4 Schneider Electric Benefits Plan Schneider Electric recognizes that each employee has different needs. A healthy single person may have fewer health and dental expenses than an employee with a family. That is why Schneider Electric offers a Flexible Benefits Plan designed to meet your needs today and in the future. This plan encourages you to take ownership of your benefits decisions and select the coverage that is appropriate for you and your family. The following benefits are Company-paid: Basic Life and Accidental Death and Dismemberment (AD&D) Insurance Short-Term Disability Out-of-Province/Emergency Medical You may elect the following coverage for a cost: Health Care* Dental Care* Optional Life Insurance for you, your spouse, and/or child(ren) Long-Term Disability *Note: You may opt out of Health and/or Dental coverage as long as you can prove comparable spousal coverage. If you wish to opt out, you must contact PeopleLink within 31 days of your date of hire, or during the specified re-enrolment period. If proof of comparable coverage is not received within the specified timeframe, you will be in enroled in the default coverage detailed in this guide. Eligibility You are eligible for the benefits detailed in this document if you are: Actively working at least 20 hours each week*, Continuously employed at least as long as the waiting period, and A resident of Canada. *Note: Employees working less than 24 hours per week are not eligible for Long-Term Disability. Also, temporary employees may be exempt from all benefits and should refer to their offer agreement. If you are eligible for benefits, you may cover your spouse and/or child(ren). Please refer to the Sun Life booklet for detailed definitions of these dependent(s). Enroling in the Flex Plan If you are a new hire, you have 31 days from your date of hire to enrol online at You will be responsible for choosing your coverage status, Health and Dental options, and other optional benefits. If you do not enrol within 31 days from your date of hire, you will be automatically enroled in the default coverage shown later in this guide. During the re-enrolment period, you have the option to change your benefit elections within the specified enrolment period. If you wish to opt out of Health and/or Dental coverage, please contact PeopleLink within 31 days of your date of hire or during the specified re-enrolment period. You will also be required to complete a Proof of Comparable Coverage form and return it to PeopleLink. *Note: Re-enrolment occurs every two years. Life Event If you experience a life event during the year, such as getting married, having a baby, or spouse s change of coverage, you must contact PeopleLink within 31 days of the event. PeopleLink will open an event on for you to make updates to your benefits. Refer to the How to Enrol guide for more details on enroling in your benefits. 3
5 Health and Dental You and Schneider Electric share the cost of Health and Dental coverage. Schneider Electric will provide you with flex credits to help pay for your coverage. Health Coverage Type Option 1 Option 2 Option 3 (Default Plan) Prescription Drugs* 50% 80% 90% Pay Direct Card Supplementary Health 50% 90% 100% Semi-private Hospital 0% 90% 100% Out-of-Province/Emergency Medical Coverage Vision Care (i.e. laser surgery, prescription glasses, contact lenses) 100%; $1,000,000 lifetime maximum for you and each insured dependent 100%; $1,000,000 lifetime maximum for you and each insured dependent 0% 90% $250 maximum every 12 months for each dependent child under age 18 and every 24 months for you and all other insured dependents * Dispensing fee cap: Flex Plan pays 100% of the dispensing fee up to a maximum of $6.50 per prescription. 100%; $1,000,000 lifetime maximum for you and each insured dependent 100% $300 maximum every 12 months for each dependent child under age 18 and every 24 months for you and all other insured dependents If you choose the lowest level of coverage (Option 1), your flex credits will cover the cost of your premiums and you will have flex credits left over, which you will receive as taxable income unless you elect to transfer your remaining flex credits to: Your HSA to pay for expenses that qualify as a medical expense under the Income Tax Act The Group RRSP (refer to Page 7 for enrolment details) If you choose higher levels of coverage (Options 2 or 3), your flex credits will pay part of your premiums. You will be responsible for paying the difference through payroll deductions. Dental The Dental Plan covers Dental Care for you and your covered dependent(s). It includes preventive care like exams and cleanings. Options 2 and 3 cover more services and have a higher annual limit. Do you anticipate any unusual expenses in the coming year? Will your