3 3 JAMAICA ASSOCIATION OF PROFESSIONALS IN NUTRITION AND DIETETICS JAPINAD has been established since 2002 with over one hundred professional and para-professional members. The association is formerly affiliated to the Caribbean Association of Nutritionist and Dietitian (CANDi), the International Conference of Dietetic Association (ICDA), American Overseas Dietetic Association (AODA), and more recently the University Council of Jamaica (UCJ) accreditation committee. JAPINAD is the sole professional association representing dietetic and nutrition professionals practicing in both private and or public settings in Jamaica. The professional association has key responsibilities for developing and maintaining 1) the professional code of ethics for practicing Nutritionists and Dietitians, 2)competencies for training and qualification of entry level professionals, 3) opportunities for continuing education such as Workshops and short courses, and 4) active advocacy for improve work related conditions and compensation for its members, while safeguarding the delivery of nutrition information both by the professional and to the client. It is also the prerogative of JAPINAD to publish documents relating to standards of practice, position statements on controversial and topical issues, management guidelines for nutrition conditions, curriculum and competencies for training and development in specialized area of nutrition and dietetics such as supervised practice, sport nutrition, and diabetes education. The professional association has a track record for involvement in the development of national policies such as the Jamaica Breastfeeding Policy and Baby Friendly Hospital Initiative, the National School Feeding Policy, the Jamaica Food and Nutrition Security Policy and Action Plan, and the CARICOM Regional Food and Nutrition Security Policy, the Pan American Health Organization special Committee on Recommended Dietary Allowance for Caribbean populations. JAPINAD also represents nutrition and dietetics professionals on the Council for Professions Supplementary to Medicine. As a member of the CPSM, the association assess and decides on application for professional registration, monitor for infringement of professional code of ethics, recommend new guidelines and amend outdated policies for nutrition and dietetics as well as safeguard the integrity of the profession through evaluation of continuing professional education credits as a part of yearly re-registration.

4 4 DEFINITION AND SCOPE OF WORK Nutrition and Dietetics Nutritionists and Dietitians are Nutrition Scientists who apply scientific principles of nutrition to guide feeding, eating and utilization of food components to ensure the nutritional health of individuals, groups and populations. More specifically, they conduct clinical, dietary and ecological assessments in order to determine, implement and evaluate dietary prescriptions and/or programme plans to: 1. Ensure individuals attain physical fitness and optimum health through being properly nourished; 2. Implement medical nutrition therapy (MNT) to facilitate wellness, and recovery from illness and/or nutrition related diseases; 3. Guide policies, research, and measures at institutions, community groups and for populations to prevent malnutrition (under-nutrition and obesity) and reduce the incidence of these and related chronic illnesses such as diabetes, cardiovascular diseases, HIV/AIDS, and other disorders of modern society. Professional Training and Practice: Nutritionists and Dietitians are professionals with a minimum training of a Bachelor of Science degree specializing in Nutrition Science. This is followed by supervised practice/ internship for dietitians, or a practicum and/or research project in the case of nutritionists. This equips them to provide evidencebased information and guidance about the impact of food policies, the national and international food environment, food chemical components and bodily interactions that affect the health and wellbeing of humans. They offer these services by applying a standardized nutrition care process or protocol (NCP) that includes conducting nutrition assessments, formulating nutrition diagnosis, prescribing appropriate diet/nutrition therapy or intervention such as nutrition education and/or counseling; foodservice systems management in hospitals, in the school system, at workplaces and in other institutions and programmes in community settings. Dietitians are integral members of the medical team with responsibility primarily for the secondary and tertiary nutritional care of individuals and groups in collaboration with physicians and other health care professionals in institutions and/or the community. Nutritionists are integral members of the medical and multi-sectoral teams that may include the government sectors of health, agriculture, education, trade and commerce, among others. Nutritionists play a pertinent role in targeted individual, group or population based programmes aimed at promoting health and the primary and secondary prevention of diet-related illnesses. Nutrition Assistants and Dietetic Assistants are para-professionals trained at the technician level through an associate degree or undergraduate diploma in dietetics (3 years didactic + supervised practice). They function under the supervision of a registered dietitian or nutritionist contributing to the implementation of secondary and tertiary individualized nutrition care plans in institutions and primary and secondary care nutrition intervention programmes in population and/or targeted groups.

