2 2 Everest College, Associates Degree of Nursing (Accelerated full-time 2 year program) Thank you for your interest in the Associate Degree (A.A.S) Nursing Program at Everest College Henderson Campus. Upon successful completion of the Program, graduates will be eligible to take the National Council Licensure Examination for Registered Nurses (NCLEX-RN). This Information Packet provides the most current information about the Nursing Program and the required policies and procedures for acceptance and admission to the Program. Everest College offers a two (2) year accelerated full-time (classes during the day, clinical times may vary) Nursing Program. The academic year is divided into four (4) twelve (12) week quarters with a week off between quarters (exception is Christmas week). The Program is eight (8) quarters in length. Nursing and General Education courses are offered together starting in Quarter One (1). Nursing courses progress from basic nursing care to more complex patient care. Students are selected for admission to the Program two (2) times per year. New Class Starts begin both in January and July. *Some general education (no science or nursing courses) classes are offered on-line they may vary per quarter.* It is recommended that you take Algebra on-ground in the classroom. Consult the library for the courses (Computer, Algebra, Gen. Psych. & Soc.) that can be tested out of by a proficiency exam without having the credits. The Student Success Program (SSP) is a means provided by Everest School of Nursing for the student to reinforce key nursing learning activities. The SSP is available up to three hours each week and is staffed by a nursing faculty member. The faculty is there to assist students to remediate learning activities either identified by the student or a faculty or clinical staff member. Learning activities in the SSP cover a variety of topics from the classroom to clinical areas. SSP is required of all students at the beginning of every Quarter when enrolled in nursing courses. Once the first quiz and/or assignment is graded, students who earn an 84% or higher are excused from SSP as long as that grade is maintained. If an 84% is not achieved on a quiz, assignment, and/or exam, the student must return to or remain in SSP. Students who achieve a grade of 84% or higher can still attend SSP to reinforce learning experiences. Plan ahead: Obtain official (sealed) high school OR GED certificate & College Transcripts if you are going to transfer in a course. Review the prep sheets for the Wonderlic & HESI exams; start FAFSA (school code is ). Take exams early on in the process! 4 4 TAKE THE STEPS TO BECOME A REGISTERED NURSE (RN) PRE-SCREEN REQUIREMENTS: 1. Attend a Nursing Program Information Session on the Everest College - Henderson Campus: All prospective Nursing Program applicants are required to attend a ONE-HOUR Information Session. Information about the Nursing Program and the required policies and procedures for acceptance and admission to the Program will be presented at the session along with a tour of the facilities. Offered M and F at 11am (10:30am meeting with Adm Rep). *Note that Everest College-Henderson Campus also offers the following other Programs of Study: Diploma Programs: Medical Administrative Assistant Associate Degree Programs: Accounting, Business, Criminal Justice, Paralegal and Medical Insurance Billing & Coding. 2. The applicant must be a high school graduate or have a GED certificate: Applicants educated in high school in a foreign country must have their transcripts translated and evaluated by an official agency approved by Everest College. The foreign high school education must be certified that it is equivalent to high school education and graduation in the United States. Official sealed transcripts must be presented prior to taking the HESI exam. Official Transcripts must be in a sealed envelope from the school or institution. Students can have the transcripts sent to them at their residence address in the sealed envelope by their school and then the student is responsible to bring the transcripts to Everest College prior to taking the HESI exam. 3. Pass the Wonderlic: (Prep Sheet available) Take The Wonderlic Basic Skills Test (Version 1) over two twenty-minute time frames that consist of two components: Verbal and Quantitative. Governmental issued picture ID is required at time of test such as a driver s license or passport. A score of at least 290 is needed on each section to move to the next step. There are no test fees for the student, Everest College will pick-up the cost for this. Time commitment: One hour. Applicants are notified of their scores by a nursing program representative upon completion of the Wonderlic exam. If an applicant does not pass both sections then the applicant can retest (Version 2) with extensive documented remediation practice at the next available nursing start (wait period may be up to 6 months).. Students Check List: Make sure to bring the following: Students are required to bring a government issued picture ID card (license or passport) Have read the Wonderlic Prep Sheet Wonderlic Testing Schedule Wonderlic tests will be offered starting March 7-June 1 Room #103 Computer Lab o Wednesdays 9am no need to schedule *Please arrive 15 minutes early to check in for the test*--if late you will not be allowed to test. 4. Pass the HESI A2 Admissions Examination Assessment: ( Study prior to taking the HESI A2 Admissions Examination Assessment. Governmental issued picture ID is required at time of test such as a driver s license or passport plus Official sealed High school transcripts/ged Certificate must be presented prior to taking the HESI exam. Official Transcripts must be in a sealed envelope from the school or institution. Students can have the transcripts sent to them in the sealed envelope by the school and then the student is responsible to bring the transcripts to Everest College. Take the HESI (Version 1) over a four-hour time span that consists of four (4) academic exams, math, reading, comprehension, grammar, vocabulary, critical thinking and a personality profile that gives hints on how to be successful in school, a learning style inventory that helps identify the strategies for

5 5 learning, and a critical thinking test that evaluates problem solving in common situations. Also, at this time A&P HESI is given if transferring in A&P (80%) score with only one version. The following tests (personality profile, learning style inventory, and critical thinking) are required, but do not influence admissions status. However, an applicant must score a minimum of 70% in each of the following four (4) academic categories in order to pass HESI: Vocabulary and General Knowledge, Grammar, Reading Comprehension, and Math. If an applicant does not pass the HESI (Version 1) Assessment, the applicant may take the HESI (Version 2) Assessment and retest within a minimum of one week. The student must complete the remediation log given to the student at the time of the version 1 test. The student can retest if an earned score of 60% to 69% was achieved in only two (2) of the above academic categories. If the second score in an academic category falls below the minimum required of 70% in HESI (Version 2), the applicant can reapply and retest for the next available nursing start process after extensive remediate is done with documentation to support this. There is no test fee for the student since Everest College will pick up the test fees for this. Time commitment: 4 hours most students complete in less than 3 hours. Applicants are notified of their scores by a Nursing Program Representative upon completion of the HESI Assessment. HESI scores are valid for only one (1) year for admission to the Nursing Program. *Extensive remediation documentation consists of date, time, and practice work done.* Students Check List: Make sure to bring the following: Government issued picture ID card to exam Official sealed High school Transcripts or GED certificate HESI Login and Password (Prep sheet instructions on how to do this) HESI tests will be offered starting March 7-June 1 Room #103 Computer Lab o Wednesdays 10:30am-2:30pm no need to schedule *Please arrive 15 minutes early to check in for the test*--if late you will not be allowed to test. Can take HESI on the same day as Wonderlic since most students complete within 3 hours. 5. Complete the Essay Component and Interview: (Prep Sheet available) The applicant will make an appointment with the Nursing Program Department in order to complete the essay component and then the interview to follow. The applicant will complete the Essay Component on a Computer within 90 minutes. The question will be a randomly selected essay question(s) that will be typed on site. The 30 minute personal interview will be with the Nursing Department. Time commitment: 2 hours Students Check List: Make sure to bring the following: College transcripts- Only if transferring in college credits (Official College transcripts are required). Official Transcripts must be in a sealed envelope from the school or institution. Students can have the transcripts sent to them in the sealed envelope by the school and then the student is responsible to bring the transcripts to Everest College. Note that proficiency exams can be taken for the following courses: Computers, Algebra, General Psychology and Sociology prior to sitting in the class and with a passing score. Please see the library for further details. Previous allied health education experience (Bring a letter of recommendation with dates of employment and supervisors name and contact information) Any former or continuing CCI students provide a signed letter from their program chair to show good standing status. First Come-First Served Basis based upon eligibility.

