1 ACCESSING AND MANAGING YOUR EMPLOYEE STOCK PURCHASE PLAN ACCOUNT USER GUIDE Through Computershare s secure Employee Stock Purchase Plan (ESPP) participant website, you can access and manage your account information with convenience and ease by following the detailed steps below. Participant Service Representatives (PSR) are available at (866) to assist you with Web navigation Monday through Friday 8:00 a.m. to 7:00 p.m. ET in accordance with the NYSE business calendar. Simply follow these three easy steps to log in to Computershare s website: Step 1: Connect to the Internet Step 2: Log on to www-us.computershare.com/employee Step 3: Input your company code or company name and click Next Once connected, activating your ESPP account couldn t be easier. Enter company code or company name 1

2 To access your account through the Web, you will need a user ID or account number and a Personal Identification Number (PIN). Select user ID type (if applicable) Enter your user ID and PIN 2

3 TERMS AND CONDITIONS The first time you enter your login information you will be prompted to read and agree to the site s Terms and Conditions and Privacy Statement. Upon reading the Terms and Conditions and Privacy Statement, click Yes to indicate acceptance of the terms prior to clicking Next 3

4 PIN CHANGE AND SECURITY QUESTIONS For security purposes, upon accepting the Terms and Conditions, you will be asked to change your PIN to a 5-11 digit number of your choice. You will be prompted to enter your new PIN and then repeat the entry to confirm the update. Once your PIN has been updated, you will be asked to answer three security questions. If you have lost or forgotten your PIN, you may change it any time online by correctly answering these three questions. You can also request a new PIN via the interactive voice response (IVR) system or through a PSR. Enter your NEW PIN twice once to change the PIN and a second time to confirm Enter your CURRENT PIN for validation The three security questions range from your pet s name to your favorite color or high school name 4

5 CONTACT DETAILS AND CONFIRMATION Once you update your PIN and select your security questions, you will be asked to confirm your contact details and the security information you provided. Confirm your address Review all details and click Submit if accurate 5

6 Now that you have successfully logged in and updated your PIN information, you may access your account information and perform transactions such plan enrollment, tax certification, and selling your shares. PORTFOLIO SUMMARY The Portfolio Summary page provides upfront account balances for all of your holdings. From this page, you can also review your plan correspondence such as statements or tax forms. Click the Details link to obtain information pertaining to your account holdings, transaction history, and to perform sale transactions. Access important company announcements here Click on Details for additional account information 6

7 PLAN ENROLLMENT The ESPP plan member site allows you to enroll in your company s plan during open enrollment periods. In addition to enrolling in the plan, you may also increase, decrease or cancel your deductions. Your portfolio page will display a new plan with an open enrollment period. Click Enroll in New Offering to begin the process. Prior to enrolling in the plan you will be asked to confirm that you have read the plan provisions. By clicking Agree you authorize Computershare to notify your company to begin payroll deductions. 8

8 PLAN ENROLLMENT Enter the amount to be deducted from each pay period (may be a dollar amount or percentage depending on your plan) Enter your PIN to confirm deduction amount. For your records, a confirmation screen will appear after clicking Submit. 9

9 TAX CERTIFICATION W8-BEN Failure to certify your tax identification number prior to selling your ESPP shares or receiving dividends can result in taxes being withheld at the backup withholding rate. You may certify your tax status by following these three steps. Select your citizenship status 10

10 TAX CERTIFICATION W8-BEN To complete the W8-BEN electronically, you will need to confirm your permanent residence address by either indicating that the address on file is correct or by entering your permanent residence address in the online form. Confirm your permanent residence address Complete the Treaty Benefits section to determine if you are eligible for a reduction in your US withholding tax on dividends (if applicable) 11

11 TAX CERTIFICATION W8-BEN Review the summary and confirm that the information provided is accurate. Enter PIN to affirm information provided is correct. Your PIN serves as a legally binding signature. For your records, a confirmation screen will appear after clicking Submit. 12

12 Knowledge Center Knowledge Center provides access to an abundance of equity compensation information, ranging from the basics of employee stock purchase plans to advanced tax information. The tool contains feature articles, a glossary, FAQs, and quizzes. The Knowledge Center also includes a Tax Center section dedicated solely to frequently asked tax questions and sample annotated Schedule D forms. 17
