1 Survey Product Overview - Survey (LES) The Survey (LES) is a 360 multi-rater feedback process providing experienced professionals with an opportunity to receive feedback on their job performance from the people around them their manager, peers, subordinates and customers. It provides employees and managers with insightful, 360-degree performance feedback needed for them to develop and succeed. This survey is designed to assist in professional development. Every aspect of the process is customized to your organization and the core competencies believed to be important to your professional growth and development. When used effectively, the survey will: Encourage candid and honest communication between you and your colleagues Help you identify your professional strengths and developmental opportunities Establish a quantitative baseline that will allow you to assess your progress Give you the opportunity to improve your skills by being more aware of others' perceptions and observations of your performance Guide your conversations with your colleagues to ensure you remain informed of others' observations and suggestions Improve engagement and performance Participants complete the survey via a web-based application. Executive Development Associates platform allows you to gather and analyze 360-degree feedback quickly and easily. The right platform simplifies the process of getting information from the right people and putting it into a single review. Once results are available, professional feedback is crucial to gain the most out of your results. Professional feedback is offered in the following ways: An EDA consultant delivers feedback either by telephone or in person A qualified internal professional delivers feedback following industry best practice guidelines Executive Development Associates certifies internal professionals to deliver the feedback sessions During the feedback session, key performance strengths are identified, areas for professional development are established and a plan of action is designed. Participants also receive a gap analysis detailing how the professional s self-perception compares to the perceptions of others. e x e c u t i v e d e v e l o p m e n t. c o m
2 COMPETENCIES Page 2 Product Versions The LES is ideal for leadership development. EDA offers 5 versions of the Survey (LES). The LES II and LES III are different versions of the original EDA LES, each one being based on the earlier version. We believe the LES III is most up to date for the current marketplace, but some of our customers prefer earlier versions. The Physician (PLES) and Healthcare (HLES) created for the healthcare industry, one for practicing physicians and the other for general healthcare leaders. 3 Page 3 Customization The LES may be further customized if a more specific approach is desired. EDA may simply design a 360 using current competencies of IIHC clients or we offer validation studies where complete customization from the ground up is available. Logistics Targets are identified (by supervisor or HR). Targets are the people being assessed. EDA sends identified targets (or supervisors) a 360-degree Feedback template. Targets (or supervisor) will fill out spreadsheet with up to 35 Raters. Raters are the people who will be filling out information on the identified Targets. Once all contact information is gathered from the Target and individual Raters have been notified of the upcoming survey, EDA will begin the 360-Degree survey process. EDA initiates survey with introduction to the Target and Raters. EDA initiates reminder s throughout the survey timeline as needed. Once everyone has completed the survey, EDA will aggregate the date and provide a detailed report rating the participant s scores against those of others. Optional: One of our Executive Coaches will then provide a telephone debrief of results focusing on strengths and weaknesses of the Target to assist development. 360-Degree Survey Fees Description of Services & Products 360-Degree Survey 360-Degree Survey with EDA Consultant Debrief Customization of 360-Degree Survey based on Client Competencies Customization of 360-Degree Survey with Validation Study Sales Tax (May Be Applicable) Details $225. USD per target * Each target s survey may choose up to 35 raters $450. USD per hour $12,000. USD flat fee * Client may utilize up to 12 Competencies $4,0000. 4 Page 4 Product Licensing Options EDA will provide Service Partner a 360-degree survey to utilize with their clients. There are several ways EDA could partner with IIHC to accomplish this goal. I. Purchase rights to a single version of the Survey. Service Partner to rename survey as Client proprietary survey. Service Partner would have the ability to utilize and adjust survey as necessary to fulfill their client needs. No support from EDA with technology platform or customer service. Fee: $9, II. License (per use) any EDA Survey and use EDA technology platform. EDA will set up a partner portal on our technology for Service Partner to access and provide surveys to their clients. This portal would be branded for Service Partner. EDA would provide training and customer service to Service Partner as needed. If EDA changes to a new technology platform Service Partner would migrate to the new platform with EDA. a. Service Partner will be responsible for individual survey fees through service provider (current pricing is $99. USD per survey). Individual survey fee is subject to change based on technology platform. b. Set Up Fee: $2,500. USD. One-time fee. Fee will include 10 survey licenses. c. License Fee $25. USD per survey. Billed on a monthly basis. III. EDA is available to provide customization services for 360-degree survey development to Service Partner clients as needed.
