4 APPLYING TO THE ADN TRANSITION PROGRAM NOTE: Applicants are advised that the North Carolina Board of Nursing requires finger prints and a criminal background check prior to being issued a license. A history of felony or misdemeanor convictions may result in the denial of a license to practice nursing. NOTE: The information provided in the student medical form, criminal background check and drug screening is not considered in the admission decision. It is possible, however, that the results may prevent the applicant from participating in the clinical rotations and thus from continuing in the program. NOTE: Students with pierced ears may wear only one small post earring in each ear. Students will follow agency personnel policies regarding wearing any other body piercing/gauging ornaments. ***ALL POTENTIAL APPLICANTS MUST ATTEND A MANDATORY TRANSITION INFORMATION SESSION. (See page 1 for details). i

5 APPLYING TO THE ADN TRANSITION PROGRAM Application period begins September 2, 2015, and ends November 6, LEVEL I: MANDATORY STEPS 1-4 Unfortunately we will be unable to accommodate Level II appointments during the months of July and August. We do not schedule same day appointments and no next-day appointments will be made after 3 p.m. each day. 1. ATTEND A MANDATORY TRANSITION INFORMATION SESSION. *All LPNs interested in applying for the Transition program must attend one of these information sessions: July 29, 2015 at 10:00 a.m. OR October 7, 2015 at 3:00 p.m. 2. FILL OUT A COASTAL CAROLINA COMMUNITY COLLEGE APPLICATION FOR ADMISSION (pg.4). 3. TAKE THE PLACEMENT TEST AND RECEIVE THE MINIMUM SCORES OR HIGHER (pg.4). *Students testing at Coastal Carolina Community College must wait at least 2 days after testing for scores. Students must confirm test scores, in person, with an Academic Counselor. 4. SUBMIT OFFICIAL TRANSCRIPTS FROM HIGH SCHOOL AND ANY COLLEGE/UNIVERSITY ATTENDED (pg.4). 5. MEET ADN PRE-REQUISITES (Math and Chemistry see below). Completed Algebra II in high school with a C (77%) or higher* (Must be equal to 1 credit to qualify) OR Minimum of MAT 070, DMA 50 or a higher level transferrable College level algebraic Math from any College or University with a C (77%) or higher* (Statistics will not count as an algebraic math). AND Completed Chemistry in high school within the last 5 years (2011 or after) with a C (77%) or higher* (Must be equal to 1 credit to qualify) OR CHM 090 at a North Carolina Community College or a transferrable College level Chemistry (a minimum of 4 semester hour credits) from any College or University with a C (77%) or higher* If the applicant registers for CHM 090 or higher level chemistry and/or MAT 070, DMA 50 or a higher level math in the Fall of 2015, the Academic Counselor for Nursing must confirm that the applicant has registered for those courses before proceeding with a Level II application. Once verification of the grade has been made, the Level II application will be viewed as complete and submitted at that time. *Earning a grade of C- or lower does not satisfy the ADN pre-requisites. Students must have fully completed steps 1-5 before scheduling a Level II appointment. 1

