Details of the shared savings distribution methodology are discussed, as well as the MPCACO's performance. These meetings have proven to be helpful with regard to the discussion of questions, concerns, and suggestions. Please ask your office staff if they have scheduled this meeting yet, and if not, contact Sarah or Charlotte to schedule the meeting. Population Health Manager Christina Smith, LVN comes to us with 34 years of healthcare experience in Cardiology, ICU, ER, Med/Surg and QA/Risk Management. Her specialty is Cardiology. She stated that what attracted her to this position is the ability to be a patient advocate. She is a Dallas, TX native. She has been married for 30 years and has three wonderful sons, two great daughter in-laws and four fur kids. In her spare time she likes to read, craft, bake and walk her pups. Whenever she can, she loves to do volunteer work. In her position, she will focus on low risk patients within our patient population with goals to keep the patients healthy, build relationships with the patients and anticipate their needs, solidify and promote primary care utilization. Health Coach AnnMarie Mercado, MA was born and raised in Dallas, Texas. Before accepting this position, she worked in a busy, fast paced family practice for 16 years as a medical assistant. She is currently attending her last year at Dallas Baptist University where she is working on obtaining a Bachelor's of Art and Sciences in Health Care Management. She is happily married to her high school sweetheart for twelve years and has two sons. In her spare time she enjoys physical activities like kickboxing, strength training, and hiking. She is also an avid movie watcher and enjoy going to the theatre with family In her position, she will focus on educating and enhancing selfmanagement within our patient population with an emphasis on our low risk patients. She will also support other clinical positions within the ACO departments. Data Analyst Meet Anuj Parikh. He comes to us from Texas Health Organization for Physicians (ACO) as a Clinical Quality Outcomes Specialist. He has a MBA/MHA from UTA with Bachelors in PrePharmacy and HealthCare Administration. He is in ACHE. Anuj is responsible for the implementation and maintenance and coordinating the activities of data management, data aggregation, analysis and the reporting process. Anuj joined Methodist Patient Centered ACO January 13, 2014.
4 Page 4 of 5 Quality Reporting CMS requires that Medicare Shared Saving Programs (MSSP) collect and submit data as part of the 33 ACO Quality Measures. Dallas Methodist Physicians Network (DMPN), as an agent for the MPCACO, and other designated individuals within our system will begin the process of collecting quality measure information next week. The time permitted by CMS to collect, review, and upload all of the data modules is limited to six weeks beginning January 27, 2014; therefore, we request your cooperation and assistance in identifying medical records and allowing the nurse auditors immediate access to these Medicare beneficiaries' records. Seven of the measures will be collected by CMS using a mailed survey to Medicare beneficiaries attributed to the MPCACO; six measures are based on care coordination/patient safety. The remaining measures focus on outpatient care provided by physicians; these measures include preventive health, diabetes, hypertension, ischemic vascular disease, heart failure and coronary artery disease. Two of the outpatient measures are "composite" measures: diabetes (5 bundled measures) and coronary artery disease (two bundled measures). CMS selects up to 616 Medicare beneficiaries for each measure; of those beneficiaries, we must submit data for 411 beneficiaries consecutively as rank ordered by CMS. Each beneficiary may have multiple measures which require abstracting. This is a significant undertaking and one in which the MPCACO will be engaged on an annual basis for the duration of the MPCACO's contract with CMS for the MSSP. The collection of data for reporting quality measures and overall physicians' performance will impact potential shared savings between the MPCACO and CMS over the next two years. The first year of reporting is a demonstration to CMS of our ability to collect the data, review it for consistency and accuracy, and upload the data within the time constraints issued by CMS. Only in subsequent years will performance targets be included.
