2 SUmmaRy The medical Imaging market is approaching a period of rapid change with new electronics based corporations seeking to establish a position in a market currently dominated by three companies. The global market is large and growing although there are the usual socio-economic pressures on all healthcare markets that will restrict adoption of new technologies unless they can be shown to reduce costs to the healthcare provider. advances in technology, in electronics, engineering and photonics as well as in software and chemistry are changing the way that patients are being treated and leading to opportunities in preclinical research. Scotland has an excellent opportunity to participate in the coming changes based on the first class research being undertaken in the academic base in collaboration with the NhS and commercial organisations. Scotland has a small number of companies focussed on medical Imaging and if this sector is to grow and capitalise on the significant market opportunities it will need support in skills, capital and infrastructure. Background Scotland has a long and impressive history in this sector dating back to the seminal work on mri in aberdeen in the 1970s. more recently work has continued in developing new modalities, novel software and in the tracers and contrast media required to produce high quality information for clinicians and researchers. advances in technology, in electronics, engineering and photonics as well as in software and chemistry are changing the way that patients are being treated and leading to opportunities in preclinical research.
3 Market and Key Drivers The global market for diagnostic imaging was valued at $15.8 billion in 2008 and is predicted to reach $24.6 billion by Sales of imaging equipment such as PET, CT and mri are big-ticket items and sales in the US have recently been hit by restrictions on hospital capital spending due to the deficit Reduction act of 2005 and the recession. however, growth of medical imaging will continue to be driven by: New hospital construction and changes in private medical insurance provision in countries such as brazil, in China, India (with growing populations and wealth) and the US over the next decade; Increasing numbers of independent diagnostic centres offering private mri and CT scans which will also stimulate demand; growing interest in the application of imaging in preclinical and early clinical drug development. however, many drug companies have opted to work in collaboration with academic and clinical research facilities to offset the high infrastructure cost of facilities, equipment and skilled personnel of medical imaging; growing global population, increased ageing of the population, chronically ill people and increasing awareness and need for medical services; advanced imaging technologies and increased adoption of digital imaging which has increased consumer expenditure on medical imaging services. On the other hand, huge R&d costs have resulted in pricing pressure on the manufacturers. general Electric (ge), Siemens, and Philips dominate the medical imaging hardware market and while ge remains the current leader of medical imaging, Philips and Siemens are now allocating increased amounts of capital to healthcare. There is growing interest in imaging from telecommunications companies such as Samsung, Nokia and honeywell as the importance of data handling becomes more critical to better patient care. Industry Needs most imaging systems are considered to be near the peak of the resolution possible for them so the un-met needs are mainly in image capture and processing, to eliminate artifacts for example. The remaining needs can currently be broken down into a few categories: New tracers, for all modalities New scintillation and detector materials, for most modalities molecular imaging, for most modalities The first two are essentially incremental developments of existing technology. The contrast agents have difficulty with regulations as they are treated like new drugs. New materials help with direct detection and can help reduce costs and improve imaging. molecular imaging comprises the development of tracers to act as biomarkers for study of biological systems and aid in the development of new pharmaceuticals. They can be used for PET, SPECT and Optical modalities. There is an emerging market in stratified medicine which will require imaging modalities in order to bring new products to market and in stem cell research (see report on stratified medicine at innovation/foresighting-reports.aspx)
4 Scottish capabilities most of Scotland s capabilities are centred around the NhS and academic hubs in the major conurbations and these centres remain committed to the purchase of new equipment and to both applied and fundamental research as shown by the recent decision to award Varian medical Systems a 20m contract to supply eight Truebeam radiotherapy treatment systems to hospitals in Scotland along with two Clinac ix linear accelerators, ancillary equipment and software. There are also a number of companies who are actively involved in developing new approaches to diagnosis as shown opposite. 5 Opportunities for Scotland The main opportunities for Scotland will be based on developing new collaborations between companies and academics and also between academic centres of excellence around the world. There is also an opportunity for the development of novel tracers and software solutions for a growing market. In Scotland there is currently little commercial activity around medical imaging. however, there is a broad and deep expertise in imaging technologies and their application both in veterinary and human medicine and an encouraging portfolio through which Scotland could build business opportunities for the future. There are a number of assets that are underutilised due to location and availability of trained staff e.g. mri and CT scanners in NhS facilities that might be used by clinical researchers if access could be arranged. greater cooperation and use of assets in a cash constrained environment would be beneficial for Scotland. Key to this is the funding of additional staff. Software could be further developed to provide a more detailed and integrated analysis of a number of modalities e.g. X Ray, CT and mri together based on existing technology. microfluidics could be an exciting area for co-development especially for preclinical research. Telecoms companies are looking to diversify into healthcare and this will impact on opportunities for new developments this will be especially important for opportunities with the NRS* connected/nodal infrastructure and Picture archiving and Communications Systems (PaCS). however, there is a broad and deep expertise in imaging technologies and their application both in veterinary and human medicine and an encouraging portfolio through which Scotland could build business opportunities for the future. *NRS is an initiative implemented by the Chief Scientist Office of the Scottish government and health boards to streamline the NhS R&d management permission process for multicentre1 research in Scotland.
