1 Lighthouse Rules & Regulations (Revised January 2, 2014) Admission Criteria: 1. Sincere motivation to participate in a meaningful recovery program. 2. Minimum of 18 years of age. 3. Employed or employment ready. (Disability, SSI, Retirement & Unemployment benefits are also accepted) 4. Picture ID & Social Security Card 5. No outstanding warrant s, violent criminal felons, or convicted sex offenders. 6. Physical detoxification from alcohol, illicit drugs, and or prescribed narcotic medications. (controlled substances prohibited) *New residents are on restriction for up to 2 weeks. (case by case) Premise restrictions and/or other activities, along with limited calling & physical visitations are designed to help start a program of recovery, and to become fully acquainted and familiar with the Lighthouse operations, class, & meeting schedules, and to get acquainted with the other residents. House Staff pre-approval is required for residents to continue their regular employment schedule as well as seeking new employment during this restriction period. Residents are required to use a pre-approved "Buddy System" during their restricted period at all times. ***Personal cell phone use is restricted for ONE MONTH.*** (case by case) *House staff will monitor resident capability, performance, participation, and attitude to determine continued residency. Resident Initial Staff Initial. Necessary Items: Quantity Twin Fitted Sheet, Blanket/Comforter, Pillow & Towel set Winter Coat/Jacket, Bath Shoes/Slippers & Sleep Wear Slacks, Jeans, or Shorts Combined Long/Short Sleeve & T-Shirts Combined Sock Pairs, Underwear, Under T- Shirts Laundry Soap, Laundry Basket, Personal Toiletries, Razor....1 Optional: Bicycle (if not driving), Alarm Clock/Radio Personal Snacks & Milk (if wanted) & Special Food Items Funding: ***$10.00 fee for late rent payment*** Any transport fees from client location to LRH are additional or clients responsibility and must be paid prior to transport. Weekly service fees are due no later than 1:00pm of each Saturday in the LRH office. All payments are to be paid by Cashiers Check, Money Order, or Credit Card. (no cash or personal check accepted) Weekly, Biweekly, Semi-Monthly, and Monthly paid residents fees are due on their individual paydays, A minimum of 48 hour eviction notice will be served to any resident that becomes delinquent on their service fees. Resident Initial Staff Initial. Visitation: *Intoxicated visitors, illicit drugs and or alcohol will not be allowed on premises. Visitors are only permitted in the common lower level areas and outside grounds. Visitation after restriction time is: Monday thru Saturday 1:00pm to 4:00pm & 6:00pm to 8:00pm Friday & Saturday only. Sunday 2:00pm to 6:00pm. *Children visitation is Saturday & Sunday only! (Non-Residents and children are prohibited in personal areas.) *All visitors including fellowship members must leave premises by curfew. Premises include outside grounds and vehicles. (Male visitors are prohibited at ladies house.) Children are allowed in common rooms only and must be directly supervised and never left unattended. Any visitation appointments that are not during regular visiting hours must have pre-approval by House Staff. This includes fellowship members. Resident Initial Staff Initial. Personal Property & Dress Code: *The Lighthouse Recovery Home is not responsible for any damage or theft to resident personal property. Any and all security measures are each resident's responsibility and at their own discretion and cost. *Dress Code: No Hoodies are to be worn on/over your head in the Lighthouse or at outside meetings. Also, pants are to always be worn at waist height. No facial piercings - only stud earrings are allowed. ***No new tattoo s or piercings are allowed without written approval from the Lighthouse staff.*** Resident Initial Staff Initial.

