3 SOUTH PIEDMONT COMMUNITY COLLEGE HEALTH PROGRAMS ADMISSIONS REQUIREMENTS Below are the general admissions requirements for all health programs followed by the program specific requirements. Please keep in mind that admission to all health programs is a two-step process. Step 1: Complete your SPCC Admissions File 1. Complete the SPCC Application for Admission Submit an application for admission online at: If you already have an account with CFNC, use your login information to fill out SPCC s application. If you do not have an account with CFNC, you may create one prior to filling out the application. You may apply to only one health program. 2. Submit official high school, GED, or adult high school transcript It is your responsibility to request transcripts and pay any applicable fees. Official transcripts should be mailed directly from the high school, state GED office, or adult high school to SPCC. Unofficial (student-issued/faxed/ ed) transcripts will not be accepted. 3. Submit official transcripts from all colleges listed on your application A Request for Transcript Evaluation Form must be completed and sent to the Registrar for evaluation of prior course transfer credit. Credits over 10 years can be appealed through the Transfer Credit Appeal process. All foreign transcripts (secondary and post-secondary) must be evaluated by an authorized translation company at the student s expense. 4. Prepare for and take the College Placement Test (CPT) You have one opportunity to take the CPT (Reading, Sentence Skills, Math Diagnostic, and Computer Competency). This initial test is free. CPT scores are valid for two years. Reading, Sentence Skills, Computer Competency, Arithmetic, and Algebra are good for two years. Math diagnostic is good for five years. Note: All applicants to the EMS Bridge program must achieve the minimum cut score for the Emergency Medical Science program. However, if an applicant has completed college level English and Math classes with a final grade(s) of C or higher or is able to complete the required developmental educations class(es) prior to the application deadline and has earned a final grade (s) of C or higher, the CPT requirement may be waived. Accuplacer scores from other colleges may be accepted if taken within the last two years for Reading, Sentence Skills, Computer Competency, Arithmetic, and Algebra, and five years for the Math diagnostic. Official score report must be sent from original institution to SPCC. Applicants with the following scores taken within the last two years are exempt from taking Accuplacer Reading and Sentence Skills: 3
4 SAT ACT Asset Compass Reading Writing/English Applicants with the following scores taken within the last two years are exempt from taking Accuplacer Math diagnostic: SAT ACT Asset Compass Math Numerical/Pre-Algebra 41 and 47 and Intermediate Algebra/Algebra SPCC must receive an official score report from ACT, the College Board for SAT, or from the college for Accuplacer, Asset and Compass. The Asset and Compass are products of ACT and Accuplacer is a product of College Board. 5. Complete the New Student Orientation online View the online orientation video. The video is available at: It does require Flash Player. At the end you will be asked to answer a few questions correctly so make sure you are paying attention. 6. Apply for Financial Aid Complete the Free Application for Federal Student Aid (FAFSA) and application process. Refer to the website for further information and application deadlines. Step 2: EMS Bridge Program Admission Requirements 1. Meet College Placement Test (CPT) scores or completion of appropriate developmental coursework. Note: All applicants to the EMS Bridge program must achieve the CPT minimum cut score for the Emergency Medical Science program. However, if an applicant has completed college level English and Math classes with a final grade(s) of C or higher or is able to complete the required developmental educations class(es) prior to the application deadline and has earned a final grade (s) of C or higher, the CPT requirement may be waived. 2. Students wishing to take the EMS Bridging courses must first complete the EMS Bridging Crosswalk. All coursework and certifications need to be completed through an accredited institution. An official transcript needs to be submitted for all coursework and certifications. (See page 13 for Non-Credit to Credit form). Successful completion of the requirements for the EMS Crosswalk, including but OR National Registry paramedic certification and affiliation with an Emergency Medical Provider. *Other state s certification may be approved; determined on a state-by-state basis. Note: Paramedic certification must be maintained throughout the entire length of the EMS Bridge program. o Proof of certification in Basic Trauma Life Support (BTLS) or (PHTLS), Advanced Trauma Life Support (ACLS), and Pediatric Advanced Life Support (PALS). o Current CPR certification, (Health Care Provider) to include first aid, child, infant, and adult CPR. This course is available through SPCC s Career Development & 4
