3 Introduction These instructions have been designed to assist teachers, administrators or other school designee in providing salary and teaching data. It is crucial that every comprehensive school CareerTech teacher (or his/her designee) complete this form timely and accurately to assist in timely payments being sent to the schools on a quarterly basis. The due date for completion of the Salary and Teaching Schedule is: September 30 Thank you for the time and effort it takes to provide this information. If we may be of assistance, please contact us at: Oklahoma Department of Career and Technology Education Information Management Division 1500 West Seventh Avenue Stillwater, OK (405) or (800) , extension carol.hall@careertech.ok.gov Getting Started To access the system, enter the following address after connecting to the Internet: From the Information Management Division (IMD) homepage, select Comprehensive Schools and then select Data Submission. Finally, select Salary and Teaching Schedule. You will now see the Salary and Teaching Schedule Screen. See Figure 1. xxxx Figure 1 Salary and Teaching Schedule Setting Your Password Each instructor must set his or her own personalized password. It is imperative that you protect your password to ensure that no one can access your data. In order to set your password, click on Set Password. Enter your last name (for example, Jones), your 5-digit PID number, the last 4 digits of your social security number, and the password you wish to use. The 2 Instructions

4 password may be up to 10 characters long. Click the Update button. Your new password will be displayed. Click the back button twice to return to the password screen. There is no need to reset your password if you are at the same school and same position as last year. If you have forgotten your password, click on Find Your Password. You will be required to enter your last name, PID number and the last 4 digits of your social security number to retrieve your password set previously. Figure 2 Set Password Screen Click on Login to Enter Salary and Teaching Schedule Information. You will now see the Login Screen. (See Figure 3) Figure 3 Login Screen Logging in to the Salary and Teaching Schedule To access the Salary and Teaching Schedule, you must first enter your last name, PID, and password. The last name field is case sensitive, so you must use upper and lower case (for example, Smith). Click on the SUBMIT button. If you get a message saying you were not found in our database, please verify that your name, PID, and password have been entered correctly. If you are still not found in our database, you will need to call the Information Management Division at (405) or send an to carol.hall@careertech.ok.gov before you can continue. In the event you are asked if you want Window s to remember your password, always answer NO. Failure to do so will prevent you from logging in on future sessions. If your login is successful, you will see a Welcome Screen similar to that in Figure 4 that displays your name, position, and school. Please carefully check the information that is listed and contact us if the information is incorrect. Click on the CONTINUE button. 3 Instructions

(See Figure 5) Please note that all information must be submitted in this report before you allowed to save the data. You cannot enter partial data and return at a later time to complete the remainder of the report. Figure 5 Main Menu Select Enter Salary and Teaching Schedule Information option. TEACHING EXPERIENCE The first screen displayed reflects the number of years teaching experience you have for in-state as well as out of state. Please verify each line and make corrections as needed. For new teachers, all items should be 0. Do not count the upcoming school year in your calculations. (See Figure 6). Click on the SAVE button to continue. Figure 6 Teaching Experience 4 Instructions

6 TEACHER AND SALARY INFORMATION See the Salary and Teaching Schedule Worksheet at the back of this guidebook in the event you would like to complete the form before entering data. The top portion of this form (Figure 7.A) reflects information we have on file for you as well as asking you to provide additional information for the current school year. Click in each box to enter the information requested. Two of the data elements have triangular shaped black marks to their right, which is a drop-down box. When you click on the drop-down box, you will see a list of the valid responses to that question. Simply move your mouse pointer to the correct answer and click on it. For all other data elements, type in the requested information. You can also use the tab key to move between boxes and enter the correct response. For complete definitions of each data element, see Appendix A. Figure 7.A For the Unduplicated Enrollment by Grades section (Figure 7.A), click in each box and enter the number of students for each grade. Each student should only be counted ONE TIME regardless of the number of courses for which they are enrolled. TEACHING SCHEDULE For the Teaching Schedule (Figure 7.B), click in each box to enter the information requested. For the Course Name, click on the drop-down box to see the courses that are applicable to your division. For all other data elements, type in the requested information. You can also use the tab key to move between boxes and enter the correct response. For additional explanation on each data element, see Appendix A. 5 Instructions

