1 JOB DESCRIPTION #17.1 TITLE: Licensed Practical Nurse FUNCTIONAL TITLE: LPN JOB CATEGORY: Nursing REPORTS TO: Clinical Leader/ Nursing Supervisor BASIC FUNCTIONS: A licensed practical nurse bears full responsibility for the quality of health care he/she provides to the residents at Seven Hills at Groton. A licensed practical nurse may delegate nursing activities to assigned health care personnel. The delegating licensed practical nurse shall bear full responsibility for making an appropriate assignment, adequately teaching, directing and supervising the delegate(s), as well as the outcome of that delegation; all within the parameters of his/her generic and continuing education and experience. Provides quality care to children and young adults in accordance of the Nurse Practice Act of Massachusetts. QUALIFICATIONS: Licensure to practice as an LPN in Massachusetts is required. Prefer experience as a Certified Nursing Assistant if new graduate. SCHEDULE: As determined by Director of Nursing upon hiring. RESPONSIBLILITEIS AND OBJECTIVES: The following description of duties is intended to outline the essential responsibilities of the person in this position. It is not intended to be an exhaustive list of job duties and responsibilities. Delivers competent nursing care Conveys through nursing practice a clear sense of the role and identity. Contributes to individualized plan of care in which needs are identified and achievable goals are set. Implements nursing care plan in an organized manner; establishes and reorders priorities as needed. Evaluates outcomes of basic nursing interventions. Incorporates the prescribed medical regimen into the nursing plan of care. Deliver care that is consistently reflective of the standards of care at Seven Hills at Groton. Administers medications and treatments safely according to established nursing practice.
2 Page 2 Systematically assesses the resident s basic health status, records and related health data. Recognizes changes or trends in patient status. Participates in analyzing and interpreting data and making informed judgments as to the specific elements of nursing care, using appropriate resources and NP/MD consultation. Participate in the health teaching required by the family and other responsible parties. Collaborate, cooperate and communicate with other health care providers to ensure quality and continuity of care. Delegates responsibilities for the performance of selected resident care activities to assistive personnel within the established delegation framework and provides appropriate supervision. Ensure that all nursing personnel assigned to you comply with the written policies and procedures established by this facility. Meet with your assigned nursing staff, as well as support personnel, in planning the shift s services, programs and activities. Ensure that your assigned certified nursing assistants (C.N.A. s) are aware of the resident care plans. Ensure that the C.N.A. s refer to the resident s care plan prior to administering daily care to the resident. Make a written and oral report/recommendation concerning the activities of you shift as required. Admit, transfer and discharge residents as required. Complete accident/incident reports as necessary. Appropriately notify/inform supervisory staff. Perform administrative duties such as completing 30 day summaries and edits as necessary. Encourage attending physicians to review treatment plans, record and sign their orders, progress notes, etc., in accordance with established policies. Receive telephone orders from physicians and record on the Physician s Order Form. Accurately transcribe physician s orders to resident charts, Kardex, medication cards, treatment/care plans, as required. Chart nurse s notes in an informative and descriptive manner that reflects the care provided to the resident, as well as the resident s response to the care. Report all discrepancies noted concerning physician s orders, diet, charting error, etc., to the Nursing Supervisor. Sign and date all entries made in the resident s medical record.
