In this New Employee Benefits Orientation Guide, you will learn about the benefit plans that are available to you and your eligible dependents, plan coverages, premium rates, instructions for how to make your new hire benefit elections, and important contact information. 3 Benefits Overview This guide provides information about eligibility requirements, enrollment procedures, benefit plans, and important contact information to help you make the best decisions about your coverage options. Eligibility Citizens employees who work at least 30 hours per week are eligible to participate. You also may elect coverage when available for your eligible dependents. NOTE: Domestic partners may be eligible for enrollment in the medical and/or dental plans. If your domestic partner meets the eligibility criteria and you would like to enroll a domestic partner, you must submit a notarized Affidavit of Domestic Partnership for Human Resources approval. Enrollment All benefits are effective on the first day of the month following your 30-day waiting period. You will have 30 days from your date-of-hire to make your benefit elections. If you miss the opportunity to enroll, you will not have another chance to enroll until our annual open enrollment period, which takes place in the fourth quarter of each year. Plan changes only can be made during open enrollment or with a qualifying life event, (i.e. birth or adoption of a child, marriage or divorce, death of a dependent, change in spouse s benefits due to a change in spouse s employment status, etc.). If a qualifying life event occurs, you must notify Human Resources within 31 calendar days of the qualifying status event. Otherwise, you must wait for Citizens annual open enrollment to make changes. 3

4 Medical Citizens provides the Capital Health Plan Health Maintenance Organization (HMO), and two medical plans through Florida Blue: the BlueCare HMO and the BlueChoice Preferred Provider Organization (PPO). Which Medical Plan Works Best for You? HMO or PPO? HMO Plan Overview HMO members must receive care from physicians, hospitals, or other high-quality providers who participate in the plan s network. If members receive care outside of the network, they are responsible for 100 percent of the payment (except for qualifying emergencies). The HMO plans do not require that members receive a referral from a primary care physician (PCP) to see a specialist. PPO Plan Overview The PPO plan offers more flexibility than the HMO plan. You can decide whether you want to use a health care provider within or outside of the Florida Blue PPO network. In-network care is covered by lower preset co-pays or higher coinsurance proportions that typically keep your out-of-pocket costs low. 5 Dental and Vision Dental Citizens provides dental insurance through MetLife. The PDP Plus PPO dental plan provides the option to choose between in-network and out-of-network providers for dental care. Preventative care is always covered at 100%. For care received out-of-network, the provider may bill you for amounts exceeding the plan s payment schedule Dental Premiums Coverage Tiers Semi-Monthly Premium Employee Only $1.91 Employee and Spouse $3.81 Employee and Child(ren) $4.51 Family $6.41 Vision For a complete list of dental providers, visit click the Find a Dentist link, and select the PDP Plus network. You can purchase vision insurance through Davis Vision for you and your dependents at an additional premium paid by you. 6 Life and Accidental Death and Dismemberment Insurance Group Life Insurance Citizens provides basic life insurance at two times your basic annual earnings and accidental death & dismemberment coverage at two times your basic annual earnings at 100% no cost to you. The maximum benefit is $500,000. Beneficiary Designations Since this is a benefit provided to all Citizens employees, it is extremely important that you designate beneficiaries for this insurance in Kronos during your 30-day new hire benefit enrollment period. Imputed Income For basic life insurance, according to the IRS, the first $50,000 in coverage is non-taxable. However, amounts of coverage in excess of $50,000 must be included in income, using the IRS Premium Table, and are subject to social security and Medicare taxes (Internal Revenue Code section 79). What this means to you is that if, for example, you earn $30,000 a year (rounded up), Citizens provides, as a benefit to the employee, $60,000 in life insurance. Therefore, the IRS says that $10,000 of that coverage is a taxable benefit. The Premium Table, referenced in the code, is an age-based table, with rates increasing with age. NOTE: Group Term Life (GTL) will be listed under Gross Pay on your pay stub. The amount of GTL will be added to your gross income, taxed for social security and Medicare, and then backed out again. The only difference in your net check will be an increase in your taxes. If you want more information on Group Term Life, visit theirs website ( 6

7 Life and Accidental Death and Dismemberment Insurance Optional Life Insurance Optional Life Insurance Supplement Life Insurance Coverage For you: You can purchase one, two, or three times your basic annual earnings up to $500,000. Amount available with no Statement of Health application: $350,000. Age reductions: to 65% at age 65, to 45% at age 70, and to 35% at age 75. Benefits cease at retirement. Spouse Life Insurance For your spouse: You can purchase any amount between $5,000 and $150,000, in increments of $5,000, as long as you have elected supplemental life for yourself. Spouse life coverage cannot exceed 50% of your supplemental life amount. Amount available with no Statement of Health application: $35,000. Coverage ends when your spouse turns 70 years old. Dependent Life For your eligible children: An eligible child must be under age 19, or age if he or she is a full-time student and depends on you for 50% or more of his or her support. You can purchase any amount between $2,500 and $10,000, in increments of $2,500, as long as you have elected supplemental life. Eligibility ends on the child s 25th birthday. No coverage is available for children under 14 days old. If a child is between 14 days and 6 months, he or she is eligible for $1,000 in dependent life coverage. Optional Accidental Death and Dismemberment (AD&D) Optional AD&D coverage is available through AIG. It helps to protect your family s financial security in case of death or certain disabling injuries. It pays a benefit to you or your designated beneficiary for loss of life or for certain permanent injuries due to an accident that occurs while you or your dependents are covered by the plan. You may enroll for Voluntary AD&D insurance at any time without having to provide evidence of insurability. 7

