2 About this Guide Life events and family status changes can affect your benefits needs. This Guide helps you understand what adjustments you can make to your JPMorgan Chase benefits to meet those needs. Overview Generally, the elections you make during the annual enrollment for health care and insurance plan coverage stay in place for the entire calendar year, unless you experience a life event or family status change. Use this Guide to: Learn what life events and situations qualify as a status change Understand the eligibility requirements to cover your dependents Determine what are allowable changes and when they become effective Consider the factors that may effect your decisions on making benefits changes Remember, any changes should be made within 31 days of the event. 4 Life Events Life events tend to require flexibility in all parts of our lives, including our benefits. The JPMorgan Chase benefits plans are ready to help you manage these changes. Qualified Events Several life events permit you to adjust your JPMorgan Chase benefit coverage. These events, known as qualified status changes, include: Marriage/Domestic Partnership/Civil Union You get married or establish a domestic partnership or civil union You get legally separated, divorced or end a domestic partnership or civil union Children You have a baby, complete an adoption, or assume guardianship Your child no longer qualifies for JPMorgan Chase benefits Family Members You or your family member loses benefits coverage under another employer s plan You or your family member gains benefits coverage under another employer s plan Your child/elder care arrangements change A family member who is covered by JPMorgan Chase benefits dies Moving You move out of your medical or dental option s service area Changing Your Benefits Status Life Events 4
5 Eligible Dependents The JPMorgan Chase Benefits Program provides dependent coverage to meet a variety of lifestyle needs. This section gives an overview of important definitions and criteria for eligible dependents. Your responsibility You are responsible for understanding and complying with the eligibility rules. You will be asked at annual benefits enrollment and any time you make benefit changes to confirm your dependent s eligibility and provide supporting documentation. You should: Review the dependent eligibility rules. Remove dependents no longer eligible for JPMorgan Chase benefits. Report any dependents experiencing a status change within 31 days of the event. Send documentation verifying your dependent s eligibility for coverage, when requested. See page 6 for more information. Definitions Dependent An eligible dependent includes a: Spouse Domestic partner Children of yours or your spouse/domestic partner up to the end of the month when they turn 26, with certain exceptions, as outlined on the next page. Spouse A spouse refers to any person to whom you are: Legally married as recognized by U.S. federal law. Same-sex marriage Under Federal regulations, same sex couples who were legally married in a jurisdiction that recognized their marriage (such as Massachusetts, California, Canada, or Spain) will be treated as married for federal tax purposes and are eligible for the JPMorgan Chase benefits plans. Domestic Partner A domestic partner requires that you and your domestic partner must meet all of the following conditions: Be age 18 or older; and Not be legally married to or the domestic partner of anyone else; and Have lived together for at least six months and have a serious, committed romantic relationship; and Changing Your Benefits Status Eligible Dependents 5
6 Domestic Partner (continued) Be financially interdependent (share responsibilities for household expenses); and Not be related to each other in a way that would prohibit legal marriage. Or Have registered as domestic partners pursuant to a domestic partnership ordinance or law of a state or local government, or under the laws of a foreign jurisdiction. Go to: My Health > 2015 Benefits Resources Children The term children includes: Your natural children, stepchildren (children of your current spouse), children of your current domestic partner (if your domestic partner is also covered by the JPMorgan Chase benefits plans) and legally adopted children. Children under your legal guardianship (as established by a court of law) whom you claim on your tax return as dependents or for whom you provide more than 50% of their financial support. Foster children placed in your care by an authorized placement agency (such as a state protective services department) or established by a signed court order. A child under age 18 who lives with you and for whom (1) adoption proceedings have already begun; and (2) you have the legal obligation to support (in whole or in part). Children who are alternate recipients under a Qualified Medical Child Support Order (QMCSO), as required by law. Qualifications Children can qualify up to the end of the month in which they turn 