3 ASSOCIATE DEGREE IN NURSING TRACK PROGRAM CURRICULUM CODE: AAS.ADN.TRK STEPS TO COMPLETE AN ADN APPLICATION INCLUDE: 1. Complete Lake Land College Intent to Enroll form online or submit it to Admissions and Records. Choose the Nursing Track Major (AAS.ADN.TRK). 2. Request a paper copy of the ADN Intent to Enroll form from the Nursing Department in Neal Hall. 3. Submit the completed form to the Nursing Department. 4. Submit transcripts to Admissions and Records by February 1. Up-to-date high school transcript and final transcript upon graduation. All college transcripts, if applicable. 5. Complete the Lake Land Compass Placement Test. You may submit your ACT scores in lieu of taking the placement exam. ACT scores should be reported directly to the college from ACT. Test results are valid for five years. The outcome of testing determines whether you are eligible to take the Pre-RN exam. 6. Take the NLN Pre-RN exam. Sign up through the Nursing Department. A minimum score of 90 is required to be considered for admission. 7. You must have completed one year of high school biology or BIO 100 at Lake Land College and one year of high school chemistry or CHM 101, CHM 111 or CHM 120 at Lake Land College with a C or above. 8. You will receive points for healthcare experience. DEADLINE FOR COMPLETED NURSING DEPARTMENT APPLICATION FILE: FEBRUARY 1 *NLN Pre-RN test is offered the first Friday of April, September and December. Payment for NLN test is due three weeks prior. 4 COSMETOLOGY TRACK PROGRAM CURRICULUM CODE: CRT.COS.TRK STEPS TO COMPLETE A COSMETOLOGY APPLICATION INCLUDE: 1. Complete the Lake Land College Intent to Enroll form and submit to Admissions and Records. Choose the Cosmetology Track major (CRT.COS.TRK). 2. Complete the Lake Land Compass Placement Test. Call to schedule the test or you may submit your ACT scores in lieu of taking the placement test. ACT scores should be reported directly to the college from ACT. 3. Complete all Cosmetology Intent Packet forms and submit them to Peggy Strange, director of Cosmetology. 4. Submit an up-to-date high school transcript to the Cosmetology Department, to the director of Cosmetology and to Admissions and Records. 5. Submit your final high school transcript to Admissions and Records and the Cosmetology Department upon graduation. 6. Beneficial high school classes include: art, business, chemistry, computer, English, psychology and speech. 5 DENTAL HYGIENE TRACK PROGRAM CURRICULUM CODE: AAS.DH.TRK STEPS TO COMPLETE A DENTAL HYGIENE APPLICATION INCLUDE: 1. Submit the Lake Land College Intent to Enroll form online or submit to Admissions and Records. Choose the Dental Hygiene Track Major (AAS.DH.TRK). 2. Complete a Dental Hygiene Intent Packet by January 31 for the year you are applying. The packet is available from Dental Hygiene office. Call to request one. 3. Submit your transcripts to Admissions and Records by January 31. Your up-to-date high school transcript and final transcript upon graduation. All college transcripts. 4. Complete the Lake Land Compass Placement Test. Call to schedule a test time or you may submit your ACT scores in lieu of taking the placement exam. ACT scores should be reported directly to the college from ACT. 5. College chemistry, anatomy and physiology I, and microbiology must be completed with a C or above before entering the program. Submit the Lake Land College Intent to Enroll form online or submit to Admissions and Records. Choose the Massage Therapy Track Major (CRT.MT.TRK). 2. Submit the online Massage Therapy Application. 3. Submit official transcripts to the Admissions Office by March 1/July 1 for fall registration or October 1/December 1 for spring registration: Up-to-date high school transcript and final transcript upon graduation. All college transcripts. 4. Complete the Lake Land Compass Placement Test. Call to schedule a test time or you may submit your ACT scores in lieu of taking the Compass test. ACT scores should be reported directly to the college from ACT. PART-TIME PROGRAM - The five-semester, part-time night program begins each fall semester. The deadline for the five-semester, part-time program is March 1/July 1. FULL-TIME PROGRAM - The three-semester, full-time day program begins each spring semester. The deadline for the three-semester, full-time program is October 1/December 1. ADMISSION IS DETERMINED BY THE ABOVE INFORMATION. STUDENTS ARE INFORMED OF THEIR STATUS BY APRIL/JULY FOR THE PART-TIME PROGRAM AND NOVEMBER/DECEMBER FOR THE FULL--TIME PROGRAM. 9 PHYSICAL THERAPIST ASSISTANT TRACK PROGRAM CURRICULUM CODE: AAS.PTA.TRK STEPS TO COMPLETE A PTA APPLICATION INCLUDE: 1. Submit the Lake Land College Intent to Enroll form online or submit to Admissions and Records. Choose the Physical Therapist Assistant Track major (AAS.PTA.TRK). 2. Submit a completed PTA Program Application online. 3. Complete the Lake Land Compass Placement Test. Call to schedule a test time or you may submit your ACT scores in lieu of taking the Compass test. ACT scores should be reported directly to the college from ACT. 4. Submit transcripts to Admissions and Records by March 1. Up-to-date high school transcript and final transcript upon graduation. All college transcripts. 5. You must complete a pre-requisite biology class with a grade of C or better within the past five years (if you are a high school senior, you must complete the class by graduation). One year of high school biology, anatomy, or zoology, or Lake Land College Bio-Science I (BIO100) or equivalent is also required. STUDENTS ARE INFORMED OF THEIR STATUS BY APRIL. ONCE SELECTED, FINAL ADMISSION IS CONTINGENT UPON SUBMISSION OF THE FOLLOWING BY JULY 1: 1. Complete health examination and immunization form. 2. Proof of Hepatitis B vaccination series started. 3. Confirm twenty (20) hours of observation in a physical therapy setting. 10 PARAMEDICAL SERVICES CURRICULUM CODE: AAS.PS. STEPS TO COMPLETE A PARAMEDICAL* APPLICATION INCLUDE: 1. Submit Lake Land College Intent to Enroll form to Admissions and Records. Choose your academic program major as Paramedical Services (AAS.PS). 2. Complete the Lake Land Compass Placement Test. Call to schedule a test time or you may submit your ACT scores in lieu of taking the Compass test. ACT scores should be reported directly to the college from ACT. Students who test into reading will be required to take it their first semester. 3. Complete EMS/Paramedical Services paperwork in the EMS office, located in the Northwest Building, room 26, or call to schedule an appointment. 4. If you have taken college courses from another institution: Request the official transcript to be sent to Lake Land Admissions and Records. Complete the Transcript Evaluation form (in person or from the Admissions and Records website). 5. Complete (or currently be registered for) EMS 050, EMT Basic. Prerequisites are: CPR certification for the American Red Cross Professional Rescurer OR American Heart Association Healthcare Provider OR Lake Land College course EMS Complete (or currently be registered for) BIO 050 Basic Anatomy and Physiology, OR BIO 225 Human Anatomy and Physiology I.