5 o Continuing Education Division, at Career Development & Continuing Education, Medical Professions, CPR & First Aid Certification. Provide written proof of a minimum of 3000 paid hours as a paramedic. Note: Can also include 500 hours from initial paramedic education as shown thru official transcripts within the last five (5) years. 3. Take the TEAS V test and submit a copy of your TEAS V score with your packet. TEAS V scores are valid for two years prior to the program deadline. Contact the Testing Center at the LLP or OCH campus to set up an appointment for this test. (See page 15 for Testing Center Permission Form TEAS.) TEAS scores not taken at SPCC must be officially transferred from ATI Testing and must be the same version administered at SPCC (TEAS V). 4. All course work must be at least a C ; and your program GPA must be at least a 3.0 to be considered for admission. NOTE: All required general education and previous EMS courses must be taken at SPCC. 6 SOUTH PIEDMONT COMMUNITY COLLEGE EMS BRIDGE PROGRAM Application Period for Fall Semester 2014 Ends July 15, 2014 Application Period for Spring Semester 2015 Ends November 30, 2014 How do I apply for admission to the EMS Bridge program? Complete Step 1 and 2 listed on pages 3, 4, and 5. When you have completed both steps, collect only the required documents listed below and put them into a 9 x12 envelope with your full name, SPCC ID, and program printed clearly in the upper right corner. Your packet should only include: EMS Program Application pages 11 and 12. Proof of current North Carolina* OR National Registry paramedic certification and affiliation with an Emergency Medical Provider. (*Other state s certification may be approved; determined on a state-by-state basis.) Proof of certification in Basic Trauma Life Support (BTLS) or (PHTLS), Advanced Trauma Life Support (ACLS), and Pediatric Advanced Life Support (PALS). Copy of completed Crosswalk (Non-credit to credit) form. (page 13) Proof of a minimum of 3000 paid hours as a paramedic. Copy of current CPR card. Copy of TEAS V test results. When are students admitted into the EMS Bridge program? Health programs at SPCC have limited enrollment and a competitive admissions process. Students are admitted into the EMS Bridge program beginning fall and spring semesters each year. Students who submit a complete application packet by the July 15 deadline will be given priority consideration for admission for the fall 2014 semester class. Students who submit a complete application packet by the November 30 deadline will be given priority consideration for admission for the spring 2015 semester class. NOTE: If the number of qualified applicants meeting these deadlines is not sufficient to fill all program openings, complete application packets will continue to be accepted until the maximum enrollment number is met. What do I need to do to be considered for entry into the EMS Bridge program? To be considered for admission into the EMS Bridge program, you must complete the requirements listed on pages 3, 4 and 5 and submit a complete admission packet by the deadline. What are the required CPT scores for the EMS Bridge program? Combined Reading and Sentence Skills score 166 or higher Computer Skills: 80 Arithmetic: 55 (MAT 060) Algebra: 75 (MAT 070 and MAT 080) OR Mastery in DMA 010 through DMA 080 6
7 If you do not meet these required CPT scores, you must enroll in and complete the appropriate developmental course(s) prior to the program application deadline. Recent SAT, ACT, ASSET or COMPASS scores may be used in place of some of these requirements. What is a TEAS test? The TEAS V (Test of Essential Academic Skills) measures basic essential academic skills related to success in Allied Health programs in English, reading, mathematics, and science. For more information on the TEAS test, go to TEAS study guides are available at SPCC bookstores for purchase. How do I take the TEAS test? Contact the SPCC Testing Center for a TEAS test appointment (see page 15 for contact information). The TEAS test may be taken once every three (3) months, for a cost of $45 (fee subject to change) per test (payable to SPCC s Business Office on the day of the test). Complete the Testing Center Permission Form TEAS (see page 15), attach the receipt for payment of the testing fee, and bring both to the Testing Center on the day of your appointment. Be sure to bring a photo ID to your testing appointment! NOTE: Calculators and outside study aids are not allowed during the test. Use of these or refusal to follow all Testing Center rules will result in test disqualification. Is there a waiting list? There is no waiting list to enter into any SPCC health program. Students are chosen for the EMS Bridge program based on their TEAS score and completion of the EMS Bridge Crosswalk requirements. If you are not accepted into the program for the upcoming fall or spring semester, your packet will be shredded. Please make a copy of your packet documentation prior to submission. Enrollment Services is not able to copy your packet for you. How will I know if I m accepted into the EMS Bridge program? Once your application packet has been received, your Crosswalk documents will be reviewed by the Program Director and the Registrar. The Program Director and the Registrar must approve your Crosswalk documents by checking the appropriate items on the Crosswalk and signing the Crosswalk document. A copy of the approved Crosswalk will be sent to the Dean of Allied Health. (See page 13 for Non-Credit to Credit form). You will receive a letter from the Admissions Office within a month of the program application deadline notifying you of your provisional acceptance into the program. ONCE YOU RECEIVE THIS LETTER, you must do the following: (1) Meet with the EMS Program Director and complete the EMS Bridge Program on-line orientation. (2) The EMS Program Director will register you for your EMS Bridging courses. 7
8 Disability Services SPCC provides an ongoing program of services that encourage students with disabilities to integrate themselves into all aspects of college life. The Disability Services provider is available to assist with registration, counseling, academic advisement and arrangements for appropriate accommodation. Reasonable accommodations are established on an individual basis. Students must provide documentation of a disability from a physician or other licensed professional in order to establish eligibility for services.
