3 1.0 General Information 1.1 Purpose of Training All entities covered by the Health Insurance Portability and Accountability Act (HIPAA) must implement ICD-10 for medical coding by the Centers for Medicare and Medicaid Services (CMS) compliance date. This mandate affects all Indian Health Service (IHS), Tribal, and Urban (I/T/U) sites. ICD-10 inpatient and outpatient total more than 155,000 codes compared to ICD-9 s 17,000 and only about 20% of ICD-9 codes crosswalk to ICD-10 codes. The transition to ICD-10 will affect almost every employee and contractor in IHS and will require extensive coordination throughout Indian Country and beyond (payer, vendors, etc.). Conversion to ICD-10 will meet HIPAA mandates and position trained I/T/U staff to take a lead role in implementing ICD-10 in their respective programs. Business processes, such as, accurate code assignment, documentation improvement and its impact on the revenue cycle must be factored into the ICD-10 migration. The purpose of this course is to provide participating professionals with an understanding of: The necessary skills and knowledge on the processes, procedures, policies, and system updates for ICD-10 implementation The differences between ICD-9 and ICD-10 The rationale for ICD-10 adoption Existing tools, risks and industry updates The importance of clinical terms and meanings How ICD-10 affects business processes The clinical documentation requirements as a result of ICD-10 adoption 1.2 Prerequisites Code Set Versioning must be completed at your facility and within your Area or 2012 Draft ICD-10-CM, ICD-10-PCS Code Books are the responsibility of each participant to bring to this course. 1.3 Guidelines for Receiving Continuing Education Credit At the conclusion of class, a link to Survey Monkey will be distributed to all classroom participants. Once the Survey Monkey is completed a certificate for AAPC or AHIMA CEU will be available for printing. 1

13 5.0 Biographical Sketches The Indian Health Service (IHS) Office of Information Technology (OIT) National Electronic Health Record (EHR) Training and Deployment Program and Health Information Management (HIM) Consultants workgroup worked diligently to prepare the necessary deployment and training documents to facilitate the deployment of the Meaningful Use (MU) of an EHR. We hope that you find both the training and training documents helpful. The Office of Information Technology conveys its sincere thanks to all the members and guests of this workgroup. They spent long hours preparing and updating these documents and even longer documenting their experience; they deserve our appreciation. Without these dedicated workgroup members this EHR & MU for Health Information Management hands-on course would not be possible. LCDR Mollie Ayala, MHI OIT USET EHR Training and Deployment Coordinator LCDR Mollie Ayala is a Commissioned Officer with the United States Public Health Service and has been with the Indian Health service and the Phoenix Area since She is a recent graduate of Arizona State University with a Master s degree in Healthcare Innovation. In addition, she has served in a variety of positions ranging from IT Specialist where she implemented an area level Help Desk system that is currently providing a centralized level of support for all Phoenix area facilities. She then moved on to become a Revenue Application Coordinator for the Phoenix Area Management Services Organization. In this position, she was responsible for providing specialized 3 rd Party and Accounts Receivable application support for 12 clinics and hospitals. Her current role will be to serve a Deployment Coordinator/CAC for the ARRA EHR Meaningful Use training and deployment program. 11
14 Robin Bartlett, PharmD, MSP Nashville Area Clinical Applications Coordinator and Pharmacy Consultant CDR Robin Bartlett is currently serving as Nashville Area Clinical Applications Coordinator, Chief Area Pharmacy Consultant, Meaningful Use Coordinator, Training Coordinator, and Improvement Support Team member since April CDR Bartlett is a Commissioned Officer in the USPHS and has been in the Indian Health Service since After graduating with a Doctor of Pharmacy degree from University of Florida, CDR Bartlett completed a one-year Indian Health Service Pharmacy Practice Residency Program at Cherokee Indian Hospital. CDR Bartlett transferred to Whiteriver