7 Figure 7.B Teaching Schedule Comments Section The Comments Section should be used to explain the other reimbursement indicated above. It can also be used to provide additional information for courses taught. Be brief in your comments in that this section is limited to 255 characters. Saving the Report After you have filled in all the boxes on the Salary and Teaching Schedule, select SAVE at the bottom of the screen. If you have entered all the information correctly, you will see a message at the top of the screen that says **THE INFORMATION ON THIS PAGE HAS BEEN SAVED**. Otherwise, you will see an error message that reads ****ERROR ENCOUNTERED-PLEASE MAKE THE FOLLOWING CORRECTIONS AND PRESS SAVE ****. Correct the errors and click SAVE again. Print one copy by selecting the print icon or print from the File menu in your Internet browser window. Select Return to Main Menu. NOTE: After selecting SAVE, the Total Salary + Benefits, Total Unduplicated Enrollment by grades, Total Male and Total Female will be calculated. EXIT SALARY AND TEACHING SCHEDULE You should now see the Main Menu screen. Select Exit Salary and Teaching Schedule At this point, click on EXIT to leave the Salary and Teaching Schedule. 6 Instructions

8 APPENDIX A The following provides additional information for each data element requested on the Salary and Teaching Schedule Report. Years of Teaching Experience: Indicate the total years teaching experience you have for instate and out of state. DO NOT include the upcoming school year in the calculations. Certificate Information select from one of the following types: 1 Provisional I 2 Emergency 3 Standard 4 Life 5 License 6 None 7 Provisional II 8 Alternative License Expires: Enter the expiration year of your certificate (example: 2014) Highest Degree Held select from one of the following degrees: 1 - < H.S. Diploma/GED 2 H.S. Diploma/GED 3 Technical Diploma 4 Associate 5 Bachelor 6 Master 7 Doctorate Contract Dates: Enter as mm/dd/yyyy. Example: 07/02/2014. Base Salary: Enter your base salary as per your contract for this current school year. The $200 per month CareerTech reimbursement should NOT be included in this amount. Health Insurance: Enter the health insurance benefit amount paid by your school if applicable. If no assistance is provided, enter 0. Teacher Retirement: Enter the teacher retirement benefit amount paid by your school if applicable. If no assistance is provided, enter 0. Other: Other salary received is defined as other monies NOT pertaining to teaching your class. Examples might include: Stipend for serving as a coach, class sponsor or cheer sponsor Travel allowance and/or meal expenses Life insurance Summer pay Extra duty pay Vision plan Salary protection NOTE: If an amount is entered in the Other section, an explanation must be entered in the Comments section at the bottom of the form. Please be specific but brief. The Comments section is limited to 255 characters. 10 Appendix A

9 Total Salary + Benefits this section will be calculated automatically after the Salary and Teaching Schedule is saved. Contact Name/Position/Phone: Enter the name of someone at your school who can answer questions about your salary, their position and a telephone number. Unduplicated Enrollment by Grade: Enter the number of students for each grade that you will have in your classroom this year. Students that may be enrolled in multiple courses should only be counted one time. The TOTAL amount will be calculated automatically once the report is saved. Schedule Type: This information reflects the type of schedule your school is operating under. Examples would be traditional, block, trimester. If this is not correct, please contact the Information Management Division. The following information will reflect your teaching schedule for the upcoming school year. If you have courses that will be offered multiple times per year because of the manner in which your school operates (for example, block scheduling), list each occurrence for the entire school year. Period: Indicate the course period (example: 1 st period, 2 nd period, 3 rd period.) Course Name: Select your correct course for the period selected from the drop down box. If other is selected, please explain in the Comments section. Begin Time and End Time: Enter the time each course begins and ends. Begin Date and End Date: Enter the date each course begins and ends. Grades: List all grade levels for each student included in each course. # Males and # Females: Enter the number of males and females in each course. For courses that may be offered later in the year (for example, next semester), enter in your best estimate based on pre-enrollment information. Comments: Use this section to explain the amount entered in the Other box at the top of the report in the salary section as well as any other classes. Elaborating on what you are doing that hour on your schedule, and what the Other pay is for. Please be brief as this section is limited to 255 characters. Sample Schedule 11 Appendix A
