4 About Meridian Meridian Behavioral Healthcare is a private, not-for-profit charitable organization and North Central Florida s leading safety-net provider for people who experience mental illnesses, addictions and related social challenges. Accredited by CARF (Commission on Accreditation of Rehabilitation Facilities), recovery-based care is provided consistent with the highest internationally recognized standards, a commitment to continually enhance the quality of services and a focus on consumer satisfaction and accountability. 5 I mpact Every day, mental illnesses and addictions affect thousands of families across Florida and can affect persons of any age, race, religion or income. Mental illnesses and addictions are biologicallybased brain disorders and are estimated to affect one in five families in America. One in 17 Americans suffers from a serious mental illness. Alcohol and drugs do not discriminate: One in eight Americans has a significant problem with alcohol or drugs, with 40 percent of the group having a dual diagnosis, or co-occurring psychiatric/addictive disorder. Although addiction to drugs or alcohol begins with a personal choice to use these substances, research shows that, for many, a physiologically-based dependence soon sets in. Drug dependence produces significant and lasting changes in brain chemistry and function. It is generally accepted that chemical dependency along with associated mental health disorders is one of the most severe health and social problems facing the U.S. today. Ways to Give Meridian depends greatly on the generosity of individuals and corporations in order to continue to provide charitable care and necessary services. 6 Service Area Meridian s service area in North Central Florida covers a geographic area of almost 7,500 square miles that is largely rural and sparsely populated, serving the following 11 counties: Alachua, Bradford, Columbia, Dixie, Gilchrist, Hamilton, Lafayette, Levy, Putnam (acute care services only), Suwannee and Union. Meridian has numerous Treatment Centers available to Florida residents: Inpatient Treatment Centers located in Gainesville and Lake City Outpatient Treatment Centers located in: Bronson, Cross City, Gainesville, Jasper, Lake Butler, Lake City, Live Oak, Starke and Trenton Residential Group Homes and Treatment Centers located in Gainesville and Lake City More than one million miles travelled each year to provide needed services. 7 Mental Health Services Adult Care Coordinators - Recovery focused services to people who experience a severe and persistent mental illness. Helps individuals identify and coordinate needed services and supports, and to utilize personal strengths, preferences and abilities, to maximize independence, quality of life and recovery. Adult Outpatient Counseling - Services including assessment and treatment. Therapy is offered for a range of issues including, but not limited to: anxiety, depression, mental health, substance abuse, and victimization/abuse (sexual, physical, emotional). Services are provided in clinics, health departments, Federally Qualified Healthcare Centers or at home and are offered in a number of modalities including crisis intervention, individual, family and group. Camp Meridian - Group therapy for children and adolescents, grades 3-8. This summer program is designed to facilitate growth in the areas of social skills, problem solving, conflict resolution, anger management and increased self-esteem through therapeutic group activities. Includes Challenges ROPES Course and outings. Children s Care Coordinators - Assessment of child s needs, advocacy for services and funding, linkage to services including psychiatric, therapeutic, legal and after school activities, and measurement of service effectiveness. Children s Intervention Program - Designed to provide psycho-educational services to persons at risk for problems and support services to persons already receiving treatment, to reduce the likelihood of a family disruption and/or the need for residential placement and to build resiliency in the face of a mental health or substance abuse problem. Includes psycho-educational interventions or groups, Challenges ROPES Course and Late Night Basketball. Children s Outpatient Counseling - Services including assessment and treatment. Therapy is offered for a range of issues including, but not limited to: behavior problems, anxiety, depression, mental health, substance use and victimization/abuse (sexual, physical, emotional). Services are provided in schools, clinics, health departments, Federally Qualified Healthcare Centers or at home and are offered in a number of modalities including crisis intervention, individual, family and group. Crisis Stabilization Unit (CSU) - Meridian operates two CSUs located in Alachua County (Gainesville) and Columbia County (Lake City). Each provides psychiatric evaluation and shortterm in-patient treatment to adults & children experiencing a mental health, emotional, or behavioral crisis, including suicidal thoughts/feelings or other severe psychiatric issues that cannot be addressed on an outpatient basis or for which evaluation is Court-ordered. Average length of stay is three to five days. All persons presenting for services are screened for admission criteria and come voluntarily or under a Baker Act (F.S. 394). Family Crisis Intervention Team - Short-term, crisis and family oriented wrap-around program. Interdisciplinary Team of counselors, care managers, and a nurse works together with families with children at risk of receiving more intensive services, such as placement into a CSU, residential facility, foster home, or Juvenile Detention Center. Provides individual and family counseling, case management and medication monitoring in homes, schools and other community settings. More services
