1 WHILE YOU ARE AT THE HOSPITAL The attending physician has already completed the attestation of death (SP3) that the hospital will send to the Directeur de l'état civil and the funeral home. If a coroner is involved, he/she will send to the hospital an authorization for disposition of the body. The hospital will then forward this authorization to the funeral home, which is responsible for sending the information to the Directeur de l'état civil. You do not have to go to the Admissions Department following the death of your loved one. What you need to do Be sure to collect all of the deceased's personal effects. Remember to ask for the deceased's health insurance card. Helpful advice: Ask the duty nurse if the deceased had any personal possessions in the room, and if any valuable items were kept with the Service de sécurité under lock and key. If necessary, you must visit the Service de sécurité (Room G-1020, main entrance) in person in order to collect any valuable items. Today: To be done right away Continued on back

An advisor will guide you through the rest of the process. The funeral home will make the arrangements to collect the body from Hôpital du Sacré-Cœur de. Gather the following documents and items belonging to the deceased before your first visit to the funeral home: Prearrangement number Social Insurance Number Health insurance card Birth certificate or place of birth Name of the parish where the deceased was baptized or registered Father s family name and first name Mother s maiden name and first name Name of the cemetery plot Clothing, shoes, jewellery, undergarments, glasses, make-up Recent photographs Dentures Today: To be done right away
3 WHEN YOU ARE AT HOME You must contact the office of the Directeur de l état civil in order to obtain the required civil status documents (forms, certificates, copies of an act of death, attestations), or submit a request via the website. 4 WHILE YOU ARE AT THE HOSPITAL A member of the HSCM Pastoral Department team is always available to meet with those who have experienced the loss of a loved one in order to provide spiritual support. They will offer comfort, a listening ear and compassion in an environment that strives to respect your personal beliefs and values. Please do not hesitate to call the pastoral team, at , ext Every year, the HSCM Pastoral Department holds a Eucharistic celebration, in French, at 4:05 p.m. on the second Wednesday of November in honour of all those who have passed away during the year. The celebration is held in the hospital chapel (Room G-3115), and is followed by a time of refreshments and fellowship with the priest for those who wish to take part.
