6 FORMULARZ ZGŁOSZENIOWY APPLICATION FORM Formularz zgłoszeniowy - only for the UWr records Erasmus nominations - are sent to the partner universities by the International Office Automatic confirmation of the nomination - via individual account

7 Learning Agreement LA (learning agreement) - agreed list of subjects to study abroad, to be prepared yourself and accepted by a Polish Erasmus Coordinator Changes to LA modification of the previously agreed learning agreement, to be done if necessary within the first month after arrival Research project as a content of LA minimum15 ECTS to be achieved abroad International Office requirement Final number of obligatory ECTS credits - to be decided by the Polish Erasmus Coordinator Deadline for preparing LA: defined usually by the host university, but no later than the 15th June The only eligiable LA forms at

8 APPLICATION PROCEDURE HOST UNIVERSITY After the nomination by Wroclaw International Office, the host university contacts the student and asks for: application form (different from FORMULARZ ZGŁOSZENIOWY!!!) LA (learning agreement), transcript of records from the home university (optional) Application for accommodation (optional) Application for the language course (optional) Sometimes also: confirmation of language skills additional Erasmus nomination other documents Many host universities have their own on-line procedure system which should be followed CHECK THE HOST UNIVERSITY WEBSITE incoming Erasmus students IMPORTANT! No reply from the host university please come to International Office!

9 ACCOMMODATION/VISA/INSURANCE Accommodation abroad: student residence/private market deposit deadline!!!! Visa/residence acceptance letter fro the host university temporary residence holders apply for visas at the embassies in Poland temporary residence abroad info from host university Health insurance: European Health Insurance Card Additional insurance recommended

10 LANGUAGE OF STUDY Language preparation at home university Obligatory on-line language test (EN, FR, DE, ES, IT) Before Erasmus After Erasmus => the results just for the EU statistics Optional on-line language course provided by the Polish International Office (ACCESS FOR THE ON-LINE COURSE) Language courses at the host universities

11 ERASMUS CONTRACT Before the departure: Contract at the individual Erasmus account Signed at the international Office at least 2 weeks before the departure Required documents: (Learning Agreement, bank details, exact dates of the beginning and end of study period abroad) The contract includes: Financial conditions, method of payment Length of study Recognition of the stay (both finance and credits) IMPORTANT! It is necessary to present proof of the payment of the semester fee (UWr ) while signing the contract

12 ERASMUS CONTRACT Contract SIGNED FOR ONE SEMESTER ONLY, exceptionally for the whole academic year, minimum period 3 months Possibility of changing the period of study (annex to the contract) Erasmus grant for maximum 5 months Grant will be paid in full to the bank account of the student in Poland
