2 Request for Proposal Specifications 1. Overview Achievement Prep is a network of high-performing college preparatory schools located east of the Anacostia River in Ward 8 of Washington, DC. Founded to close the achievement gap and address the educational needs in the community. Achievement Prep has been an award-winning public charter school recognized for its immediate impact in closing the achievement gap between low-income and affluent students in Washington, DC. Achievement Prep is soliciting proposals from qualified vendors for Special Education and Mental Health Services. 2. RFP Process and Instructions a. Schedule & Deadlines Achievement Prep proposes the following scheduled for submission and review of contracts. RFP Released Friday, June 12 th, 2015 Responses Due Friday, June 26 th, 2015, 5:00 EST Service Start Date Wednesday, July 1 st, 2015 b. Submission of Proposals Proposals must be received via only by 5:00pm, Friday, June 26, Proposals received after 5:01pm, Friday, June 26, 2015 will not be considered. Please send proposals as PDF attachments to bids@achievementprep.org. Achievement Prep will not be liable for any cost incurred by respondents in the preparation and production of responses to RFPs or in the negotiations associated with award of contract. c. Evaluation of Proposals Proposals will evaluate complete proposals and recommend awards based on prior experience, price, and qualifications. d. Contract Period The initial contract period is July 1, 2015 June 30, Each RFP presented has the right to extend the term of the contract up to three (3) one-year terms. Subsequent contract periods would be July 1, 2016 June 30, 2017; July 1, 2017 June 30, 2018; and July 1, 2018 June 30, 2019.

Board of Trustee and Public Charter School Board Approval Upon approval of the contract by Achievement Prep s Board of Trustees, Achievement Prep is then required to submit the contract to the DC Public Charter School Board for approval. A contract between Achievement Prep and the selected vendor shall not exist until an approval by the Board of Trustees and the DC Public Charter School Board. f. Contract Award Achievement Prep reserves the right to cancel or modify a contract award for any reason without penalty or fee. The awarded proposal and a copy of this RFP shall be made part of the contract by reference and attachment. It is understood and accepted by both parties that in the event of a conflict of language between this RFP and the contract, the RFP language shall be considered superior. 3. RFP sites a. Sites of work include the Wahler Place Middle School Campus located at 908 Wahler Place, SE and the Mississippi Avenue Elementary School Campus located at 1500 Mississippi Avenue, SE. The Network Office will be located at the Wahler Place Middle School Campus. Sites will be designated within the scope of work. 4. Scope of Work a. Special Education Services. The vendor of special education services will perform the following services; consultative and psychological assessment services, maintain telephone and personal contact with clients, and perform all services in accordance with professional standards set by the American Psychological Association. Scope of services includes the following evaluations; comprehensive psychological, neuropsychological, psychoeducational, clinical, ADHD, as well as, functional behavior assessment, and behavioral intervention plan. All evaluations services shall include; data collection, clinical interview, administration of assessment tools, scoring and interpreting results, generating a report and MDT/I.E.P meeting. Evaluations may also include, other assessment protocols and functional behavioral assessments. Assessment data in all evaluation services shall include; classroom observation, parent interview, child or adolescent clinical interview, teacher referral information, teacher(s) consultation, and school records review. Standard evaluation reports shall include; background history, clinical interview, analysis and integration of assessment data, case conceptualization summary, realistic and practical recommendations emphasizing the child s or adolescent s strengths while identifying strategies and resources to address less developed skills and specific referrals for intervention services.

4 b. Mental Health and Related Services. The vendor of mental health and related services will perform the following services: support a safer and more supportive school environment; provide professional development for staff on a variety of behavioral health topics; perform case management for scholars; and be on hand in the aftermath of traumatic events effecting the school environment. Scope of services includes, but is not limited to, the following: prevention, early intervention, clinician services, speech and language services, and occupational therapy. Services shall address a range of mental health and psychological concerns related to the scholars served, including but not limited to: school adjustment and attendance problems, cases of abuse, and emotional difficulties. Service providers will complete mental health screenings for scholars, including identifying at-risk scholars for further evaluations. Service providers will also be able to provide certified and licensed speech and language pathologists and occupational therapists to help scholars fully participate in their learning. Additionally, programs developed by service providers will be school-based and for the purpose of crisis intervention and prevention, treatment, and promotion of positive social and emotional development. Programs developed will embed mental health services in the culture of the school. c. IT Services. The vendor of information technology (IT) services will perform the following services: network management and infrastructure support; system monitoring and response; network security; management of network backup software; help desk support; software support; hardware management and support; application management and support; remote communications support; on-site support for services that require in-person attention; system and application documentation; license, warranty, and asset tracking; ongoing IT enhancement consultation; and quarterly reporting on system monitoring, help desk activities, software updates, and patch management. Scope of services includes, but is not limited to, the following: desktop application support; network administration services; server and workstation administration services; , security, and backup efforts; and planning for major system enhancements and/or upgrades to existing systems. Services shall address all of the IT needs of a growing network of schools, including recommendations for future purchasing and technology needs. Service providers will ensure that help desk support is available during school hours and on-site enhanced troubleshooting is scheduled as needed. Service providers will also maintain a data backup policy, with procedures in place to handle daily, weekly, and monthly backup of and computer data and information.

5 Additionally, service providers will proactively monitor network equipment, including performance indicators to report on threshold limitations and maintain network documentation for daily, weekly, and monthly services. 5. Reimbursement Process Vendor must submit invoice for payment no later than ten (10) days within completing service. Payment will be made within thirty (30) days.