26, regardless of student or marital status, financial dependence on parents, residency with parents, or eligibility for coverage under another health plan. After age 26, your unmarried children may continue coverage if they are unable to support themselves due to a medical or physical disability that began prior to age 26 and depend on you fully for financial support. Complete Eligibility Verifications Promptly JPMorgan Chase reserves the right to conduct eligibility verifications on dependents at any time. Within 30 days of adding a new dependent, a blue envelope will be sent to your home address requesting materials verifying your dependent s eligibility (i.e., birth certificate, marriage license, etc.). You must supply acceptable supporting documents within 60 days. If you fail to provide satisfactory proof that your dependent(s) meet the current eligibility requirements, your dependent s coverage will be terminated retroactively to the enrollment date and you will be responsible for any claims approved and paid. 7 Timing of Your Changes Transitions can be easier when you know what to expect. Take the time to understand when changes will take effect before making decisions on your JPMorgan Chase benefits. 31-Day Rule You have 31 days to contact the accesshr Contact Center or the Benefits Web Center to report a status change for benefit changes to be effective on the date of the qualifying event (e.g., marriage, divorce, birth of a child, etc.). After 31 Days. You can still make certain benefits changes but they will be effective only after you report the status change. You will have a gap between the date of the qualified event and when your new coverage begins. Example: Your family has a new child as of January 6, 2015 WHAT HAPPENS WITHIN 31 DAYS AFTER 31 DAYS You enroll your child in JPMorgan Chase benefits February 5, 2015 February 10, 2015 Your new benefits become effective January 6, 2015 February 10, 2015 Gap in your child s benefits None January 6 - February 9, 2015 In some cases, you can cover the gap by paying for coverage on an after-tax basis. Please call or go online to report a status change as soon as possible. When Changes Take Effect Review the chart below to learn the effective dates for benefits changes, if you report your status change within 31 days. BENEFITS PLAN ALLOWABLE CHANGES Medical Dental Vision Health Spending Account Child/Elder Care Spending Account Supplemental Term Life Insurance Accidental Death and Dismemberment (AD&D) Insurance Enrollment Coverage become effective on the date of the status change if you newly enroll yourself and/or any eligible dependents. Cancellation Coverage will end on the last day of the month in which the status change occurs, if you cancel coverage for yourself and/ or any eligible dependents. Enrollment and Cancellations Enrollment and cancellations are effective on the date of the status change if you enroll yourself and/or any eligible dependents or cancel coverage. The following benefits plans allow changes only during Annual Enrollment: Long-Term Disability Plan, Group Personal Excess Liability Insurance Plan, and Group Legal Services Plan. Your Paycheck You will see changes to your payroll deductions (including any amounts owed) on the next available pay following your reported status change. 8 Marriage, Domestic Partnership, and Civil Union Changes to your marital and partnership status can significantly affect the benefits coverage that you require. Luckily, when these types of changes occur, you can adjust your JPMorgan Chase benefits to meet your needs. The 31-Day Rule You have 31 days to contact the accesshr Contact Center or the Benefits Web Center to report a status change for benefit changes to be effective on the date of the qualifying event (e.g., marriage, legal separation, divorce, etc.). Beyond 31 days, you can still make certain benefits changes but they will generally be effective only after you report the status change online or by phone. See page 7 for details. You get married or enter a domestic partnership or civil union. Allowable Changes A new marriage or partnership may prompt you and your spouse/domestic partner to compare your options and reassess your needs. The changes you can make to your JPMorgan Chase benefits are listed below. NOTE: JPMorgan Chase is pleased to be able to recognize your marriage, regardless of the gender of your spouse, but you may live in a state that does not recognize same-sex marriage. In this case, the firm must continue to impute income for state tax purposes on the value of the benefits provided to your domestic partner but will gross up the imputed income value. 