2 Document Purpose This document provides instructions on how to create a new JetPay Unified Employee Self Service Account. Your employer may allow you to view and print your check stubs, view and print your W-2 forms, view and print your 1099-M forms, submit Federal and/or State tax withholding forms, and receive notification of new documents. Accessing JetPay Unified Employee Self Service You can access the Employee Self Service portal by the following method: In the address bar of your browser type o Click Login. o Choose Employee Self Service 2 May 18, 2017

3 Creating your Employee Self Service Account Before you can view your check stubs, you will need to create a JetPay Unified account and register for Employee Self Service. This can be done in a few easy steps. To get started, click Register for an account. 3 May 18, 2017

4 Step 1: Create your JetPay user profile Enter the fields needed to create your profile. All required fields are denoted with a red asterisk *. Step 2: Register with Employee Self Service Click Register to see your paystubs online. 4 May 18, 2017

5 A dialog box will display asking for additional information. You can find your company and employee number on your pay stub or voucher, or by contacting your employer. If you wish to see a sample pay stub with the location of the company number and employee number, click on the question mark. 5 May 18, 2017

7 Step 3: Complete your profile Next, you will be returned to the Create Profile page where you can complete your profile. A new area will appear in the Accounts section, labeled Linked Accounts. You can choose to set Employee Self Service as your default when logging in by simply clicking the Employee Self Service icon. 7 May 18, 2017

8 Once you are sure you have filled out all required fields, click the Submit button to finish creating your JetPay profile. 8 May 18, 2017

9 Step 4: Confirm your address to activate your account A confirmation will be sent to your primary address. This will contain a link to confirm your and activate your account. Click the large link to complete your JetPay account setup. When you click the link, your browser will open a new tab to confirm that your account is ready to use. Close this tab or window, and navigate to the login page at 9 May 18, 2017

10 Step 5: Log in to your account Log into your account by providing your username and password and clicking the login button. 10 May 18, 2017

11 View or Print your Check Stubs Click MY INFO, then click Check Stubs. You will see a list of all your checks or payroll vouchers. You can view or print any check stub in this list. 11 May 18, 2017

12 View your profile Your employer may allow you to view some of your information stored by the employer. To view the information, click MY INFO, then select Profile. Your Employee information will display, showing Identification, Address, Contact, and Emergency Contact information as configured by your employer. 12 May 18, 2017

13 Click Pay Info to view your Employment Status, W-4 Federal Withholding status, Direct Deposit information, State Withholding status, and Local Withholding status. If your employer has enabled W-4 form submit, you can update your Federal and State tax information by clicking the Forms Plus Icon. Forms Plus Icon 13 May 18, 2017

14 Click Notification to subscribe to receive an notifying you when your check stubs, W-2 forms, and/or 1099 forms are ready for you to view. If the notifications tab is not present, your employer has not enabled employee notifications. Click User Account to view and update your profile information. You can also change your password, or connect another JetPay account from this screen. 14 May 18, 2017

15 What should I do if I don t receive the ? If you do not receive the confirmation , first check your spam or junk mail folder. If the confirmation has not arrived, simply log in with your new JetPay username and password. You will be prompted to check that your address is correct and to re-send your activation/confirmation May 18, 2017