Indian Hospital in 2002 and served the role of a Clinical Staff Pharmacist. CDR Bartlett transferred back to Cherokee Indian Hospital in 2004 and served as a Clinical Staff Pharmacist, Pharmacy Intern Experiential Program Director, Inpatient Pharmacy Services Coordinator, Joint Commission Medication Management Workgroup Team Leader, and electronic MAR Implementation Project Manager. CDR Bartlett is certified as a National Clinical Pharmacy Specialist (NCPS) in Anticoagulation through the IHS Clinical Support Center and an adult pharmacy-based immunization provider. CDR Robin Bartlett graduated with a Master of Science in Pharmacy degree program with special emphasis in Patient Safety and Risk Management from the University of Florida in Tonya Billie Clinton Service Unit Medical Records Administrator Ms. Billie has 16 years of HIM experience, 15 of which have been in the private sector in non-profit and profit facilities. A majority of her experience has been in the inpatient setting of specialty and Trauma One facilities. She currently serves as the HIM Director of the Clinton Service Unit. CAPT Deborah Burkybile, RN, MSN, CPC OIT USET EHR Training/Deployment Consultant Deborah has been a Registered Nurse for 30 years. During this time her nursing practice led her to work in a variety of private sector hospitals, clinics, tribal facilities, and for the last 22 years in Indian Health Service. CAPT Burkybile has been on assignment to OIT since 2005 as the National EHR Training/Deployment Consultant and has worked diligently to train and deploy the IHS RPMS EHR across the nation in federal, tribal, and urban health facilities. Deborah is a citizen of the Cherokee Nation of Oklahoma. She received her Commission in the U.S. Public Health Service in 1988 and presently works from the Nashville Area Office. Deborah has functioned in a number of nursing practice roles including Community Health, Addictions, Injury Prevention, and Managed Care. She is also a Certified Professional Coder. Deborah is strongly committed to improved patient through the use of the IHS EHR and has found her assignment to OIT to be one of the most satisfying nursing experiences to date. 12
15 Pat Cerna, RHIT, CPC HIM Supervisor, Pascua Yaqui Tribe Pat is the Health Information Manager for the Pascua Yaqui Tribe in Tucson, AZ. Prior to her position with the Pascua Yaqui Tribe, Pat worked for the Tucson Area Indian Health Service in Health Information Management for 20 years. Pat is a member of the American Health Information Management Association (AHIMA) and Health Information Management Association of Arizona (AzHIMA). Pat obtained her Registered Health Information Technician (RHIT) credential in Pat is a member of the Tohono O odham Nation. Janice Jan Chase, RHIT OIT - Tucson, HIM Practice Management and ICD-10 Federal Lead Jan is an IT Specialist for the Resource Patient Management System (RPMS), Health Information Management (HIM), Practice Management applications and the ICD-10 Federal Lead for the IHS Office of Information Technology (OIT). Prior to her position with OIT in December 2009, Jan worked for the Tucson Indian Health Service in various Headquarters, Area and Service Unit positions for 17 years. Her IHS career includes Contract Health Service, Management Development, HIM Supervision, Performance Improvement, Privacy and Compliance. Jan also was the past Tucson Area Health Record Consultant and National HIM Program Co-Lead and served on various IHS workgroups. She is a member of the American Health Information Management Association (AHIMA) and Health Information Management Association of Arizona (HIMA of AZ). Jan is a member of the Gros Ventre Tribe of Montana with paternal ties to the Mandan Tribe of North Dakota. Teresa Chasteen, RHIT Bemidji Area Clinical Applications Coordinator Teresa is the Bemidji Area Clinical Applications Coordinator. Her previous position at the Cass Lake Indian Health Service was the Director of Health Information, where she was the Project Lead for EHR Implementation. She served as one of the Bemidji Area Health Information Management Consultants. She started her Health Information Management career in 1984 and has been in the health care field since Teresa has worked in Indian Health Service since She obtained the Registered Health Information Technician (RHIT) in 1992 from the College of Saint Catherine Saint Mary s campus. David Civic, M.D., M.M.M Phoenix Area Performance Improvement Dave is a physician who was raised in upstate New York and trained in Family Medicine at Brown University. He joined the Indian Health Service in 1989 as a Medical Officer in Mescalero. He has served as Clinical Director in Mescalero, Medical Director for a California Tribal program at MACT Health Board, provided care in the California County Hospital System at Tuolumne General Hospital, worked in a hospital owned physician group in central Maine at Maine 13
