12 Dental Plan - Nevada The Anthem Blue PPO dental plan covers: Most preventive and diagnostic services (exams, cleanings and X-rays) at 100% when members see an in-network dentist, starting on Day 1 of the policy Basic services (fillings) after a six-month waiting period Major services (crowns, etc.) after a 12-month waiting period Plan pays a set amount for each basic/major service, and is the same whether members see an in-network or out-of-network dentist. Member is billed the difference. $50 annual deductible (up to $150/family) Annual maximum of $1,000 This product leverages the dental PPO network 12
13 Tonik Enhanced The Tonik Enhanced plan is a buy-up to the embedded Tonik dental plan and covers: Basic services (in addition to those covered under embedded Tonik) after a six-month waiting period does not cover fillings Plan pays 80% of charges for basic services and 50% for major services when a member sees an in-network dentist Plan pays a set amount for each basic/major service when members see an out-of-network dentist. Member is billed the difference Annual maximum of $1,000 This product leverages the dental PPO network 13

29 Selling Term Life Can Add To The Bottom Line Individual life commission = 20% first year, 10% renewals EXAMPLE Selling five $50,000 policies to 45 year olds at $25/month premium would yield $1,500 in annual premium. 20% of that is $300. Brokers who sell just five policies a month would earn an extra $3,600 a year! And don t forget, every year they renew, you get another $1,800! 29
30 Selling Dental Can Add To The Bottom Line Individual dental commission = 10% EXAMPLE Selling five Anthem Blue Dental PPO members at $36.91/month would yield $2,215 in annual premium. 10% of that is $222. If you sell just five dental members a month, you would earn an extra $2,664 a year! And don t forget, every year they renew, you get the same amount! 30
