3 SECTION 2 - General Terms and Conditions Original Sheet 3 CONNECTING ARRANGEMENTS FOR CUSTOMER-PROVIDED PUBLIC ADDRESS OR LOUDSPEAKER PAGING SYSTEMS (MAINTENANCE ONLY) Regulations The rates and regulations contained in this Section for connection with customer-provided Public Address or Loudspeaker Paging Systems when associated with Centrex Service with switching equipment located on the customer's premises apply only to service ordered by customers prior to January 1, 1975, and to subsequent additions which in the opinion of the Telephone Company do not require a change out of equipment. Customer-provided Public Address or Loudspeaker Paging Systems, which include amplifiers, receiving speakers, and associated wiring used to transmit paging messages or announcements in one direction only, may be used in connection with Telephone Company facilities furnished for Centrex Service with switching equipment located on the customer's premises subject to the following conditions: Connection with Telephone Company Facilities Connection of customer-provided Public Address or Loudspeaker Paging Systems with facilities of the Telephone Company shall be made only through connecting equipment or arrangements furnished by the Telephone Company. The connecting equipment or arrangements will include a connecting block or equivalent to which the customer-provided Public Address or Loudspeaker Paging System will be connected. Telephone Company facilities when so connected may be used only to transmit messages or signals to customer-provided Public Address or Loudspeaker Paging Systems. Such Public Address or Loudspeaker Paging Systems may not be used to originate messages into Telephone Company facilities. Responsibility of the Telephone Company The use of customer-provided Public Address or Loudspeaker Paging Systems in connection with the facilities of the Telephone Company is permitted only on the condition that the liability of the Telephone Company for damages arising out of failures or defects in the connecting equipment or arrangements, not caused by the negligence of the customer, or of the Telephone Company in failing to maintain proper standards of maintenance and operation and to exercise reasonable supervision, shall in no event exceed an amount equivalent to the proportionate charge to the customer for the connecting equipment or arrangements for the period of service during which such failure or defect in the connecting equipment or arrangements occurs.
4 SECTION 2 - General Terms and Conditions Original Sheet 4 CONNECTING ARRANGEMENTS FOR CUSTOMER-PROVIDED PUBLIC ADDRESS OR LOUDSPEAKER PAGING SYSTEMS (MAINTENANCE ONLY) (cont'd) Regulations (cont'd) Obligation of the Customer The operating characteristics of the customer-provided Public Address or Loudspeaker Paging System shall be such that the equipment will function properly with the facilities of the Telephone Company and will not interfere with any of the services offered by the Telephone Company. Upon notice from the Telephone Company that the equipment of the customer is causing or is likely to cause hazard or interference, the customer shall make such changes as may be necessary to remove or prevent such hazards or interference. The customer indemnifies and saves the Telephone Company harmless against claims for libel, slander or infringement of copyright arising from the improper use of material transmitted over its facilities; against claims for infringement of patents arising from combining with, or using in connection with facilities of the Telephone Company, apparatus or systems of the customer; and against all other claims arising out of any act or omission of the customer in connection with facilities provided by the Telephone Company. 5 SECTION 2 - General Terms and Conditions Original Sheet 5 AND TERMINAL EQUIPMENT (MAINTENANCE ONLY) Regulations Except as otherwise provided for in this Service Guide, Part 2, installations of new Connecting Arrangements will not be made (a) after July 1, 1979 for connection of customer-provided terminal equipment, (b) after January 1, 1980 for connection of customer-provided communications systems, and (c) after July 1, 1980 for equipment-to-equipment connections. Treatment with respect to grandfathered installations which include such Connecting Arrangements is covered in this Service Guide, Part 2.

10 SECTION 2 - General Terms and Conditions Original Sheet 10 AND TERMINAL EQUIPMENT (MAINTENANCE ONLY) (cont'd) Other Connecting Arrangements Message Registration Customer-provided message registers used in conjunction with customer-provided communications systems may be connected to the exchange facilities of the Telephone Company through a Connecting Arrangement furnished, installed and maintained by the Telephone Company. This arrangement provides indications of message registrations for outgoing calls placed over associated Central Office facilities to such registers located on the customer's premises. This arrangement is offered subject to availability of Central Office Equipment and facilities and requires a Type 1011 or 1012 Private Line Channel, as specified in Part 15, between the Central Office and the customer's premises in addition to the rates shown below. Arrangement to permit the connection of customer-provided message register equipment to the exchange facilities of the Telephone Company. Dial Access Arrangement which provides trunk level access via a PBX or Centrex System to a customer- provided equipment (e.g., radio or loudspeaker paging system, dictation equipment or information retrieval system).
11 SECTION 2 - General Terms and Conditions Original Sheet 11 AND TERMINAL EQUIPMENT (MAINTENANCE ONLY) (cont'd) Rates - Other Connecting Arrangements Message Registration Nonrecurring Charge Arrangement to permit the connection of customer-provided message register equipment to the exchange facilities of the Telephone Company - per Central Office facility arranged /CEK/* Dial Access Arrangement which provides trunk level access via a PBX or Centrex System to a customerprovided equipment (e.g., radio or loudspeaker paging system, dictation equipment or information retrieval system) - per arrangement located on different premises from the serving vehicle /DCL/* $ NOTE: Additional equipment at Service Guide rates and charges may be required when associated with certain serving vehicles. * Not available for new installations after July 1, In addition, two Type 2001 channels at Service Guide rates and charges are required. Charges as specified in Part 3 for Premises Work Charges apply

13 SECTION 2 - General Terms and Conditions Original Sheet 13 CONNECTIONS Data Access Arrangements (Existing Stock Only) Nonrecurring Price For connection of customer-provided data transmitting and/or receiving equipment - basic arrangement for manual operation, each /CDT/ - arrangement for unattended sending and receiving through a contact closure type control interface, each /CBT/ - power supply for use with contact closure type interface when not supplied by customer /CBV/ - arrangement for unattended sending and receiving through a voltage type control interface, each /CBS/ $ Prices as specified in Section 3 for Premises Work Charges apply.