16 General Health Associates, and served as Clinical Director at Phoenix Indian Medical Center from He is currently employed at the Phoenix Area Office Quality Management Program, providing physician consultation in accreditation, credentialing, quality improvement and process redesign. Dave is married to Valerie, and they have two children, Cary and Bryce. They live in Arcadia, central Phoenix. Dave is an avid skier, runner, biker, swimmer and outdoor nut. Val is the Director of the Arizona Housing Alliance, a nonprofit supporting the affordable housing effort in Arizona. CDR Cornelius ( Neill ) Dial, RPh Portland Area Clinical Applications Coordinator In his 16 years working for the Indian Health Service, CDR Dial has served at the Clinic, Service Unit and Area level. He started his IHS career as a COSTEP at the Ft. Thompson Health Center on the Crow Creek reservation in the Aberdeen Area, served in the Albuquerque Area and later at the Navajo Area s Gallup Indian Medical Center as the Pharmacy Clinical Applications Coordinator / Pharmacy Package Administrator. He is the Vice Chair of the Pharmacy Specialty Group Committee, serves on the IHS National P&T Committee and the National Pharmacy Council. CDR Dial hails from North Carolina and is a graduate of the University of North Carolina Chapel Hill. Deanna Dennis, CPC, CPC-H, CPC-I Billings Area Business Office Coordinator and ICD-10 Area Coordinator Ms. Dennis is a credentialed PMCC Instructor credentialed through the American Academy of Professional Coders (AAPC); attending certified AHIMA ICD-10 Training; worked for various departments in IHS for 35 years; 20 years of work experience in third party billing at the Service Unit level, served as a SU-Business Office Manager and Billings Area Business Office Manager/Coordinator. She is an enrolled member of the Crow Tribe of Montana. She began with the Department of Agriculture in Minneapolis, MN and relocated in 1989 to her home town of Bemidji, MN where she began work with the Indian Health Service. 17 Jamie Furniss, RHIT, CCS-P Portland Area HIM Consultant Ms. Furniss has served as the Portland Area HIM Consultant since Jamie has worked for the Indian Health Service in Fort Hall, Idaho since At Fort Hall she has been the HIM Supervisor since 2007 and the EHR Clinical Applications Coordinator since Patricia Gowan, MPA, RHIA, CPC, Certified AHIMA ICD-10 Trainer United South and Eastern Tribes (USET) HIM & MU Consultant Ms. Gowan is currently the Health Information Management (HIM) Meaningful Use Consultant for the United South & Eastern Tribes Regional Extension Center (USET REC), Nashville, TN. She worked for the Indian Health Service for 23 years and retired in August Her past experiences include national Lead HIM Consultant, Phoenix Area HIM Consultant, HIM Director, Whiteriver Service Unit, and Assistant HIM Director, Phoenix Indian Medical Center. She received her graduate degree at Northern Arizona University, Flagstaff, AZ and undergraduate degree at Carroll College, Helena, MT. Leslie Racine, CPC Area PCC/Coding Coordinator, Area Planning/Statistical Officer Ms. Racine is a Program Analyst for the Billings Area Indian Health Service. She is an enrolled member of the Gros-Ventre Tribe of Fort Belknap, Montana. Leslie serves as the Billings Area Patient Care Component (PCC) Coordinator and Coding Coordinator with over 30 years combined experience to include the transition from APC to PCIS to PCC to the EHR. In this role, she provides analysis of visits for various clinics, providers, ancillary services, and subsequent guidance to staff and tribes. 16
