1 CERTIFIED NURSING ASSISTANT Job Summary and Performance Criteria (See full job description for physical demands) Department Assigned: Nursing Supervisor Title: Registered Nurse/Licensed Practical Nurse Purpose of Your Job Position The primary purpose of your job position is to provide each of your assigned residents with routine daily nursing care and services in accordance with the resident s assessment and care plan, and as may be directed by your supervisor. Delegation of Authority As a Certified Nursing Assistant, you are delegated the administrative authority, responsibility and accountability necessary for carrying out your assigned duties. Job Functions Every effort has been made to identify the essential functions of this position. However, it in no way states or implies that these are the only duties you will be required to perform. The omission of specific statements of duties does not exclude them from the position if the work is similar, related or is an essential function of the position. Competency Comments Miscellaneous Information Evaluation Information explaining the Competency Evaluation columns, as well as the Risk Exposure Category legend is provided in the Job Position Analysis Information section located on the last pages of this job description. (Note: The number assigned to each individual column corresponds to that same number in the Analysis section). Duties and Responsibilities Administrative Functions Risk Exposure Potential to Blood and/or Body Fluids. Satisfactorily ( = YES) Exceeds Expectations in performing function ( = YES) Does not perform function satisfactorily. (1) (2) () (4) Identify residents before administering treatments, serving meals, etc., as necessary. Record entries on flow sheets, notes, charts, etc., in an informative and descriptive manner. Use authorized abbreviations when recording information. Report changes in the resident s condition to your supervisor. Report accidents and incidents to your supervisor on the shift that they occur. Report for work as scheduled. Notify your supervisor per policy if you will be late/absent for your scheduled shift. Admission, Transfer and Discharge Functions Ensure that the resident s room is ready for receiving the resident (i.e. bed made, name tags up, etc.), greet resident and make comfortable. Introduce resident to roommate, if any, other residents and staff as appropriate. Inventory and mark the resident s personal possessions and clothing as instructed. Assist residents with packing their personal possessions when they are being transferred to a new room or when being discharged.

Satisfactorily ( = YES) Exceeds Expectations in performing function ( = YES) (1) (2) () (4) Does not perform function satisfactorily Follow work assignments and/or work schedules in completing and performing your assigned tasks. Cooperate with inter-departmental staff as well as other facility staff to ensure that nursing services can be adequately maintained to meet the needs of the residents. Create and maintain an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment throughout the department and shift. Report complaints made by the resident to your supervisor. Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals to your supervisor. Personal Nursing Care Functions Participate in and receive the nursing report upon reporting for duty. Assist residents with daily dental and mouth care (i.e. brushing 2 teeth/dentures, oral hygiene, special mouth care, etc.). Assist residents with bath functions (i.e. bed bath, tub or shower bath, etc.) 2 as directed. Assist residents with dressing/undressing as necessary. 2 Assist residents with hair care functions. Assist residents with nail care (i.e. clipping, trimming, and cleaning the 2 finger/toenails). NOTE: Does not include diabetic residents. Keep residents clean shaven. 2 Keep residents clean and dry (i.e., change gowns, clothing, linen, etc., 1 when it becomes wet or soiled). Make beds and change bed linens. Keep linens tight to avoid wrinkles 2 from forming under the resident. Position residents in correct and comfortable position. 2 Assist resident with bowel and bladder functions. Report bowel 1 movements and character of stools as instructed. Maintain intake and output records as instructed. Collect specimens as instructed (i.e., urine, sputum, stools, etc.). 1 Assist residents in preparing for medical tests (i.e., lab work, x-ray, 2 therapy, dental, etc.) and examinations. Assist with lifting, turning, moving, positioning and transporting residents 2 into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc. Assist residents to walk with or without self-help devices as instructed. Perform restorative and rehabilitative procedures as instructed. 2 Assist in preparing the resident for a physical examination. 2 Weight and measure residents as instructed. Measure and record temperature, pulse, and respiration (TPR s), as 2 instructed.

Ensure that residents who are unable to call for help are checked frequently. Check residents routinely to ensure that personal care needs are being met in accordance with wishes. Assist with the care of the dying resident. Provide post-mortem care as 2 instructed. Observe and report the presence of pressure areas and skin breakdowns to 2 prevent decubitus ulcers (bedsores). Report injuries of an unknown source, including skin tears. Provide daily catheter care. 1 Provide daily perineal care. 1 Provide daily Range of Motion Exercises. Record data as instructed. 2 Reposition residents as necessary. 2 Turn medications found in the resident s room/possession over to your supervisor. Provide residents with validation orientation as instructed. Watch for and report changes in room temperature, ventilation, lighting, etc. Food Service Functions Prepare residents for meals (i.e., take to bathroom, wash hands, comb hair, 2 raise bed, position tables, place bibs, take to/from dining room, etc.). Assist with feeding as indicated (i.e., cutting foods, feeding, assist in 2 dining room supervision, etc.). Assist residents with identifying food arrangements (i.e., informing resident with sight problem of foods that are on tray, where it is located, if it is hot/cold, etc.). Record the resident s food/fluid intake. Report changes in the resident s eating habits to your supervisor. Keep residents water pitchers clean and filled with fresh water and within easy reach of the resident. Serve between meal and bedtime snacks. Perform after meal care (i.e., remove trays, clean resident s hands, face, 2 clothing, take to bathroom, brush teeth, clean dentures, etc.). Staff Development Attend and participate in scheduled orientation, in-service training and educational classes to maintain current certification as a Nursing Assistant. Safety and Sanitation Functions Wash hands before and after performing services for the resident. Keep the nurses call system within easy reach of the resident. Immediately notify your supervisor of any resident leaving/missing from the facility. Follow established safety precautions in performing duties.

