Please complete and submit one questionnaire sheet. The reverse side of the Sample answers can be used as a copy of the questionnaire for the respondent s memorandum. I. Basic notes (1) Please write down your answers clearly in black or blue ink. (2) Please use block letters and Arabic numerals. (3) Please fill in amounts of money in units of ten thousands, rounding off fractions. When an amount is less than 5,000, fill in 0. (4) Please fill in the ratios by integer (e.g. 6.3% 6%, 1.5% 2%) so as to make the total be 100%. When the total does not amount to 100%, adjust the category with the largest ratio. (5) Survey units constitute individual establishments. Therefore, please provide information pertaining to your establishment only according to the question contents, regarding your establishment or main business( ). Establishments covered by this survey The targets of this survey are establishments categorized within 391: Computer programming and other software services and 392: Data processing and information services in accordance with the Japan Standard Industry Classification (JSIC). The details are as follows: (1) Targets in Computer programming and other software services are establishments engaged in the following services in response to customers requests. (i) Services for computer programming and services related to examination, analysis and consultation in relation to program creation (ii) Services for computer package programming( ) and services related to examination, analysis and consultation related to package program creation ( ) Software packed in a box as a program manua l set, software already installed in PCs,

2 etc. and shipped, and game software, etc. (2) Targets in Data processing and information services are establishments engaged in the following services in response to customers requests. (i) Services for conducting consigned calculations using computers (including cases where customers conduct such operations on their own) (ii) Services for entering data onto computer media (data entry services) (iii) Database services for the collection, processing and accumulation of various types of data (information on real estate, weather, or scientific technology, etc.) and provision thereof as information (iv) Services for controlling and operating users data processing systems and computer facilities, etc. (v) Services for conducting various types of research including market and public opinion research The following establishments are excluded: (i) Establishments engaged in Internet based services (JSIC: 401) (Note) These services include Application Service Provider (ASP) services and contents distribution services, although some of the ASP services that include the creation of software comprehensively are covered by the survey. See the explanation of main businesses below for the details. (ii) Establishments engaged in the sale of software Establishments that solely conduct the sale of software products developed by other establishments (JSIC: J-Wholesale and retail trade) (iii) Establishments engaged in internal business Establishments engaged in software services or data processing and information services as internal business solely for their own purposes (in such areas as data processing for financial institutions, etc.) (iv) Establishments engaged in the manufacturing of information records (audio disk records, video disk records, audio tape records, magnetic cards, etc.) Information recording materials, except for newspapers, books, other printed material, etc. (JSIC: 3296) (v) Establishments engaged in services for providing news to newspapers, periodicals and television News syndicates (News suppliers) (JSIC: 4151) (vi) Inquiry agencies (JSIC: 8091 Detective agencies and credit bureaus ) and tourist information agencies (JSIC: 8399 Miscellaneous living-related and personal services, n.e.c. ) (vii) Management consultants (JSIC: 8093 Business consultants ) (Reference) Japan Standard Industry Classification (JSIC) The JSIC is the statistical standard for publishing survey results by industry. All economic activities related to the production or provision of goods and services which are conducted at establishments as social division of labor are classified according to four stages (see the website of the Ministry of Internal Affairs and Communications for the details. (1) Computer programming and other software services (JSIC: 391) (i) Custom software services (JSIC: 3911) Establishments engaged in services related to computer programming and services related to examination, analysis and consultation on program creation under consignment of customers e.g. Custom software services, programming services, information system development services, and software creation consultation services (ii) Package software services (JSIC: 3912) Establishments engaged in services related to computer package programming and services related to examination, analysis and consultation on package program creation e.g. Package software services and game software creation services

3 (2) Data processing and information services (JSIC: 392 (i) Data processing services (JSIC: 3921) Establishments engaged in the provision of consigned calculations using computers (including the case where customers operate on their own) and data entry services or services related thereto e.g. Consigned calculation services, calculation centers, time sharing services, machine time services, data entry services and punching services (ii) Research and information services, except marketing or opinion research services (JSIC: 3922) Establishments engaged in services engaged in the collection, processing and accumulation of various types of data and providing them as information e.g. Database services (information services for providing information on real estate, transportation, weather, or scientific technology, etc.) (iii) Miscellaneous data processing and information services (JSIC: 3929) Establishments engaged in data processing and information services that cannot be classified otherwise, such as market research and public opinion research e.g. Market research services and public opinion research services

A parent company and a subsidiary are both independent without the relationship of the head office and a branch office in between. 1 Individual An individual establishment that has no head office, branch offices or business offices under the management of the same entity in other locations 2 Head office An establishment that has branch or business offices under the management of the same entity in other locations and governs all of them. When functions of the head office are divided in multiple locations, the one where the company s representatives, such as the president belong to shall be the head office (2) and the others shall be branch offices (3). 3 Branch office Establishment governed by a head office situated in another location Regarding the following items (No. 4-7), please fill in the amount or (ratio) for your establishment only. The portions for other establishments shall not be included. 4 Annual sales (1) Annual sales of the establishment (including consumption tax) (i) Please fill in the annual sales of your establishment during the one-year period between November 1, 2005 and October 31, The value shall not be that for profit or income, but rather for total sales before subtracting costs, which include consumption tax. Should the value during the above-mentioned one-year period not be available, or there be any unavoidable circumstances, please fill in the value for the one-year period prior to the settlement date closest to the survey date. (ii) With regard to the provision of services through in-house transactions between the head and branch offices or between branch offices, offering prices or transfer invoice prices (or when neither of these is available, costs for providing the services) shall be included in the amount of annual sales. When receiving commissions for services, including system construction, as a single package, each service is basically divided and categorized respectively, while those that are indivisible are retained within this category. Database services Services aimed at collecting, processing and accumulating various types of data onto computers and providing said data as information upon request Internet Services aimed at providing databases through networks such as the Internet (limited to services for the collection, processing and provision of information) Others Services aimed at providing data off-line through channels other than networks such as the Internet, and services aimed at providing data through packaged media such as magnetic tapes or CD-ROM Various research Think tank services, consulting services (data processing consulting services shall be included in the category of Information processing services ), and services aimed at conducting market, public opinion and economic research Others Data entry such as keypunching, worker dispatching services for information service-related lectures and training sessions which are not based on dispatching agreements under the Worker Dispatching Act (such as subcontractors), and data processing and information services other than those listed above (Note) Internet based services (JSIC:401) are not covered by this survey. (However, elements of the services are included in the targets of this survey as shown below.) (ii) Internet Data Center (IDC) services Server hosting/housing services for conducting operation and management of server systems and rental and management of servers for the Internet by connecting servers held by IDCs to clients PCs through the Internet and private circuits (Computation conducted through conventional batch process systems are covered in this survey.) (iii) Contents distribution services Services for distributing images, music, on-line games, etc. through the Internet without creating software (Services for collecting, processing and providing information on real estate, weather, or economy, etc. are covered in this survey) (iv) Others Services for supporting businesses that utilize the Internet (Portal services such

If your establishment did not acquire any assets for the relevant one-year period, fill in 0 in the column for the total.