5 Career Options A wide range of career opportunities is open to professional nutrition scientists, and dietetic/nutrition technicians in government, private sector, non-governmental organizations or independent practice. Many are involved in Health, Food Productivity Management, Agriculture Extension services, Food Industry in Marketing and Food Science Research, Education at all levels, Pharmaceutical Industry, Nutrition Research, Food Standards and Policies in nutrition related technical agencies, in Business Entrepreneurship, Development Programmes in Government or International agencies, Private Practice, Wellness and Media Consultancy, School Nutrition, and Sports nutrition. 5

6 6 QUALIFICATION AND TRAINING The Jamaica Association of Professionals in Nutrition and Dietetics( herein referred to as the professional association ) and its stakeholders proposes a competency based supervised practice modality for both the theoretical and experiential learning component of training programmes aimed at producing entry level Nutritionist and Dietitians. The competency-based education model supports lifelong learning throughout the various stages of professional practice (figure 1). 7 7 Definitions Supervised Practice: A time limited period in which the professional in training attains knowledge and skills in a real world setting under the supervision of one or more qualified preceptor in preparation for meeting the competencies of an entry level Nutritionist or Dietitian. The period of supervised practice can be obtained within a didactic programme (minimum of 4 years) or in a post- degree preceptorship programme. The overall focus of preceptorship is to ensure that, by the end of the period under supervision, the professional in training is able to provide competent and ethical professional services, in Nutrition and Dietetics. A comprehensive evaluation of the professional in training is required at the end of the preceptorship period. The evaluation should be based on the professional competencies for entry level Nutritionist and Dietitians provided by the professional association (Appendix 1). For the post degree preceptorship programme, the applicant will be required to declare area of practice as Nutritionist or Dietitian (temporarily restricted due to resources available for placement). Preceptor: Professionals qualified to provide appropriate learning experience, mentorship, coaching and assessment of trainee. The preceptor is an accomplished professional in their field with front-line experience in the practice of nutrition and dietetics. The preceptor s aim should be to facilitate the professional in training to develop and enhance: Competence- Expertise in the field knowledge, skills and application Self Confidence- belief in one s ability to make good decisions, to be responsible and to be in control Self-Direction- the assurance and ability to chart a path of professional and career development Professionalism- to understand and assume the responsibilities and ethics of the nutrition and dietetics profession Preceptors should be guided by specific guidelines developed by training institutions with input from the professional association where necessary. The proposal will also make provision for three categories (figure 2): 1. Coordinated Undergraduate Programme 2. Post Degree Preceptorship

Post Graduate Degree Programme Training Programmes Coordinated undergraduate programme The Coordinated Undergraduate Program (CUP) includes both the required foundation knowledge (academic courses) and competencies (supervised practice) in a single degree granting programme. The minimum period of 4 years including summers is required for coordinated programmes. Requirements for the supervised practice component 1. A minimum 500 hrs of direct supervision by qualified preceptor in covering entry level competencies 2. Placement organized by training institution using standards developed by the professional association (competency checklist, qualified preceptor list and approved supervised practice setting). 3. Preceptor handbook or guidelines developed by the institution with the input of the professional association where necessary. 4. Student handbook for supervised practice- including areas of rotations and division of hours for supervised practice 5. Evaluation of student by one of more preceptors using entry level competency assessment tool provided by the professional association. 6. Letter or certificate of completion for period of supervised practice.

9 9 Post-Degree Preceptorship The Post-Degree Preceptorship (PDP) is a post-baccalaureate programme that focuses only on the required competencies (supervised practice) and is eligible only to students who have already completed academic coursework as part of a baccalaureate degree. This programme will be jointly administered by the training institutions and professional association. Structure 1. Verification and assessment of baccalaureate level competencies Training institutions will evaluate competencies achieved by students at the baccalaureate level and provide a diagnostic report for placement during supervised practice. The institution may or may not require a fee for this service. 2. Application for placement with qualified preceptor The student is required to complete supervised practice application form and provide certified copies of baccalaureate degree and diagnostic report from an accredited university to the professional association The professional association will convene quarterly meetings of the Accreditation Committee for Education in Nutrition and Dietetics (ACEND) to discuss application and assign student to preceptors. Applicants are required to declare area of practice (nutrition or dietetics) before being placed in supervised practice setting 3. Supervised practice setting Students will be assigned to preceptors according to declared area of practice and availability of preceptor Setting to include government operated primary, secondary and tertiary facilities, private practice, nutrition related private sector industry Students may be required to rotate through more than one setting in order to achieve the necessary competencies 4. Evaluation