Clean up spill immediately. 2 Keep resident personal possessions and excess supplies and equipment off the floor. Store in designated areas. Wash wheelchairs, walkers, etc., as instructed. 2 Clean, disinfect and return resident care equipment to its designated 2 storage area after each use. Report hazardous conditions and equipment to your supervisor. Report safety violations. Follow established smoking regulations. Report communicable or infectious diseases to the Director of Nursing. Follow established isolation precautions and procedures. Follow established procedures in the use and disposal of personal 2 protective equipment. Use safety equipment and supplies (e.g., gait belts, mechanical lifts, etc.) 2 when lifting or moving residents. Report missing/illegible labels and MSDS s to your supervisor. Equipment and Supply Functions Use only the equipment you have been trained to use and operate in a safe manner. Use only the equipment and supplies necessary to do the job. Report defective equipment to your supervisor. Inform your supervisor of your equipment and supply needs. Care Plan Functions Review care plans to determine if changes in the resident s daily care routine have been made on the care plan. Inform your supervisor of any changes in the resident s condition so that appropriate information can be entered on the resident s care plan. Ensure that your notes reflect that the care plan is being followed. Resident Rights Maintain the confidentiality of resident care information. Ensure that you treat residents with kindness, dignity and respect. Ensure that nursing care is provided in privacy. Knock before entering the resident s room. Report complaints made by the resident to your supervisor. Report allegations of resident abuse or neglect or misappropriation of resident property to your supervisor. Honor the resident s refusal of treatment request. Report such requests to your supervisor.
5 CERTIFIED NURSING ASSISTANT WORKING CONDITIONS Page 5 Works throughout the nursing service area (i.e., drug rooms, nurses stations, resident rooms, etc.). Moves intermittently during working hours. Is subject to frequent interruptions. Is involved with residents, staff, visitors, government agencies/personnel, etc., under all conditions and circumstances. Is subject to emotionally upset residents, family members, staff and visitors. Communicates with the nursing staff and other department staff. Works beyond normal working hours, on weekends and holidays, and in other positions temporarily, when necessary. Is subject to call-back during emergency conditions (e.g., severe weather, evacuation, post-disaster, etc.). Attends and participates in continuing educational programs. Is subject to injury from falls, burns from equipment, odors, etc., throughout the workday, as well as to reactions from dust, disinfectants, tobacco smoke, and other air contaminants. Is subject to exposure to infectious waste, diseases, conditions, etc., including TB and the AIDS and hepatitis B viruses. May be subject to the handling of and exposure to hazardous chemicals. EDUCATION Desired A high school diploma or equivalent. Must be a licensed Certified Nursing Assistant in accordance with the laws of this state. EXPERIENCE Desired 6 months experience in long-term care. SPECIFIC REQUIREMENTS Must be able to read, write, speak and understand the English language. Must possess the ability to make independent decisions when circumstances warrant such action. Must possess the ability to deal tactfully with staff, residents, family members, visitors, government agencies/personnel and the general public. Must possess the ability and willingness to work harmoniously with other staff. Must have patience, tact, cheerful disposition and enthusiasm as well as be willing to handle residents based on whatever maturity level they are currently functioning. Must be able to relate information concerning a resident s condition. Must not pose a direct threat to the health or safety of other individuals in the workplace. PHYSICAL AND SENSORY REQUIREMENTS (With or Without the Aid of Mechanical Devices) Must be able to move intermittently throughout the workday. Must be able to speak and write the English language in an understandable manner. Must be able to cope with the mental and emotional stress of the position. Must possess sight/hearing senses or use prosthetics that will enable these senses to function adequately so that the requirements of this position can be fully met. Must function independently, have flexibility, personal integrity and the ability to work effectively with the residents, staff and support agencies. Must meet the general health requirements set forth by the policies of this facility that may include a medical and physical examination. Must be able to relate to and work with ill, disabled, elderly, emotionally upset people within the facility. Certain tasks in this job description may require you to push, pull, move, and/or lift up to 50 pounds to a minimum height of feet and be able to push, pull, move and/or carry such weight a minimum distance of 10 feet. May be necessary to assist in the evacuation of residents during emergency situations.

The following numbers indicate your risk potential: 1 = It is highly likely that while performing functions assigned to this task you will be exposed to blood or body fluids. 2 = This task does not involve contact with blood and/or body fluids but while performing this task it may be necessary for you to perform a Category 1 task. = This task does not involve any risk of exposure to blood or body fluids. Competency Evaluation Columns: 2. A mark in this column indicates that you perform function satisfactorily and in accordance with the facility s policies and procedures.. A mark in this column indicates that you perform this function in a manner that exceeds expectations. 4.
