Allied Health Bldg., Room N i Houston, Texas Houston, Texas ext or SAN JACINTO COLLEGE VOCATIONAL NURSING PROGRAM INFORMATION The Vocational Nursing (VN) program within the San Jacinto College District (SJCD) is a one-year training program, leading to a Certificate of Technology in Vocational Nursing. The Vocational Nursing curriculum includes a combination of class lectures, nursing skills training, and clinical experience in a variety of healthcare settings throughout the Houston metropolitan area. The program includes 576 classroom and 880 laboratory/clinical hours. Parking fees may be required at certain clinical facilities and may range from $2 - $15 per day. The Texas Higher Education Coordinating Board and the Texas Board of Nursing approves the Vocational Nursing program. Upon successful completion of the VN program, graduates may apply to the Texas Board of Nursing to take the National Council Licensure Examination (NCLEX-PN) to qualify as a Licensed Vocational Nurse. You may contact the BON at their website or you can call them at (512) SELECTION PROCESS Applicants must meet eligibility requirements; complete all appropriate paperwork and pre-entrance examination in order to be considered for acceptance. Upon completion of all requirements, applicants will be notified and scheduled to complete the nursing admission assessment (HESI A-2) examination. Qualified applicants will be considered for selection into the program based upon the nursing admission assessment (HESI A-2) test scores. The HESI A-2 examination has an estimated fee of $ It is the policy of San Jacinto College Nursing Program to select students entering the Vocational Nursing Program based on a pre-entrance nursing exam. 2 The admission entrance exam: HESI A2 The HESIA2 Entrance Exam will be administered at the conclusion of the application period Applicants will be given information on test dates/times during the application process and may not reschedule exam date The exam must be taken at a SJCD campus, preferably North Campus and include: Reading Comprehension; Grammar, Vocabulary & General Knowledge; and Math. The Scores are good for 1 calendar year The minimum score acceptable is 70, in each category During the time of application, the prospective student will have the opportunity to determine if he/she will use a previous test score (within 1 calendar year) or will re-test. Once determination is made, the prospective student may not make changes. All Applicants will be ranked according to the Cumulative Score, with admission offered to the applicants with the highest percentage After all candidates have taken the pre-entrance exam, selected candidates will: Receive an official acceptance letter from the Director of Nursing Attend a mandatory orientation session Show proof of completing required immunizations, passing physical exam/drug screen and show proof of American Heart Association (AHA) Health Care Provider CPR (BLS) Applicants are highly encouraged to begin the immunization process as soon as possible. All supporting documentation will be required during orientation. Candidates failing to abide by policy will not be allowed admission into the program. Applicants who are not selected for admission will: Receive an official letter from the Director of Nursing Be given the opportunity to pick-up information submitted for consideration May schedule an appointment with the Director of Nursing for additional counseling

3 APPLICATION PROCESS The following checklist is provided to assist you in completing the steps to apply for admission to SJCD and the Vocational Nursing Program. It is your responsibility to make sure that the office of Enrollment Services receives your college application, high school transcript, college transcripts, and test scores. Submit a completed San Jacinto College application to the Enrollment Services office. This can be done online at or or in person at the Enrollment Services office on each respective campus. All prospective students must be accepted for admission to San Jacinto College before they are eligible to apply to the Vocational Nursing Program. Note: Admission to San Jacinto College does not guarantee admission to the Vocational Nursing Program. Submit official transcripts to the Enrollment Services office. High School students must show High School graduation or successful completion of all sections of the GED. Foreign students must submit an evaluation of their High School and/or College instruction from their home land. The evaluation must be evaluated by a San Jacinto College District approved evaluation site. You may obtain a list of approved evaluation sites from the Enrollment Services office. College Transcripts students must submit all previous and current official college transcripts to the Enrollment Services office. You may at that time request a copy of the transcript that you are submitting. *Cumulative grade point average, (GPA) of 2.25 is required. Attend the mandatory San Jacinto College new student orientation. (Only First Time Students in college.) Contact an Advisor in the Counseling Center regarding any admission/placement testing that may be needed. If you have already completed your college placement test you may have an Advisor review your test scores in order to verify skill levels. Once verified your Advisor will issue you a skill level verification form to bring with you to the VN information session. Skills levels must be a minimum of 7 in reading, 7 in math, and 7 in writing. Remediation is recommended for any area, (reading, writing, and math), if skill levels are below required levels. You may contact an Advisor for details in the Counseling Center on each respective campus. 