2 Random Alcohol & Drug Testing: Each resident may be required to submit to a Breathalyzer and or U/A test at any time, with or without cause. Resident's refusal or failing of a test will result in an immediate discharge. Responsibility of any and all costs related to a failed U/A test are at the resident's expense. $25.00 test & $105 for any synthetic items: Spice, K2, Bath Salt, over-the-counter, etc. Any testing charges are to be paid at the time of discharge. Discharge Policy: (case by case) Any offense: 1 to 30 days. Readmission requires residents to: 1. Pay any outstanding past bills. 2. Be able to pass a breathalyzer & U/A test. 3. Pay the $50.00 reapplication Fee. **This policy is for the support of the residents' recovery as well as to insure the integrity of the LRH and most importantly for the safety and well being of each resident and House Staff /Volunteers - It is easier and cheaper to stay clean & sober then start over! Employment: Volunteer, LRH Community Service work, employment, side jobs, and or actively seeking employment are required of all residents that are physically and mentally capable. Certain types of employment are not permitted. IE, bars, liquor stores, etc. Residents must return to LH within 30 minutes of their punch out time. *Employment status will be reviewed regularly by the House Staff. Job search sheets must be filled out and turned in to the House Staff. *Any resident currently living in a ¾ house without employment will be relocated back to the main location until new employment has been established. *RESIDENTS ARE RESPONSIBLE FOR THEIR OWN EMPLOYMENT AND TRANSPORTATION. Automobiles: Each resident who owns a vehicle must provide a copy of a valid driver's license, vehicle registration, and proof of insurance. Some restrictions will apply during first 90 days at the Staff discretion. Residents must park vehicles in designated parking areas only. (See House Staff for designated parking areas.) *No storage or major engine repairs on vehicles is permitted on the premises. *All other modes of transportation and bicycles must be stored in their designated areas only. (see House Staff for designated storage areas) Medications: All prescription, non-prescription, vitamin supplements, powder drink, herbal supplements and any other health products as well as personal hygiene products will all be reviewed and inspected and documented during the intake. Any new medicine, prescription or non-prescription must be recorded and inspected by house staff. All controlled substances (Narcotics) are prohibited except in emergency situations. Products containing alcohol (mouthwash etc.) will not be permitted. Any medications or containers with unreadable or improper labeling will not be permitted and disposed of. The dispensing of personal medication will be the responsibility of the resident. *Failure to submit and document any and all prescribed and or over the counter medications/supplements to House Staff - could be grounds for immediate disciplinary actions and or discharge. LRH is not responsible for any personal possessions including medicine. Laundry: * House laundry & linen (bedding) are to be done weekly. Personal laundry is to be removed from appliances and completed by curfew. Residents are encouraged to share wash loads to decrease costs. Washers and dryers need to be operated at a full load capacity. Residents supply their own laundry soap and laundry basket. Watch your water usage. ***If the water bill goes up, service fees can and may go up as well.*** Mail: (letters and packages) ***Absolutely No Private P.O. Boxes Allowed!*** *All incoming resident mail must be addressed to the main men s house or women s house for inspection.

3 Meals: *1 Hot Meal per day will be provided. Dinner will be prepared and served at 5:30pm. Residents are required to meet in dining room at 5:00pm for progress reports and then to eat dinner together in designated area. Excused, early, or late dinner plates must be requested and submitted to House Staff 24 hours in advance. Residents are responsible for their own breakfast and lunch meal preparations and special food items. (See House Staff for supplemented breakfast, lunch, and snack items.) *Residents have full access of kitchen for any cooking & food preparations. (Some restrictions apply including time periods.) *After a House Staff inspection, residents are permitted to bring and store their own food items. Each food item is to be clearly marked with resident's name. (See House Staff for designated food storage areas.) *Any old, left out, stale, or spoiled food will be disposed of immediately by House Staff. *Residents are responsible for any and all clean up. Curfew: *Each resident must be signed in/out and verified by House Staff. A call is required prior to any tardiness or changes. Failure to comply will result with disciplinary action :30pm Mon - Thurs. Doors locked at 11:00pm (Lights out / T.V. off 12:00am) 2. 11:30am Fri & Sat. - Doors locked at 12:00pm (Lights out / T.V. off 1:00am) 3. 