9 Considerations As you think about your benefits needs and options, ask yourself these questions: Medical, Dental, and Vision Plans Does your spouse/domestic partner have other health care or insurance coverage available? Do the JPMorgan Chase Medical, Dental, and Vision Plans meet your family s needs? Do your dependents meet the eligibility requirements? (Read Eligible Dependents on page 5 in this Guide.) If you choose to cover your spouse/domestic partner, remind him or her to complete the Wellness Screening and Wellness Assessment during the required timeframes (go to My Health for more information) Does your spouse/domestic partner qualify as a tobacco user under the Medical Plan? Health Care and Child/Elder Care Spending Accounts Do you expect your spouse/domestic partner will have additional expenses which can be paid by the Health Care Spending Account? Will you have more child or elder care expenses? Will your spouse/domestic partner stay at home to reduce these costs? (For purposes of these Spending Accounts, your domestic partner must be a qualified tax dependent for his or her expenses to be covered.) Life and Accident Insurance Does your insurance coverage provide adequate financial support in case of an accident or death? Other To Do s Eligibility Verification. Respond to the request for documentation showing that your new dependent is eligible for the benefits plans. For more information, see page 6. Personal Information. Update your name, address, marital status, and contact information via My Personal Profile. Go to: mpp.jpmorganchase.com Taxes. Think about who you want as beneficiaries for your life insurance plans as well as your 401(k) Savings and Retirement Plans. 10 You get legally separated, divorced or end a domestic partner relationship or civil union. Allowable Changes When a marriage or partnership ends, it s important to consider your benefits needs and identify which dependents (if any) remain eligible for benefits. The changes you can make to your JPMorgan Chase benefits are listed below. BENEFITS PLAN Medical (including prescription drug coverage) Dental Vision Health Care Spending Account Child/Elder Care Spending Account Supplemental Term Life Insurance Accidental Death & Dismemberment (AD&D) Insurance CHANGES YOU CAN MAKE Drop coverage for your spouse/domestic partner Drop coverage for your dependent children Decrease or stop contributions Start, change, or stop contributions Add, change, or cancel coverage* * Coverage for your former spouse/domestic partner automatically ends as of the date of the legal separation or divorce. Coverage for eligible dependent children continues until you elect otherwise. Continuation Coverage Your former dependents may continue certain JPMorgan Chase benefits coverage at their own cost if you notify the accesshr Contact Center and remove them from your coverage within 60 days of your divorce or the end of your domestic partnership. REMEMBER: If you wish to make changes to your JPMorgan Chase benefits coverage, you must take action within 31 days of a status change. They can continue Medical, Dental, and Vision coverage. Former spouses and dependent children can get a temporary extension of these JPMorgan Chase benefits through COBRA, the Consolidated Omnibus Reconciliation Act of Domestic partners can enroll in a similar continuation coverage program provided by JPMorgan Chase. For more information, contact: accesshr Contact Center They can continue Supplemental Term Life or AD&D Insurance. Your former spouse/domestic partner can port this coverage at his or her own cost, which means they can keep the same terms of the policy regardless of any pre-existing health condition. 11 Considerations Weighing all of your options may seem overwhelming. To get started, ask yourself these questions: Medical, Dental, and Vision Plans Will you or your dependent children lose health care coverage as a result of this event? Health Care Spending and Child/Elder Care Spending Account Do you no longer have dependent health expenses to cover? Will you need to pay for day care services for your children or elderly dependents so you can continue working? Life and Accident Insurance Now that you are covering fewer dependents, are you carrying too much coverage? Other To Do s Personal Information. 12 Children When you have a new child or when children no longer qualify for benefits, you need to take action to ensure your family has the right coverage under the JPMorgan Chase benefits plans. The 31-Day Rule You have 31 days to contact the accesshr Contact Center or the Benefits Web Center to report a status change for benefit changes to be effective on the date of the qualifying event (e.g., marriage, legal separation, divorce, etc.). Beyond 31 days, you can still make certain benefits changes but they will generally be effective only after you report the status change online or by phone. See page 7 for details. You have a baby, complete an adoption, or assume guardianship. Allowable Changes You must enroll your new child in order to cover them under your benefits, even if you re already have family coverage in JPMorgan Chase benefits plans. You can also make any of the changes listed below. Health Care and Child/Elder Care Spending Accounts Do you expect your family will have more expenses for the Health Care Spending Account? Will