8 Vitality Contribution Manager The Vitality Contribution Manager is a reporting tool that allows employers to adjust workers CHP contributions based on their engagement in the Vitality platform. Vitality Contribution Manager Strategy can be based on employees: Taking the Vitality Health Review Achieving Vitality Silver Status Take Your Vitality Health Review Achieve Silver Vitality Status
9 Employer Administration Employers choose the incentive and administer the contribution adjustment. Employers can choose to adjust workers monthly CHP contributions or personal spending account contribution amounts Difference in workers contribution costs cannot be greater than 30% of the total cost for self-only coverage Using Vitality status ensures ACA compliance and Reasonable Alternative Standards in engagement tracking The cost for CHP coverage billed by the Concordia Plan Services is the SAME regardless of employers or workers participation in the Vitality Contribution Manager (VCM).

12 VCM Timeline November 1, 2015 Deadline to decide on a VCM strategy January 2016 Vitality program launch Oct 31, 2016 Deadline for workers to complete VCM requirements Prior to Dec 1, 2016 Employers will receive a report with a list of workers who have completed the requirements January 1, 2017 Employers implement adjusted contribution rates for workers New/renewed VCM strategy begins for 2017