6 LEVEL II: CALL TO MAKE AN APPOINTMENT WITH THE ACADEMIC COUNSELOR FOR NURSING 1. FILL OUT AN ASSOCIATE DEGREE IN NURSING TRANSITION APPLICATION (by appointment only. You will fill the application out in the nursing counselor s office). Application period begins September 2, 2015, and ends November 6, SUBMIT A LETTER OF REFERENCE The letter must come from a former Practical Nursing instructor or employer. 3. VERIFICATION OF EMPLOYMENT The letter must state the date and year of employment within the last three (3) years (since January 1, 2013) showing evidence that you have or will have worked full-time or the equivalent, for a minimum six (6) months as an LPN by January 1, PROVIDE A COPY OF YOUR CURRENT UNRESTRICTED NORTH CAROLINA LICENSE FROM OR COMPACT LICENSE (The name on the application must be the same name that is verifiable on the PN license). 5. SUBMIT A COPY OF YOUR CURRENT AMERICAN HEART ASSOCIATION-HEARTSAVER CPR CERTIFICATION. 6. TAKE THE TEAS EXAM (version 5) on or before October 24, 2015 (pg 6). Applicants must score equal or higher than the minimum score based on the cohort of the 2015 A.D.N accepted applicants. TEAS scores must have been earned on or after January 12, If you are unable to attend the TEAS date offered at CCCC, you may take the test at another site and have official scores sent to CCCC. UNC-Wilmington s testing center also offers the TEAS exam: 7. TAKE THE LPN STEP EXAM on or before November 6, Applicants may take the exam at CCCC on: October 14, 2015 at 9:00 a.m. (must register with by 5:00 p.m. on October 7, 2015) OR November 4, 2015 at 2:00 p.m. (must register by 5:00 p.m. on October 28, 2015) *Please contact Beth Elmore at (910) or elmoreb@coastalcarolina.edu to register for this exam. 8. FOR SPRING 2016 ADMISSION, COMPLETE THE FOLLOWING ADN GENERAL EDUCATION REQUIREMENTS BY THE END OF THE FALL 2015 SEMESTER: BIO 168 PSY 150 *Biology courses taken before Spring 2011 are considered expired for the ADN Transition program. SUBMITTING AN APPLICATION TO THE TRANSITION PROGRAM DOES NOT GUARANTEE PLACEMENT INTO THE ASSOCIATE DEGREE IN NURSING PROGRAM. THE TRANSITION PROGRAM IS A LIMITED ENROLLMENT PROGRAM. ADMISSION IS BASED ON SPACE AVAILABILITY. ALL LEVEL II REQUIREMENTS MUST BE MET BY NOV. 6, THE LPN STEP EXA, AMD TEAS SCORE WILL RANK THE STUDENTS TO DETERMINE THE BEST CANDIDATES FOR THE OPEN SEATS. ADMISSION INTO THE 2016 SPRING TRANSITION PROGRAM WILL INCLUDE A REQUIRED ORIENTATION MODULE WHICH WILL INVOLVE AN ONLINE COMPONENT AND AN ON CAMPUS ORIENTATION MEETING IN DECEMBER 2015 (date to be announced). 7 ASSOCIATE DEGREE NURSING TRANSITION ASSOCIATE IN APPLIED SCIENCE (A45120) CURRICULUM DESCRIPTION The Nursing Transition program provides currently licensed practical nurses, who have completed BIO 168 and PSY 150 or their equivalent as approved by the Registrar, eligibility for matriculation into the second semester (1 st year) of the Associate Degree Nursing Program. Upon acceptance into the program, students will be awarded 17 hours of college credit, satisfying the first semester requirements. Graduates of this program are eligible to apply to take the National Council Licensure Examination (NCLEX-RN), which is required for practice as a registered nurse. Employment opportunities include a wide variety of health care settings such as hospitals, long-term care facilities, clinics, and physician s offices. This has been identified as a limited enrollment program and may involve certain deadlines. See an academic counselor for additional information. The appropriate course work is listed below as of this printing date but may be subject to revision for the academic year. 8 KEY POINTS TO APPLYING SECTION 1: CCCC APPLICATION INFORMATION & WEBSITE Contact: (910) Students not currently taking classes at Coastal Carolina Community College need to fill out a CCCC application before applying to the Nursing Program. Applications are available on-line at OR you may pick one up in the Admissions Office. SECTION 2: TRANSCRIPTS Contact: (910) Students must submit official transcripts from high school or GED, and all post secondary colleges and schools attended, before applying to the Nursing Program. Not submitting ALL transcripts will make you ineligible for acceptance to any of the Nursing Programs. SECTION 3: PLACEMENT TESTING / SCORES Contact (910) for information on testing dates and times. All transition applicants must have current placement scores on file. Placement testing must have been taken ON or AFTER January 1, 2013 to be valid. NC DAP Reading/ English Before applying to the transition program, students must meet or exceed minimum score requirements in the three (3) subject area Reading, Writing, Mathematics. All applicants, along with the writing and reading, are required to submit a pre-algebra or arithmetic score depending on the exam taken. Students who take the NC DAP Reading/English test are required to make a minimum score of 151 to meet the placement test score requirements for the EMS program According to Coastal Carolina Community College s placement test policy, students may take the entire placement test twice within a one-year period. 4