you have more child care expenses going forward? Life and Accident Insurance Does your insurance coverage provide adequate financial support in case of an accident or death? Other To Do s Eligibility Verification. Respond to the request for documentation showing that your new dependent is eligible for the benefits plans. For more information, see page 6. Taxes. Adjust your withholding allowances and/or marital status for federal, state, and/or local taxes, as needed via My Personal Profile. Go to: mpp.jpmorganchase.com Beneficiaries. Think about who you want to designate as a beneficiary for your life and accident insurance plans as well as the 401(k) Savings and Retirement Plans. Allowable Changes When a dependent child no longer meets the eligibility requirements for JPMorgan Chase benefits due to age, his or her medical, vision, and dental coverage ends automatically at the end of the month. You can make changes to your other benefits, as listed below. 14 Continuation Coverage When your child no longer qualifies for benefits, he or she can continue certain JPMorgan Chase benefits coverage at their own costs if you notify the accesshr Contact Center and remove them from your coverage within 60 days this change in status. REMEMBER: If you wish to make changes to your JPMorgan Chase benefits coverage, you must take action within 31 days of a status change. They can continue Medical, Dental, and Vision coverage. Former dependent children can get a limited extension of these JPMorgan Chase benefits through COBRA, the Consolidated Omnibus Reconciliation Act of Your child will receive COBRA enrollment materials 30 days before his or her 26th birthday. Your child can continue this coverage by converting their coverage to an individual policy within 31 days of his or her status change. 15 Family When a dependent family member loses a job, changes careers, or chooses a new direction, the transition can affect your benefits coverage. In these situations, you are able to make changes to your JPMorgan Chase benefits coverage. The 31-Day Rule You have 31 days to contact the accesshr Contact Center or the Benefits Web Center to report a status change for benefit changes to be effective on the date of the qualifying event (e.g., marriage, legal separation, divorce, etc.). Beyond 31 days, you can still make certain benefits changes but they will generally be effective only after you report the status change online or by phone. See page 7 for details. You or your eligible dependent loses benefits coverage in another plan. Allowable Changes Your spouse/domestic partner or eligible dependents may lose a job or take a part-time position without benefits. If they lose coverage, you can change your JPMorgan Chase benefits for you and your dependents, as outlined in the chart below. 17 Your child/elder care arrangements change Allowable Changes You may find it necessary to change who cares for your children or elderly family members (including if a family member can no longer serve as a care provider) or you may experience a significant change to your child/elder care costs. In these situations, you can make changes to your Child/Elder Care Spending Account. BENEFITS PLAN Child/Elder Care Spending Account CHANGES YOU CAN MAKE Start or change contributions Considerations How much you contribute to your Child/Elder Care Spending Account should be consistent your provider costs. You should also consider the following: Will you need child/elder care for the rest of the year? Do you expect to increase or decrease your care costs this year? A dependent family member dies Allowable Changes The following chart summarizes the changes you can make to your JPMorgan Chase benefits coverage upon the death of a dependent family member. Health Care Spending and Child/Elder Care Spending Account Will your family have fewer eligible expenses under the Health Care Spending Account? Will you have more child or elder care expenses? Life and Accident Insurance Are you carrying too much or too little insurance coverage for your needs? Other To Do s Taxes. Update your withholding allowances for federal, state, and/or local taxes, as needed via My Personal Profile. Go to: mpp.jpmorganchase.com Beneficiaries. Review your beneficiary information for the life amd accident insurance plans as well as the 401(k) Savings and Retirement Plans. To make changes online, go to: beneficiary. jpmorganchase.com Documentation. Request 10 or more copies of the official death certificate for insurance claims and settling the estate. Claims. File any insurance claims. Contact the accesshr Contact Center if your deceased dependent was covered by the firm s life and accident insurance plans. Notifications. Notify relevant organizations with updated information for your deceased family member, including the Motor Vehicle Department, Social Security Administration, Voter Registration, financial institutions, and any outstanding payees. 19 Moving Moving involves many changes and adjustments.