3 Page 3 Ensure that an adequate supply of medications is on hand to meet the nursing needs of the residents. Properly order medications in a timely fashion per pharmacy guidelines. Ensure that the narcotic records are accurate for your shift. Notify the Nurse Supervisor of all drug and narcotic discrepancies noted on your shift. Review MAR for completeness of information, accuracy in the transcription of the physician s order and necessary initials to indicate doses administered. Dispose of drugs and narcotics as required and in accordance with established procedures. Responds to complaints and grievances made by family or responsible parties. Report to nursing supervisor as necessary. Follow Seven Hills at Groton grievance policy. Participate in the orientation of new residents/family members to the facility. Make rounds with physicians and actively participate in planning care for assigned Residents. Arrange for diagnostic and therapeutic services, as ordered by the physician, and in accordance with our established procedures. Consult with the resident s physician in providing the resident s care, treatment, etc., a necessary. Notify the resident s attending physician when the resident is involved in an accident or incident. Review the resident s chart for specific treatments, medication orders, diets, etc., routinely to stay adequately informed of resident s status. Notify the resident s attending physician and next-of-kin when there is a change in the resident s condition. Inspect the nursing service treatment areas daily to ensure that they are maintained in a clean and safe manner. Administer skilled nursing intervention such as, catheterization, tube feedings, suction, trach care, stoma care, applying and changing dressings/ bandages, packs and range of motion exercises, care for the dead and dying, etc., as required. Use restraints when necessary and in accordance with established policies and procedures. Obtain sputum, urine and other specimens for lab tests as ordered. Assure prompt follow-up of all lab studies and notification of MD as appropriate. Take and record TPR s, blood pressures, etc., as necessary and interpret results. Collect data from nursing assistants and interpret. Provide care for seriously ill residents as necessary.
4 Page 4 Order, per guidelines and/or recommend to the Nursing Supervisor, the equipment and supply needs of your unit/shift or assigned resident. Assist in arranging transportation for residents as necessary. Operate nursing service equipment in a safe manner. Obtain appropriate orientation to all new equipment. Utilize nursing service supplies in a cost efficient manner to avoid waste. Interact with residents and their families in a compassionate, nonjudgmental and humanistic manner that preserve and protects resident and family autonomy, dignity and rights. Develop an awareness of the therapeutic relationship. Appropriately inform family members and/or responsible parties of any changes in their child s condition or plan of care. Contact them in a timely fashion. Appropriately notify Social Services of any needs of the parent and/or responsible party. Demonstrate a willingness to be a team member and to work collaboratively. Direct staff to achieve resident care needs. Make clinical decisions which reflect the ability to prioritize and utilize colleagues in achieving desired goals. Build and maintain collegial environment that supports positive morale. Request assistance when appropriate and understands limits of ability and/or role. Share own knowledge with colleagues. Promote growth of C.N.A. staff. EXPECTATIONS: Maintain a safe and clean environment, utilizing supplies and equipment in a cost effective and efficacious manner. To carry out all responsibilities professionally and efficiently. To work in harmony and in support of the Seven Hills Foundation operating subsidiaries Assume responsibility for own self evaluation and learning. Formulate goals and objectives for identified areas of growth; seeks opportunities to meet own learning needs. Outlines goals annually in performance reviews. Attend staff meetings and in-service education sessions.
5 Page 5 Attend continuing education programs and shares information with colleagues. Participates in performance improvement initiatives as directed by Quality Assurance Coordination, Clinical Leader and DON. Comply with all regulations that impact long-term care. Stay well informed of regulatory issues that directly impact nursing practice at Seven Hills at Groton. PHYSICAL REQUIREMENTS: Work requires frequent lifting and carrying residents/objects up to 10 pounds and regularly lifting and carrying residents/objects pounds. Work requires frequent stooping and bending. Work requires frequent reaching and grasping objects at, above and below shoulder level. Work requires regular pushing and/or pulling non-motorized equipment (e.g.medication carts, beds, etc.) weighing 21 to over 40 pounds. Must be able to speak and write the English language in an understandable manner. Must be able to see and hear or use prosthetics that will enable these senses to function adequately to ensure that the requirements of this position can be fully met. Must be able to cope with the emotional and mental stresses of the position. Must be able to function independently, have flexibility and personal integrity. Must be able to work with the ill, disabled, emotionally upset residents, their families and staff. Must be able to move sufficiently to respond to life threatening emergencies, including performing CPR and assisting in the evacuation of residents during emergency situations. Repetitive hand and wrist movement as when crushing and administering medications, pouring enteral feedings, etc. STATUS: Non-exempt.
