1 Owl Claims Credit Card Pack Thank you for downloading the Owl Claims Credit Card pack. This pack contains all you need to begin your claim. Please print this document, fill in the blue boxes and return the signed pages to us at: Owl Claims Hewmar House 120 London Road Gloucester GL1 3PL Explanation of Documents 1. Client Care Letter: The purpose of this letter is to explain the process of your case. There are a number of items contained which we need to make you aware of from a regulatory perspective. However, the underlying fact is that this is a No Win-No Fee agreement. Please keep this for your own records, there is no need to return it to us. 2. Credit Card Authority: This gives us, Owl Claims (a trading name of Owl Solution Ltd), permission to contact your Credit Card provider on your behalf for a copy of your statements and details of your credit card charges. We require copies of these documents to calculate the value of your claim. It also authorises us to receive payment of the settlement monies on your behalf. Please fill out the details of your credit card account at the top and sign and date the document in the box at the bottom of the page. If there are two names on the account, both people must sign the document otherwise this will delay your claim. Please return this to us. We will take a photocopy of this document to keep in your files. 3. Credit Card Payment Authority: This document is only used to pay the Owl Solutions Limited fee if the Credit Card issuer has made repayment directly to your account 4. Contingency Fee Agreement: This is your agreement with us, Owl Claims (a trading name of Owl Solutions Limited), which relates to the 25% charge at the end of your case should you win. Please sign and date the document in the box at the bottom of the page. If there are two names on the account, both people must sign the document otherwise this will delay your claim. Please return this to us. We will take a photocopy of this document to keep in your files. Whilst we appreciate that there is a lot of information to take in, the sooner you return all the relevant documents signed and dated the quicker we can start with your claim.

2 Client Care Letter Your Claim to Recover Financial Charges Owl Claims will be acting on your behalf to recover the unlawful charges placed upon various Accounts that you hold with financial institutions. We aim to provide an efficient and professional service as experts in this field. The attached set of forms relate to your claim. Please sign each of the forms in the boxes at the end of each document. If there are two named people on the relevant account, they must also sign the document in the second signature space. You should also enter the relevant account number for each of the accounts. Please note that from the date we receive your completed documentation, we would hope to resolve your claim within 4 to 5 months. You should therefore complete and return all of the signed documentation as soon as possible in order for us to proceed promptly with your case. We will always contact you prior to accepting any offer put forward by the relevant financial institution so that you can confirm acceptance of that offer. For the avoidance of any doubt, we only charge 25% plus VAT of any monies that we recover on your behalf. There are no other charges for our services. No: Data Protection Act Disclosure Request In relation to the above account, I/we authorise and request you to supply me/us (at a maximum cost pursuant to the Data Protection Act) with: a note of the date the account was opened and a complete list of charges for the whole period during which the account has been with you (irrespective of this being for more than 6 years) together with a list of all debit interest relating to each such charge. If you are unable to comply with this Data Protection request, copy Credit Card statements will be accepted for the years in question, although you will appreciate that this will increase my/our costs, to be borne by you, in the event of a claim. Payments I/We hereby expressly and irrevocably authorise you as my/our Bank, under account number and Sort Code shown above, to forward all settlement monies in respect of my/our claim, by way of a cheque for the sum for the sum agreed, to Owl Solutions Limited. I/We authorise and request that you send such disclosure and payment to the under mentioned Owl Solutions Limited who are solely authorised to receive payment, correspond and to speak to you regarding the operation of the above account and all matters connected therewith. 5 Contingency Fee Agreement This is a legally binding Agreement between the Parties named below. BETWEEN: You, the Client(s): Ref. No: And We, Owl Solutions Limited, Hewmar House, 120 London Road, Gloucester GL1 3PL 1. This Agreement regulates our costs in dealing with your claim for excessive charges against your Loan and or your Payment Protection Insurance (PPI) Provider. 2. You will only have to pay our costs if we recover those excessive charges from your Loan and or Payment Protection Insurance (PPI) Provider. If we do recover those above charges, our costs will be 25%, plus VAT of the excessive charges recovered. 3. There are three likely outcomes of your case, where the following will apply; a. where your loan is already repaid, we will recover an actual repayment of your premium plus interest, in this instance you will pay to us 25% plus VAT of the recovered element; * b. where your loan is repaid in part and there is outstanding PPI, we will recover an actual repayment of the PPI and the remainder of the loan will be written off, you will pay to us 25% plus VAT of the whole recovery and written off element; * and c. where your loan is fully written off, as it is larger than the PPI element, you will pay to us 25% plus VAT of the written off element * 4. This Agreement will stay in force until it is terminated by either us or you. It will be automatically terminated when your claim is settled either by way of negotiation or Court process. 5. It is a condition of this Agreement that you provide us with prompt and clear instructions. If you fail to do so, despite two written requests, we may end this Agreement. 6. We may also terminate this Agreement if we inform you that it would not be advisable to pursue your claim any further. Provided you have co-operated with us, as outlined above, you will not be liable to pay us any costs if this is the case. 7. If you decline to accept our advice in relation to the acceptability of an offer made by the other party, we may accept the offer on your behalf if we fail to receive your written response within 28 days of the date of our letter advising you of such an offer. 8. If the agreement is terminated under either clauses 4 and 5 above, then we have the right to preserve our lien (hold) over all your files until any money owed to us under this agreement is paid in full.