6 6 The first eligible 40 students will be selected by notified. A conditional acceptance letter provided. ADMISSIONS PROCESS: 6. Complete the Everest College Admissions Application: The selected student verified by a conditional acceptance letter will make an appointment with the Admissions Manager to complete the Everest College Admissions Application. 7. Complete the Preliminary Financial Aid Overview: The Admissions Rep will arrange a time for the applicant to meet with a Financial Aid Representative to complete the Preliminary Financial Aid overview and obtain an estimate of the program costs. Everest College has a variety of financial aid and payment options available for individuals applying to the Nursing Program. 8. Financial Aid Ready to Start : Bring your pink F.A. copy sheet to the Nursing Dept. that states you are Ready to Start / 9. A Passed Level II Background Check: No charge to student. The applicant will complete/ sign an Authorization Form for a Level II Criminal History Background Check (no charge to the student) to be conducted which includes the following: 10-year criminal history, nationwide sex offender registry check, OIG/GSA combined search, FACIS level 3 search, Federal Criminal check and eligibility determination. If an applicant has a conviction, pending case, or uncompleted deferral/diversion of a crime related to various offenses, the applicant s admission to the Nursing Program at Everest College may be at risl/ It is the student s responsibility to resolve any issues that come up on their background check. Contact Trevor at NV Expungement Svc, for further guidance. Refer to the document titled- Information on Criminal Conviction for People interested in a Nursing Career in Nevada available on the Nevada Board of Nursing website and the Everest College Nursing Department website. Important points: 1. Even if you have been told a conviction has been expunged, sealed, dismissed, dropped, closed, etc it may still show up, you could have been convicted even if you didn t spend any time in jail, criminal convictions include misdemeanors and felonies. If you answer NO on the interview application to the nursing program and we find out you have a conviction your admission will be denied as a fraudulent application. The Campus Nursing Director will be informed of the applicant s results. The applicant must have a PASSED the Background Check. No Exceptions such as Cleared paperwork, etc 10. A Passed drug screen at a random assigned time (48 hours to complete this) at a determined location (Paperwork form provided). No charge to the student since Everest College will pay for this. Beware that random and for-cause drug testing will be done throughout the nursing program. Note you are responsible for the cost involved for for-cause drug testing. 11. Attend a mandatory campus & nursing orientation: Tentatively scheduled for Thursday June 28, 2012 from 8am-5pm is Nursing Orientation. Students who are selecte d into the program will receive more information. Prior to Clinical: (forms available on

7 7 12. Attendance standards: For success it is important that the student is present the whole time for EVERY class session. It is important that the student works no more than part-time with a very flexible work schedule and reliable transportation. Note that after 2 absences in one quarter the student will be placed on clinical probation until the end of that quarter. While on probation the student must maintain perfect attendance or they will be dismissed. 13. Physical Examination by a licensed health care provider on the forms provided (Paperwork provided) and a signed Essential Skills and Functional Abilities Form: Paid by the student. 14. Up-to-date immunizations on the forms provided (Paperwork provided) are required by the clinical agencies: Paid by the student with cost varying depending on needs. 15. CPR card Hands-on Basic Life Support (Infant, child and adult with choking and AED) that is from the American Heart Association (not the Red Cross or an on-line card) good for the complete time the student is in the program. Paid by the student if class date/time doesn t work for the student. 16. Copy of Health Insurance Card or signed waiver Obtaining RN Status: 17. Complete Nursing Program with a graduation diploma and transcripts 18. Passed Fingerprinting results: The following questions are asked on the Nevada Board application: a. Has your occupational or professional license or privilege to practice, registration, or certificate of any level ever had any disciplinary action taken or initiated against it in any jurisdiction, or have you participated or are you currently participating in a nondisciplinary program? (Does not include driver's license or car registration) b. Have you ever had a criminal conviction, including a misdemeanor or felony, or had a civil judgment rendered against you? Have you ever been arrested? c. Do you currently use chemical substances in any way which impairs or limits your ability to practice the full scope of nursing? d. Are you currently in recovery for chemical dependency, chemical abuse or addiction? e. Do you currently have a medical or psychiatric/mental health condition which in any way impairs or limits your ability to practice the full scope of nursing? 19. Passed NCLEX-RN Board Exam 20. *OPTION: Continue on for your BSN Currently Everest College has articulation agreements with University of Phoenix and Kaplan.
