for student loan forbearance during their period of residency. Applicants for forbearance must submit a written request to their loan servicer. 5 How to Apply Candidates will be considered based on their educational and professional background, achievements, and a demonstration of a strong interest in primary care. Requirements: CV with 5 year work history (explain any gaps > 2-3 months) Completed application Three letters of recommendation sealed (taped with signature) 1 preceptor 1 faculty/program director with assessment of your capabilities 1 employer (supervisor or manager) Graduate school transcripts, sealed Licensed to practice in the state of Massachusetts GPA of 3.4 or higher (preferred) ANCC, AANP, or NCCPA certified DEA and MA Controlled Substance licensure Essay (maximum of 1 page per question, please): 1. What personal, professional, educational and clinical experiences have led you to choose healthcare as a profession, and the role of the NP or PA as a practice? 2. What are the goals that you are looking to accomplish during your residency at Reliant Medical Group? Please identify specific areas of interests by lifecycle, age or setting that you would like to develop increased mastery, competence or confidence in. 3.