19 She has served 29 years as a coding specialist that qualifies her to serve as the Area Reports Specialist, Planning Officer and Statistical Officer. Her expertise in analyses of health information ensures best practices for purposes of reporting and compliance in the following areas: workload, user population, disease incidence/prevalence, health disparities, planning, morbidity, mortality, reimbursement, billing, staffing, facility planning, and other statistical analyses. Leslie has served on various national and local I/T/U committees for FDI, Health Disparities, Diabetes Reporting/Coding, ISAC, HSP Steering Committee, PSG s, Visit Definition Workgroup, and Compliance. She is a Certified Professional Coder (CPC) through AAPC for over 8 years. CDR Amy Rubin, PharmD Oklahoma City Area Office Clinical Applications Coordinator CDR Amy Rubin began her career with the Commissioned Corps and the Indian Health Service in 1999 as an Assistant Chief Pharmacist in a small clinic. In 2002, she transferred to a hospital as a clinical pharmacist and soon moved into the Assistant Chief Pharmacist position. During this tenure, CDR Rubin was actively involved in many aspects of the operations of the hospital and was actively involved in many committees. She took on the task of preparing and coordinating the pharmacy for the conversion to Pharmacy 5/7 software. She also assisted in the design and set-up of the new ambulatory care clinic including working with architectures and Information Technology staff to develop a facility with the ability to support successful implementation of the RPMS Electronic Health Record (RPMSEHR). Both of these activities led to the assignment as Acting Clinical Applications Coordinator (CAC) in November 2007 to lead the process of implementing RPMS EHR throughout the service unit (3 facilities). She remained the Assistant Chief Pharmacist during this initial phase of implementation and performed in both capacities. In May 2008, she was selected as the full-time Service Unit CAC. The Service Unit under her leadership was successful in implementing RPMS/EHR at 3 facilities. In April 2009, CDR Rubin made the transition to the Oklahoma City Area CAC. In this capacity, she provides EHR support to all sites using EHR in the Oklahoma City Area. She is actively involved in the implementation of new sites. Additional duties include serving as the Meaningful Use Coordinator for the Area and supporting the Area Improving Patient Care Initiative. She will complete a Masters in Medical Informatics in December Peggy Shults, BS Claremore Indian Hospital HIM Director Ms. Shults is the Director of HIM at Claremore Indian Hospital. She obtained her Bachelor of Science in Health Information Management Administration in Claremore, Oklahoma. Peggy has 31 years of experience in the HIM field, 22 years with IHS. 17
20 Anne Spencer, RHIT Albuquerque Area HIM Consultant Ms. Spencer is a Registered Health Information Technician and has been employed with the Indian Health Service for 23 years. Anne is currently employed with the Albuquerque IHS Dental clinic as the HIM Manager. Anne started serving as the HIM Consultant for the Albuquerque area in February Anne has held other positions within IHS as an inpatient statistician, Release of Information, inpatient coder and Third Party Coordinator as well as a Patient Registration supervisor in the Business Office. She has worked in other IHS facilities which include Gallup Indian Medical Center, Fort Defiance Indian Hospital, Tsaile Health Center and Santa Fe Indian Hospital. Anne is a member of the American Health Information Management Association (AHIMA) and the New Mexico Health Information Management Association (NMHIMA). Kelly Stewart, MS, RHIA Tucson Area HIM Consultant Ms. Stewart has been an HIM Director for over 10 years in a variety of facilities. During his commission, he has served as a pharmacist, physician assistant, quality manger, risk manager, and compliance officer for the Pine Ridge, South Dakota and Cherokee, North Carolina Indian Hospitals. He has also served as an HIV/AIDS/STD consultant, performance improvement consultant, pharmacy consultant, and diabetes clinical consultant for the Nashville Area Indian Health Service. At this time, he is the EHR Deployment Manager for the IHS Office of Information Technology and has been charged with both training and deployment of the Electronic Health Record throughout the entire Indian Health Care system. David Taylor has been awarded the PHS Meritorious Service Medal (MSM) in recognition for his accomplishments in the EHR arena.