6:00pm Sun. - Doors locked at 8:00pm (lights out / T.V. off 12:00am) *Smoking is not allowed outside after doors are locked. No exceptions! Late night smoking is only in designated areas. (see House Staff) Phone: *Personal Calls have contributed to many relapses - Be Aware! It is advised to change your cell number. *No cell phone calls/texting are permitted anywhere inside the LRH building, but are permitted outside on the grounds for work and emergency purposes. (See House Staff for resident landline phone number.) Hours: Monday-Thursday 8:00am to 11:00pm Friday & Saturday 8:00am to 12:00pm Sunday 8:00am to 1l:00pm ***A 10 minute time limit is required on calls*** No calling is allowed during meetings, classes, & meal hour. Interference with house business or abuse of the LRH resident line and/or personal cell phones will be grounds for cell phone & landline restrictions. Phone Restrictions upon intake: If you own or arrive with a cell phone, it must be turned over and held by The House Staff for a minimum of 30 days. No landline usage will be allowed during the first 72 hours upon intake. Only for verified emergency cases will you be allowed to use the landline during the initial 72 hours. Staff will make calls for you if necessary. LRH Community Service: The LRH is completely "self-supporting". Paid resident service fees (rent) are how we keep our doors open. We do offer up to one week of paid service fees for those that qualify in exchange for 30 hours of documented LRH Community Service as assigned by House Staff only if work is available. LRH Community Service does not include regularly scheduled daily, weekly, monthly, seasonal chores. Chore & Job Duties: House Staff will assign daily, weekly, monthly, seasonal, maintenance and job duties to each resident. Each resident is to complete their posted daily chores by the specified time! One 1 hour naps are allowed weekdays with Staff permission only. The upstairs is closed from 9:45am to 4:00pm weekdays. Evening chores are to be completed by 6:30pm. (including kitchen cleanup) Assigned weekly, monthly, seasonal/other chores are to be completed by no later than Sat 12:00 Noon. *Residents are to keep all areas of house, outside grounds, and garage clean. Period! This includes garbage, counter & table tops, basins, showers, toilets, floors, butt cans, ashtrays. (Pop cans need to be disposed of in designated recycle cans) All rooms are subject to inspection by House Staff at any time.

4 Personal Pass: If you leave on a day pass or overnight pass without doing your weekly & daily chores, or complete your weekly meetings, fail to prepay for your upcoming weekly services fees, and/or miss the Lighthouse Saturday 1:00pm deadline - you will be disciplined including up to discharge! *Daytime & 24 hour overnight passes are granted at the discretion of House Staff. Overnight requests must submitted in writing 72 hours in advance. Day Passes require 24 hours in advance. *Residents that are off restriction and in good standing (no demerits/infractions) and service fees are current will be eligible to apply for a pass. 1. First 30 days - Once a week - 2 hour up to 6 hour weekday/weekend pass. 2. After 30 days - Twice a week - 2 hour up to 12 hour weekday/weekend pass. 3. After 30 days - Once a month - One 24 hour overnight weekend pass. 4. After 60 days - Twice a month - One 24 hour overnight weekend pass. 5. After 90 days - Once a month - One 48 hour overnight weekend pass. (48 hour weekend passes start at 6:00pm on Friday evenings after dinner.) ***You must return by 6:00pm on Sunday evening curfew without exception.*** Residents must be accompanied during the restriction period by a pre-approved person for job search, outside fellowship meetings, and/or personal appointments, etc. Extended time passes for "Special Events" will be considered such as vacation, work, or other family or legal obligations. *It is your responsibility to ask another resident to cover your daily/weekly chore assignments and they must sign your pass as verification. If you can't make a trade with someone or get them covered while you are away you can't go! Alcohol consumption / Drug use is strictly prohibited during your pass. Residents are prohibited from visiting any public or private places where alcohol or drugs are present or being consumed. House Curriculum: Resident attendance/compliance is required as follows: Rise & Shine 7:00am Weekdays & 8:00am Weekends Morning group 7:30am Mon Fri & 8:30am Sat - Sun. Curfew 10:30pm Weekdays - 11:30pm Weekends - 6:00pm Sundays *The following items are excusable only for an emergency or employment, Failure or any tardiness will result in disciplinary action. 1. Scheduled Back to Basics, Video/CD classes, and Sobriety School. 2. If employed you must attend a minimum of one Sobriety School per week. 3. 5:00pm Evening Group (progress reports) then 5:30pm Dinner. 4. Outside AA/NA meetings are a requirement of two per week. 5. Morning Meditation Group 7:30am Mon - Fri & 8:30am Sat - Sun documented recovery hours if not employed. 15 documented recovery hours if you are employed.(10 recovery hours when working overtime) 7. The 10:00am AA Meeting if not working. 8. Review/Progress Reports scheduled meetings with House Staff. 9. Assigned chores, Assigned homework & Assigned Videos. 10. Assigned shower / Laundry schedule. *Each resident is required to have an approved sponsor" within the first 30 days. Contact between residents and/or non-residents. 1. Any fraternizing, lewd, or inappropriate behavior between residents and/or fellowship members will not be tolerated. 2. Male and female residents are not to make phone calls between the men s resident line and women's resident line, cell phones, and vice versa. Staff will make the phone calls for you if necessary. 3. Female residents will not be permitted in basement of the Men's facility unless approved by house managers and/or directors. 4. Male residents will not be permitted to go on the grounds of the Women's facility, unless approved by staff. 5. Only 30 minutes before and after classes/meetings will be allowed including outside fellowship.