9 SECTION 4: TEST OF ESSENTAL ACADEMIC SKILLS INFORMATION (TEAS) Coastal Carolina Community College will be offering the TEAS exam on the following days: TEAS TEST DATES REGISTRATION DEADLINE Saturday, October 24, 2015 Friday, October 16, 2015 This test is required; if an applicant does not take the TEAS exam by the specified date, they will not be eligible to apply for the 2016 Transition Program. There is a TEAS study guide as well as a TEAS practice test with rationales that can be purchased at TEAS version five (5) will be used for the 2016 Transition Program. To reserve a testing date for the TEAS, please click on the TEAS reservation link located at You will only be able to register once. If you do not attend your reserved date, you will not be able to apply for the 2016 Transition program. Applicants must set up an account online to take the TEAS test at Create a new account and fill in the information on the user information page. Make sure you set up the account by the date of your test reservation (Make sure you choose Coastal Carolina Community College ADN or PN). The TEAS test is computer administered. CREDIT CARDS ONLY will be accepted for payment on the day of the exam. The cost of the exam is $45.00 (subject to change). Please make sure you have an American Express, Discover, Visa or MasterCard available. These are the only cards that will be accepted as payment. You must use a credit card or a check card with one of above-mentioned insignias. Applicants must bring one form of identification and at least 2 credit cards in their name (in case one does not work for any reason). Coastal will not be responsible for any credit card that is not accepted. The exam will be administered at 8:00 am on all exam days in the Business Technology Building on the 2 nd floor. Applicants need to arrive at 7:30 am to begin the check in process downstairs. APPLICANTS WHO ARRIVE LATE WILL NOT BE ADMITTED IN THE TESTING ROOM ONCE THE EXAM HAS BEGUN. It is not possible to register and take the test for more than one exam date and use the higher of the two scores. The exam administered will be limited to 120 applicants. If an applicant is unable to take the TEAS on the date offered at CCCC, it is his or her responsibility to find an alternate testing site. CCCC will not individually proctor exams. Applicants taking the TEAS at another institution will be responsible for providing CCCC with an official copy of their exam scores on or before November 7,

10 Your TEAS scores are available immediately after the computerized examination is completed on your exam day. Additional transcripts for your scores may be purchased at the online store at for an additional fee. SECTION 5: FREQUENTLY ASKED QUESTIONS Q: Do I have to take the placement test even though I ve taken college English and/or college math? A: Yes. We have to have evidence that your academic skills are current before participating in any Nursing program. Q: Can I use the scores I received at a previous school? A: Yes, as long as they are COMPASS, ASSET, SAT, ACT, or ACCUPLACER scores and meet the previously specified time requirements. Additionally, the scores must be officially sent to CCCC from the school in which testing occurred. Scores cannot be older than January 1, Q: If I do not achieve the minimum score in one area, do I have to retake the whole test again? A: No, only the portion in which the score was not achieved. Q: What arrangements do students make when they stay out of town for clinical rotations? A: Students are expected to make their own arrangements for transportation, lodging, and meals when they are completing clinical rotations whether the rotations are local or out of town. Some students choose to commute but the majority buddy up and stay in area hotels. 6

11 SECTION 6: ADDITIONAL REQUIREMENTS Once accepted into the ADN program as a transition student, students will be required to: Complete a Student Medical Form demonstrating physical and emotional health Complete a Criminal Background Check information will be provided once accepted. The background check will be at the expense of the student. (Clinical agencies will review all criminal record checks and determine eligibility). Drug screening: once a student is enrolled, a mandatory random drug screening is required at the student s expense. Additional random drug screening may be required. Maintain American Heart Association Heartsaver- CPR certification. Complete all required immunizations. Purchase liability insurance annually. Travel at their own expense to complete clinical rotations. Students are expected to make their own arrangements for transportation, lodging, and meals when they are completing clinical rotations whether the rotations are local or out of town. Once enrolled in the ADN program, students must meet the following academic standards: Maintain passing grades on all general education courses of three or more credit hours. Students making a D or lower on related courses will not be allowed to progress. Maintain grades of C or higher (77% and up) for nursing courses. Students making a D or lower in a nursing course will not be allowed to progress. Master calculation of medical dosage with 90% accuracy every semester prior to clinical medication administration experience. Students who are not able to demonstrate math competency will be dismissed from the program. If testing positive for Hepatitis or HIV, applicants have the responsibility to: Notify the following office: Chief, Communicable Disease Control Section P.O. Box Raleigh, NC Readmission/Transfer Policy: A student requesting consideration for readmission or transfer must have an overall GPA of at least 2.0. The following steps are required: 1. Meet with the Nursing Department Head to discuss reasons for request, plans for successful completion, and educational goals. 2. Obtain a letter of reference addressing both academic and clinical performance from an instructor who taught the student in his or her last nursing course. 3. Meet with the Academic Counselor for Nursing to discuss requirements for readmission or transfer. 4. Once requirements for readmission or transfer have been met, contact the Academic Counselor for Nursing to complete an application for Readmission/Transfer Admission. Only one academic readmission is permitted. You may not be considered for regular admission into a program in which you are currently enrolled. 7

Move around in patients rooms, workspaces and treatment areas; administer cardio-pulmonary procedures. Calibrate and use equipment; position patients/clients. Hear monitor alarm, emergency signals, auscultatory sounds, and cries for help. Observe patient/client responses. Perform palpation, functions of physical examination and/or those related to therapeutic intervention e.g., and insertions of a catheter. 9