8 Disability Benefits Short-Term Disability Short-term disability (STD) benefits can provide income protection in the event you are ill or injured and unable to work. The company provides STD to eligible employees through a salary continuance program that is available upon date of hire. If STD is approved, your years of service determine the benefit amount and length of time you receive a benefit check. Use your sick time first, then vacation time for the first five work days, and then Citizens pays as follows: Years of Service Benefit Less than one year Up to 1 week at 100% of salary, then up to 10 weeks at 60% of salary. More than one year, but less than two years Up to 2 weeks at 100% of salary, then up to 23 weeks at 60% of salary. More than two years Up to 8 weeks at 100% of salary, then up to 17 weeks at 60% of salary. Long-Term Disability Citizens offers voluntary long-term disability (LTD) insurance at group rates through Liberty Mutual. You will be eligible to receive benefits after 26 weeks of a qualified disability. You will receive 60% of your basic monthly earnings, up to a maximum monthly benefit of $10,000. NOTE: Statement of Health will be required for approval if you elect LTD after your new hire enrollment period. 8

9 Flexible Spending Accounts Citizens offers two distinct flexible spending accounts (FSA) to employees: Health Care Account (HCA) Dependent Care Account (DCA) Employees can enroll in one or both accounts. Both the HCA and the DCA save you money by allowing you to set up regular tax-free deductions from your paycheck to cover eligible health and/or dependent care out-of-pocket expenses. HCA This account allows you to pay for qualified health care expenses with pre-tax dollars. Your entire annual account balance is available for services effective your date-of-hire through December 31. You can contribute a maximum of $2,550 annually to this account. NOTE: Participants can rollover up to $500 in unused HCA funds from one calendar year to the next. DCA Generally, you may contribute up to $5,000 each year. If you are married and you and your spouse both participate in a DCA, you are limited to combined contributions of $2,500 each if you file separately. This account offers tax savings on eligible dependent (child or elder) care expenses provided by qualified caregivers. Unlike an HCA, DCAs are not pre-funded; you cannot receive reimbursement for your entire annual account balance on day one. Employees can only be reimbursed up to the amount they have had deducted during that plan year. 9

10 Retirement Savings Plan It s important to plan for your future now, even if retirement is still years away. Do you know that participating in the Citizens Retirement Plan (administered by Lincoln Financial Group) is one of the best ways to save for retirement? Our plan offers tax-deferral on contributions and earnings, plus Citizens will match up to the first 8% of your contributions at 100% immediately upon hire. You may contribute up to 50% of your pay pre-tax. The annual contribution limit in 2016 is $18,000 and allows an additional $6,000 in catch-up contributions if you are 50 years or older. You will be fully vested in the company matching contribution after three years of employment at a rate of 33.33% annually. You will receive one year of vesting credit once you work 1,000 hours or more in a calendar year. Additional Benefits Pay Schedule Citizens employees are paid biweekly (every other Thursday). If you decide not to participate in direct deposit, you will be issued a live check on scheduled paydays. Holiday Schedule Citizens observes nine holidays each year and one personal holiday. Vacation Employees receive paid vacation time after 90 days of employment. The vacation year begins January 1 and ends December 31. The table below shows the paid vacation benefits (in hours) allotted to a full-time employee based on the individual s hire date and employee classification status: Classification Jan - Mar 100% Leave Bestowal Apr - Jun 75% Leave Bestowal Jul - Sep 50% Leave Bestowal Oct - Dec 25% Leave Bestowal Non-Exempt Exempt Assistant Directors and above

11 Additional Benefits, continued Sick Leave All employees scheduled to work 40 hours per week will receive 12 days (96 hours) of sick time per year after 90 days of employment. Those hired after January 1 but prior to July 1, will receive their sick leave benefit after 90 days of employment. Regular full-time employees who are hired on or after July 1, will receive 48 hours of sick leave after 90 days of employment. Other Types of Leave Citizens provides leave for bereavement, jury duty, military duty, and Family Medical Leave. Employee Assistance Program The Employee Assistance Program (EAP) is provided to you and each of your household family members through ComPsych. EAP offers professional assistance and resources for the challenges of everyday living. Wellness Program Why is wellness important? It s about putting you in control of your benefits, health care, financial security and future, and it is Citizens investment in your future. The employee wellness program offers a wide range of programs and activities to encourage and support our employees well-being. College Savings Plan Merrill Lynch Section 529 College Savings Plan is a tax-advantaged way to save and invest for future higher-education expenses and can be used at accredited post-secondary schools. Business Travel Accident Insurance The premium for this coverage is paid 100% by Citizens and may provide up to a $500,000 benefit for specific injuries or your death while traveling on company business. Emergency Travel Assistance As part of your Group Basic Life insurance benefit, Liberty Mutual provides you with emergency travel assistance through an arrangement with Frontier MEDEX. Travelers Auto Insurance You may take advantage of this voluntary benefit to help save money on your auto insurance. Competitive rates. Payroll deducted. Hybrid vehicle discount. 11