10 Preceptor to provide evaluation report upon student completion of supervised practice using the entry level competency assessment tool. More than one evaluation report may be generated where supervised practice was completed in multiple setting. The ACEND will be responsible for collating reports. 10 Requirements for supervised practice within PDP 1. Placement organized by professional association using recommended standards (competency checklist, qualified preceptor list and approved supervised practice setting). 3. Evaluation of student by one of more preceptors using entry level competency assessment tool provided by the professional association. 6. Letter or certificate of completion for period of supervised practice.

11 11 Post Graduate Degree Programme Higher level degrees meeting the required foundation knowledge (academic courses) in nutrition and dietetics (Appendix 2). The competencies (supervised practice) must be attained through a post degree supervised practice as described above. Remuneration Interns/ Professionals in supervised practice The government of Jamaica is expected to provide a no strings attached stipend to participants enrolled in supervised practice in the public sector setting ONLY. Eligibility for stipend requires completion of the didactic component of 4 years baccalaureate programme for those enrolled in a coordinated undergraduate programme or a baccalaureate degree for those enrolled in a post-degree preceptorship programme. The provision of a stipend should NOT be considered a certainty of employment within the public sector as stipends will be award for no longer than a period of 48 weeks or end of supervised practice programme, whichever comes first. Stipends shall be paid monthly by the ministry of health Jamaica or through its regional health authorities Stipends shall not surpass or be less than basic salary for entry level Nutritionist or Dietitian within the public sector Preceptors There shall be no remuneration or compensation for preceptors working within the public sector.

COMPETENCIES FOR NUTRITIONISTS & DIETITIANS GRADE COMMENTS Objective 1: To emphasize the importance of character and ethical behaviour essential to effective communication behaviours and professional performance 1. Demonstrate and practice ethical behaviour as defined by the Code of Ethics for the Dietetics/Nutrition Profession. 2. Perform self-assessment, participate in professional development activities and keep abreast of new developments in the fields of health and nutrition. 3. Actively promote the interest of the profession through professional development activities and networking with other disciplines. 4. Practices within scope of education, training and competency and consult with other professionals when a situation is beyond one s level or area of competency. 5. Apply knowledge of policy and politics to make evidence based recommendations through written and or verbal forums and to promote good nutrition and be an advocate for consumers on nutrition issues. 6. Advocate for changes to legislative and public policies as they affect food security and the nutritional well-being of the public/consumers. 7. Utilise the reflective practice approach in self-development and practice. Objective 2: To apply education and communication skills to the practice of Nutrition 8. Practically use current technological innovations (PowerPoint, sms, web applications) for information sharing and communicating with clients/patients and other members of the health care team. 9. Interpret and use scientific information in nutrition and dietetics practice.

Objective 3: To apply management skills to the practice of Nutrition 15. Assist in the development of staff training and continuing education opportunities towards the delivery of optimal nutrition care. 16. Effectively use physical, material and financial resources. 17. Assist in the development and implementation of operation standards and management guidelines for nutrition services. 18. Demonstrate impact of nutrition services through cost-benefit analyses. 19. Contribute to the development of plans, policies, proposals and budgets in relation to nutrition practice and research at the community level 20. Manage, monitor and evaluate programmes and projects. 21. Conduct nutrition needs assessment and evaluation of services to ensure needs are met and services are compatible with planned delivery at the community level. Objective 4: To utilise research skills to the practice of Nutrition 22. Demonstrate skills for designing and conducting food and nutrition research activities including collecting quantitative and qualitative data that affects community nutrition status and analyse data using appropriate technologies. 23. Assist in procurement, production, distribution and service within different food facilities and environments. 29. Apply knowledge of human resource management to food systems management. 30. Develop plans for the integration of financial, human, physical and material resources. 31. Supervise recipe development and product designs and redesigns. 32. Evaluate procedures and conduct audits for food procurement, storage, delivery and production systems to ensure that they meet nutrition requirements, cost parameters, labelling & safety standards and consumer acceptance. 33. Collaborate with relevant agencies to ensure that production of food meets guidelines for nutrition, cost parameters, labelling &safety standards and consumer acceptance at the community level 34.
