9 Spate of FFH incidents 30 Apr A lifting supervisor fell 30m through a shaft during a lifting operation; 30 Apr A worker working inside the ship s cargo hold, fell 17m through a partially-cut side shell plate of the hull onto the drydock; 3 May A worker fell almost 10m through a sky light during roof installation works; 3 May A worker fell about 3m from an A-frame ladder while trying to place a box onto the roof of a store; 6 May A worker lost his balance and fell while checking on a safety boat on board a ship; 9 May A worker fell 19m from the roof of a bin centre of a factory while attempting to retrieve a bulky object; and 11 May A worker fell through the roof of a warehouse while preparing for painting works. 23 May A worker fell from a storage rack while installing electrical wiring in the premises. 14 June A worker fell from a height of 5m while dismantling pipe barricades. 15 July A worker fell from a height of about 25m while erecting a hanging scaffold. 1 Aug A worker fall from roof of a warehouse while installing fall preventive lifelines for subsequent roof work. 9/41

12 MANAGEMENT COMMITMENT Boustead Projects recognises the potential fatal hazard of carrying out all work at height and hence requires that all such activities by subcontractors be properly assessed and planned with a formal submission of the Fall Prevention Plan since August 2009.
13 MANAGEMENT COMMITMENT 19.1 FALL PREVENTION PLAN POLICY a. Commitment to prevention of injury of persons from incident of falling from height and objects falling from height. b. Commitment to comply with applicable legal and other requirements whilst carrying out working at height of 2m and above. c. This policy shall be communicated, understood, implemented and maintained at all levels of the organization and to all interested parties. d. Boustead Projects Pte Ltd shall reviewed this policy annually to ensure that it remains relevant and appropriate to the organization and will make amendment as and when necessary. 14 Building Capabilities and Competencies Memo WSH Alert: 19/11/WSH/HH To: All Sub-contractors & Boustead Projects Site / WSH Personnel From: Howard How, Corporate Safety CC: Thomas Chu, Steven Koh, Koh Nghee Kwang, Neo Eng Huat Date: 02 May 2011 Re: WORKING AT HEIGHT (WAH) SUPERVISORS Since the launching of the Code of Practice (CP 1) for Working Safely at Height on 1st February 2010, we have promoting the implementation of the Fall Protection Plan (FPP) which forms the cornerstone of this CP. Our MD, Thomas Chu had endorsed the Fall Protection Policy since 1st August 2009 which states the company commitment to prevent injury of persons from incident of falling from heights and objects falling from height. In order to facilitate the implementation of the FPP effectively, the WSHC and MOM has rolled out the Working at Height for Supervisors course since October 2010 in order to raise the competency level of supervisors at site to tackle working at height activities. Boustead Projects Pte Ltd has been arranging various groups of sub-contractors supervisors to attend such course since November Till date, we have about close to about 100 supervisors from sub-contractors and Boustead Projects who had attended the course and carrying out WAH supervision at all sites. As such, Boustead Projects Pte Ltd will be making compulsory for all sub-contractors to have their supervisors trained as WAH supervisor at one of the three Accredited Training Provider (ATP) by the latest 30th June 2011, failing which all working at height activities by the respective sub-contractors will be stopped. 18 INCREASING AWARENESS WSHO briefed all workers on the CP1 Working Safely at Height Inspection of fall arrest systems; The inspection of fall arrest systems by a competent person should be conducted periodically and include the following: The rope or webbing including the anchorage lines (e.g. inspecting for any defects or damage and checking the anchorage); The fall arrest system harness body (e.g. inspecting for any damage to the mounting ring or the body, checking the activation of the fall arrest indicator and that the labels are present); The locking mechanisms and rope guides (e.g. inspecting the visible rope guides for excessive wear and checking that the rope runs freely through the anchorage and that the locking mechanism works properly); and The hardware (e.g. checking that the snap hooks or links work properly). That the PPE or its components have not exceeded the manufacturer s recommended usage life.
19 INCREASING AWARENESS Boustead WSHC Billy brief all supervisors and workers SWP/RA/PTW requirements before Height. All scaffolds must be erected by Trained erectors and checked by Qualified Scaffold Supervisor and there must be no unauthorized access to work at heights.

21 INCREASING AWARENESS Weekly Mass Tool Box Meeting: 17 Date / Time: 12th / 1130hrs Training description: Safe use of ladders. Company participated: TSF, San Keong, VBH, Innofield, Nylect, Tat Lee. No of Participants: 37 Training Duration: 30mins a) Always inspect the ladder prior to using it. If the ladder is damaged, it must be removed from site. b) Always maintain a 3-point (two hands and a foot, or two feet and a hand) contact on the ladder when climbing keep your body near the middle of the step and always face the ladder while climbing. c) Do not use the top 3steps/rungs of a ladder. d) Use a ladder only on a stable and level surface, provide buddy system(hold by Coworker) e) No aluminium ladder is allowed. f) A frame ladder is only allowed in constrained area.
22 INCREASING AWARENESS Demonstration from external training provider of proper life line installation and anchorage of safety harness to workers to increase their awareness when working at height.
23 IMPROVING SYSTEM PROCEDURES All working at height (WAH)activities must be covered with a permit to work on a daily basis with a trained WAH supervisor carrying out a Working at Height Checklist for the work area. (WAH Checklist for Supervisors bilingual.docx )
24 IMPROVING SYSTEM PROCEDURES Better prevention of falling from height with the use of system formwork for column and slab construction with proper design of working platform and guard rail in the system formwork.
25 IMPROVING SYSTEM PROCEDURES To eliminate falling hazards for worker working at height, the use of mobile elevated work platform (MEWP) like boom lifts and scissor lifts are commonly use at the workplaces.
26 IMPROVING SYSTEM PROCEDURES Life line installation for workers to anchor for working at height during fabrication of slab formwork.