5 Regulations *House staff will not take the role of cop, lawyer, or investigator. This means discharge can occur with or without investigation of "who did what, what was said, and who is or who is not at fault." *All rules and regulations are subject to additions and/or changes at the discretion of House Staff / LRH Board. The violation of any house rule or regulation will be grounds for an immediate disciplinary action up to permanent discharge. They are to be observed and adhered to during your residency at the Lighthouse Recovery Home. *Personal belongings will be removed from premises 30 days from discharge date without notice. Discharged residents must set up an appointment with house staff during business hours (9am to 5pm) to collect personal belongings. Discharged residents that are unable to collect their belongings in person must provide LRH House Staff with a signed and dated written request in order for items to be released to their named authorized person. Documented Occurrences: (restrictions may apply) 1st Demerit: hours restriction (confiscation of cell phone and restricted landline usage) 2nd Demerit: One week restriction (confiscation of cell phone and restricted landline usage) 3rd Demerit: Two week restriction (confiscation of cell phone and restricted landline usage) 4th Demerit: Immediate Discharge with any legal authorities being notified. House Rule & Regulation Violations 1. No resident visitors on LRH premises under the influence including family members or their significant other. 2. Verbal or physical threats, acts of violence, and/or fighting. 3. Failure to comply with House Curriculum. 4. Removal of property or its destruction or any altering of the house interior including walls. 5. Discrepancies or unaccountability with time and/or place while off of the Lighthouse premises. 6. Failure to comply with responsibilities and/or staff directions demerits within 30 Days. 8. Smoking in undesignated areas. 9. Stealing or borrowing anything without permission. 10. Falsifications or misrepresentation of any intake, verification slips, sign in/out sheets, LRH Service vouchers, or Community services. 11. Abuse of computer/tablet/phone or internet privileges. (no pornography on computers, tablets or cell phones) 12. Abuse of LRH vehicles. 13. Insubordination/disrespect towards House Staff. 14. Any form of harassment including sexual. 15. Facial piercings except stud earrings. 16. Alcohol/drug logos on any clothing or personal items. 17. Fraternizing (inappropriately) with anyone (including spouses and fellowship members) on LRH premises. 18. Illegal Activities. 19. Skipping or missing required classes/meetings. 20. Illegal gambling. 21. Leaving lights & TV on after use. 22. Leaving dishes, personal items, or garbage in sink, counter tops, basement, or in any common areas. 23. Not signing in & out. 24. Pornography of any type. 25. Violation of curfew and/or Lights Out/T.V. Off. 26. Failure/Tardy to return from personal pass. 27. Delinquent Service Fees (Rent). ***$10.00 fee for late rent payment.*** 28. New intimate relationships between residents and non-residents from date of intake until date of discharged.

6 *This is not a complete list of all the possible rules and/or violations. Resident restrictions or Disciplinary actions including up to discharge will be based on each individual case.* Please do your part to use common sense when it comes to anything that may affect your stay at the LRH or others. *A written appeal for any discrepancies will be accepted by the House Staff & LRH Board of Directors for review. Any appeal may be submitted prior to the first Tuesday of every month.* I understand and fully agree to comply and obey with all the LRH Guidelines, and the Rules & Regulations and treatment plan.