child(ren) need extensive dental work done or braces? Will you need massage therapy or chiropractic help? Do you have Health and Dental coverage under your spouse s plan? If yes, you are eligible to opt out of the Schneider Electric plan if you are not interested in having additional coverage. It can cover expenses that aren t covered by your group health or dental benefits. Contributions You have the option to deposit your unused flex credits into an HSA Eligible Dependent(s) You, your spouse, and any other dependant (e.g. a child, grandchild, or parent) for whom you are claiming a tax deduction in the taxation year in which the expenses were incurred Eligible Expenses Any expense that qualifies as a medical expense under the Federal Income Tax Act Plan Year January 1 December 31, 2015 Claims Deadline 60 calendar days from December 31 for prior plan year expenses Out-of-Province/Emergency Medical Coverage (Company-paid) This covers you and your eligible dependent(s) for all the services and supplies eligible under your group benefits plan, if you have a medical emergency while you re travelling outside the province where you live. *Note: If you opt out of medical and/or dental coverage, you will only have Out-of-Province/Emergency Medical Coverage. Benefit Amount $1,000,000 lifetime maximum Europ Assistance Your option choice things to consider... (cont.) If you choose to opt out, you cannot choose the Schneider Electric Health or Dental coverage again until the next re-enrolment period, unless you experience an eligible life event. If you choose to opt out, you lose the ability to claim up to 100% of your eligible Health and Dental expenses under both the Schneider Electric Flex Plan and your spouse s plan, if you currently coordinate benefits. 5
7 Disability Short-Term Disability (STD) Insurance (Company-paid) STD insurance provides income protection if you become disabled and are unable to work due to a non-work related illness or injury for up to 26 weeks. You automatically receive coverage of 66.67% of your weekly earnings. Benefit Amount Tax Status Eligibility Waiting Period Benefit Period Termination of Insurance Monthly Employee Cost 66.67% of weekly earnings.* STD income is taxable Date of hire 3 consecutive calendar days of disability, or, none if disability is due directly to an injury caused solely by accidental, violent and external means, and disability began within 30 days of the initial injury Up to a maximum of 26 weeks 70th birthday, or date of retirement, if earlier None. Assistance is available in English and French. * The E.I. maximum is equal to 3/5 of the maximum insurable earnings in force under the Employment Insurance regulations at the beginning of disability. Long-Term Disability (LTD) Insurance (Employee-paid) LTD coverage ensures a source of continuing income for disability lasting longer than 26 weeks. You are automatically enroled in LTD coverage of 66.67% of your monthly earnings up to a maximum of $11,000. You also have the option to elect LTD coverage of 60% of your monthly earnings. This benefit is Company-paid; however, it is considered taxable income for tax purposes * Indicates default plan Optional Employee/Spouse Life Insurance (Employee-paid) To enhance your Basic Life coverage, you may elect Optional Life Insurance coverage to provide an additional benefit to your beneficiary(ies) in the event of your death. You may also elect Optional Spouse and/or Child Life Insurance to provide a benefit to you if your spouse and/or child(ren) should die. *Note: Premiums for Employee/Spouse coverage are based on age and tobacco-user status. These premiums will increase January 1 of the following year if you and/or your spouse move into a new age bracket. Coverage for employee/spouse Optional Life Insurance is subject to approval by Sun Life. Optional Child Life Insurance (Employee-paid) Benefit Amount Option 1: $5,000 per dependent child Option 2: $10,000 per dependent child Monthly Employee Cost Option 1: $1.14 Option 2: $2.28 Coverage Termination Date Member s 65th birthday, or date of retirement, if earlier Retirement To be financially prepared when you leave the workforce and to be able to enjoy a comfortable retirement, you have to start planning for retirement today. Group Registered Retirement Savings Plan (Group RRSP) When you enrol in the Group RRSP, the Company makes RRSP deductions from your salary and remits them on your behalf over the course of the year to Great-West Life (the Company s RRSP provider). Your immediate tax liability is reduced, since the tax you pay is based on your salary after your RRSP contribution. By contributing through payroll deductions, rather than contributing after you are paid, your net income increases providing you with more cash throughout the year.