4 Vocational Nursing Application Periods Fall 2013 Semester: April 2, 2013 May 30, 2013 Spring 2014 Semester: September 3, 2013 October 31, 2013 Applications will be distributed during Information Sessions that will be held on Tuesday & 1:00pm thru 4:00 pm in the North Campus VN office located on the 2 nd floor of the Science/Allied Health Building., Room N i All required documents MUST be submitted as a complete packet during an Information Session in order to be considered for selection. You must supply the copies the VN department will not make copies for you. Incomplete or late applications will not be accepted. REQUIRED DOCUMENTATION: 1. Copy of High School Transcript/GED/equivalent 2. Original copy of Skill Level Verification Form 3. Copies of all College Transcripts Must have a cumulative GPA of (If you ve attended more than one college we will need to review all transcripts.) REQUIRED DOCUMENTATION UPON ACCEPTANCE: Please fill in the dates of your current immunization status and submit during the application process. You will be required to submit proof of compliance upon admission to the program. Client care may not be performed until immunization status is confirmed as complete. The college may elect to use a 3 rd -party to collect data. 4. 5 IMPORTANT NOTICE REGARDING HEPATITIS B REQUIREMENT All students who are accepted into the Vocational Nursing program must document that they have received the full series of three (3) Hepatitis B vaccinations, or have the serum titer drawn to confirm complete immunity to Hepatitis B, prior to clinical. The series takes approximately seven (7) months to complete. Applicants are encouraged to begin the series well ahead of the desired semester of admission. IMPORTANT NOTICE REGARDING CRIMINAL BACKGROUND CHECK & DRUG SCREEN Students who are tentatively accepted into the Vocational Nursing Program must pass a criminal background check and applicants may possibly receive a request to complete and pass a drug screen. Information about this requirement will be discussed in the information session. The program may elect to use a third-party provider for services. Information regarding background checks and drug screens will be provided in the acceptance letter. Note: Effective January 1, 1996, a person who has been convicted of a felony may be disqualified from obtaining licensure as a Licensed Vocational Nurse. If you have any questions regarding past incidents, please call the Texas Board of Nursing at (512) Since August 2004, a criminal background check and/or drug screen has been conducted by a designated investigative agency at the expense of the student. Effective January 2008 this background check must be completed before registering for classes and only those applicants with a clear background will be eligible to register. Licensure All students seeking licensure as a license vocational nurse must pass the NCLEX-PN administered by the Board of Nursing. The Board may delay or refuse to admit persons to their examination or refuse to issue a license or certificate of registration for any of the following reasons: 1) For any criminal offense, including those pending appeal, have you: a. Been convicted of a misdemeanor? b. Been convicted of a felony? c. Pled nolo contendere, no contest, or guilty? d. Received deferred adjudication? e. Been placed on community supervision or court- ordered probation, whether or not adjudicated guilty? f. Been sentenced to serve jail or prison time? Court- ordered confinement? g. Been granted pre- trail diversion? h. Been arrested or any pending criminal charges? i. Been cited or charged with any violation of the law? j. Been subject of a court- martial; Article 15 violation; or received any form of military judgment/punishment/action? (You may only exclude Class C misdemeanor traffic violations.) NOTE: Expunged and Sealed Offenses: While expunged and sealed offenses, arrest, tickets, or citations need to be disclosed, it is your responsibility to ensure the offenses, arrest, tickets, or citation has in fact, been expunged or sealed. It is recommended to you submit a copy of the Court Order expunging or sealing the record in question to the BON office with your application. Failure to reveal an offense, arrest, ticket, or citation that is not in fact expunged or sealed, will at a minimum, subject your license to a disciplinary fine. Non- disclosure of relevant offenses raises questions related to truthfulness and character.

6 NOTE: Orders of Non-Disclosure: Pursuant to Tex.Gov t Code (b), if you have criminal matters that are the subject of an order of non-disclosure you are not required to reveal those criminal matters on this form. However, a criminal matter that is the subject of an order of non-disclosure may become a character and fitness issue. Pursuant to the other sections of the Gov t Code chapter 411, the Board of Nursing is entitled to access criminal history record information that is the subject of an order of non- disclosure, even if you properly did not reveal that matter, the Board may require you to provide information about that criminal matter. 2) Are you currently the target or subject of a grand jury or governmental agency investigation? 3) Has any licensing authority refused to issue you a license or ever revoked, annulled, canceled, accepted surrender of, suspended, place on probation, refused to renew a professional license, certificate or multi-state privilege held by you now or previously, or ever fined, censured, reprimanded or otherwise disciplined you? 4) Within the past five (5) years have you been addicted to and/or treated for the use of alcohol or any other drug? 5) Within the past five (5) years have you been diagnosed with, treated, or hospitalized for schizophrenia and/or psychotic disorder, bipolar disorder, paranoid personality disorder, antisocial personality disorder, or borderline personality disorder? Declaratory Orders for Eligibility for License The Texas Board of Nursing has identified certain circumstances that may render a potential candidate ineligible for licensure as a registered nurse in the State of Texas. The Board provides individuals the opportunity to petition the Board for a Declaratory Order as to their eligibility in accordance with article (a) of the Nursing Practice Act.
