The plan has a contract with Group Health Cooperative to pay health and prescription drug claims incurred under the terms of the plan. The total premiums paid for the plan year ending December 31, 2013 were $3,976,118.
3 For Washington Dental Service Dental Plan for Faculty, Administration and Staff This is a summary of the annual report of the Washington Dental Service Dental Plan for Faculty, Administration and Staff, EIN , Plan No. 506, for period January 01, 2013 through December 31, The annual report has been filed with the Employee Benefits Security Administration, U.S. Department of Labor, as required under the Employee Retirement Income Security Act of 1974 (ERISA). The plan has a contract with Delta Dental of Washington to pay dental claims incurred under the terms of the plan. The total premiums paid for the plan year ending December 31, 2013 were $1,053,338. Because Delta Dental of Washington is a so called experience-rated contract, the premium costs are affected by, among other things, the number and size of claims. Of the total insurance premiums paid for the plan year ending December 31, 2013, the premiums paid under this experience-rated contract were $1,053,338 and the total of all benefit claims paid under this experience-rated contract during the plan year was $921,217.