8 Mental Health Services (continued) Forensic Intervention and Outreach Program - Designed to assist individuals, referred by the court, defense counsel, state attorney, court services or jail, that have been charged with an offense and have been assessed by this program to have a mental illness. Services are strength-based, focused on reducing risk by providing intensive case management, enhancing competence and court advocacy. Homeless Enrichment Liaison Program (HELP) - Case management, intervention and outreach to help homeless individuals obtain addictions and mental health treatment services. Also assists with emergency shelter referrals, employment, food/food stamps, GED/job training, healthcare, social services, Social Security Income/Disability benefits, housing, transportation and other supportive services. Housing Opportunities People Empowerment (HOPE) Program - Transitional housing for people who experience a mental illness or are dually diagnosed and homeless. Assists individuals to become self-sufficient by providing necessary services and supports. Joyce House / Adult Women s Group Home - Group home for persons who experience a severe and persistent mental illness. Joyce House requires capacity for semi-independent living. Mental Health Court - A misdemeanor therapeutic court operating in Alachua and Columbia Counties. A Forensic Case Manager is assigned to each individual and maintains contact with the courts regarding his/her progress toward retaining competence. Psychiatric Outpatient Services (Medical Services) - Staff psychiatrists and Advanced Register Nurse Practitioners (ARNP) provide initial and ongoing psychiatric evaluation, and ongoing medication management. Rehabilitation and Community Reintegration Services - Assists individuals to make positive changes in their lives, by teaching psycho-social skills to enhance education, work and social integration in the community. Self-directed care, based on a recovery model that utilizes one s strengths and abilities to encourage and motivate the person to meet their individual goals. Supportive Employment / Job Coach - Dedicated to helping people who experience a severe and persistent mental illness, choose, obtain, and retain a job. Helps to guide and support each individual in obtaining employment in the community in their field of interest.
9 Substance Abuse/Addiction Services Adult Criminal Drug Court - Partnership between the 3rd Judicial Circuit, County Department of Corrections or Court Services, State Attorney, Public Defender, local law enforcement and Meridian, providing intensive services for drug-related felony offenders who have been abusing substances. Aftercare Program - Provides continuing services for persons who have completed a higher level of substance abuse treatment, such as residential, outpatient or traditional outpatient programs; a vital part of recovery. Coping with Addiction through Recovery and Empowerment (CARE) - Specialized day treatment program addressing issues related to addiction and mental health and the bio-psychosocial impact it has on women. Addresses issues including: anxiety, depression, parenting, domestic violence, relationships, self-esteem, and employment. Child Substance Abuse Outpatient and Intervention Programs - Services including assessment and treatment. Therapy offered for a range of issues including, but not limited to: substance abuse, mental health, and victimization/abuse (sexual, physical, emotional). Services are provided in schools, clinics, health departments, Federally Qualified Healthcare Centers, or at home and are offered in a number of modalities including crisis intervention, individual, family, and group. Day/Night Treatment: Support, Treatment and Recovery (STAR and HOPE) - Outpatient programs to help individuals suffering from addictions achieve their highest level of functioning without entering inpatient treatment. Intensely structured and supportive environment including professional, cognitive-behavioral and empathetic counseling and support. Dependency Drug Court - Operating in Alachua, Columbia, Levy and Suwannee Counties, providing intensive services for Family Safety clients whose children have been adjudicated dependent and are at risk of termination of parental rights due to parental substance abuse. Detoxification Unit - Residential, medically supervised, detoxification program for children and adults at risk of withdrawal from prescription medications and/or illegal substances. Program accepts involuntarily ordered clients (Marchman Act) as capacity allows. Evidence-based Prevention Programs - Provides community activities and education aimed at preventing substance abuse, including structured, drug-free activities and enhancement of protective factors to those identified as having risk factors for substance abuse. HIV Outreach and Intervention Program - Education covering HIV, AIDS and other Sexually Transmitted Diseases, stages of infection, transmission, myths and prevention, to heighten customers awareness of their risk factor. Free confidential pre- and post-test counseling opportunities and risk reduction education. Referral to partnering agencies within the community for follow-up regarding Case Management needs. Older Adult Outreach Services - Outreach services help identify risk behaviors and provide education, screening, assessment, and referrals for intervention and treatment services. Emphasis on the special needs of elders and their families providing information about substance abuse, resources for treatment services and the impact of substances on the health of these individuals. Opiate Treatment Program (OTP) / Methadone Treatment - Helps patients with an opioid dependency make major lifestyle changes with the use of medication (methadone or suboxone), individual and group counseling and random drug testing. Dual diagnosis services are available. Outpatient Detox Program - Helps those with a desire to remain alcohol free, but a history of relapse. A new medication, Vivitrol, helps to give a layer of protection against the first drink. Intensive Outpatient Treatment is provided with this program. Recovery Center / Youth Residential Treatment Center - Residential substance abuse treatment program for adolescent males aged Program accepts people involuntarily ordered to seek treatment (Marchman Act), as capacity allows. Sid Martin Bridge House Residential Program - Residential substance abuse treatment for adults and specialized treatment for co-occurring disorders. Treatment Alternatives for Safer Communities (TASC) - Provides assessment and case management services to persons who have committed substance related, non-violent crimes. Links the judicial system to treatment alternatives to incarceration.

School-based Prevention Services - Provides school activities/strategies aimed at preventing the development of alcohol, tobacco, and other drug abuse problems. Services involve multiple contacts to reinforce and promote positive, drug-free behaviors. Many services are provided directly in the schools. Parents become integral parts of these efforts as they participate in homework assignments and other aspects of the prevention activities through the evidenced-based curriculum. Sexual Abuse Treatment Program - Provides individual, family and group counseling to sexually abused children, affected siblings and non-offending caregivers with no means of payment. The aim of these services is to reduce the trauma caused by the child sexual victimization, keep re-victimization from occurring and enable families to have healthy, non-abusive relationships

Transitions is located in Gainesville; Williams Manor is located in Lake City.

17 Supported Employment Supportive Employment, Job Coach - Dedicated to helping people who experience a severe and persistent mental illness choose, obtain and retain a job. The program helps to guide and support each individual in obtaining employment in the community in their field of interest. Our consumers are qualified for a wide variety of skilled and unskilled jobs. Some of the most common jobs that are filled include: Cooks/Prep People/Dishwashers Maintenance/Janitorial/Custodial/Housekeeping/Maid Service Clerk/Cashier/Customer Service Office Assistant/Mail Clerk/Data Entry Vehicle Detailers Production Workers Event Staff The Supported Employment Program also helps local businesses by providing pre-screened, qualified applicants, who are reliable and have lower turnover. Participation in the program enhances a business s community image by demonstrating commitment to nondiscrimination and a diversified workforce. Additionally, participating businesses may also qualify for savings through tax credits, such as the Small Business Tax Credit and the Work Opportunity Tax Credit. The Suppoted Employment Program provides training and screening. Prior to beginning the job search, our consumers have worked closely with a Job Coach. Most have participated in prevocational classes and training programs. Many of our consumers have also been involved in one of our Transitional Employment Positions here at Meridian. Progressive Alternative To Homeless (PATH) - Permanent supportive housing for persons who are mentally ill or dually diagnosed, in recovery and homeless. Satellite, Sunset and Choice Apartments / Supported Housing - HUD properties for independent, income-based affordable housing; one- and two-bedroom apartments for individuals reintegrating into community life. Supported Housing is intended to help individuals make the final transition to fully independent living. Supported Housing apartments are located in Alachua County and are accessible to any person who resides in Meridian s Service area.
