Mosaic Life Care: Health Care Reimagined Combining Traditional Health Care with a Focus on Key Life Elements
Welcome to our program. Mosaic Life Care is health care, re-imagined. Our innovative life-care model combines traditional health care and a focus on key life elements that affect overall wellness. Mosaic Life Care is the next generation of health care. Mark Laney, M.D. President and CEO Mosaic Life Care at St. Joseph, Mo. Curt Kretzinger COO Mosaic Life Care at St. Joseph, Mo.
Who we are.
Where we are going. Vision: to be an innovative organization that helps to reinvent American healthcare. Mission: to establish a life-care organization by focusing on patient-centered optimal health, respectively.
Becoming life care. A new approach to health care To really help you live life well, we don t just want to treat you when you are sick we want to help you stay well in mind, body and spirit. So we designed a new approach to health care by asking, What if, What if we became life care?
Living life care. The vision and mission are supported by a platform of seven principles that affect overall wellness:
A strong foundation. Performance and Efficiency: The Baldrige journey had created a robust, staff-driven performance improvement infrastructure and culture. Electronic Health Records: Mosaic Life Care was an early adopter of electronic health records and the EHR has achieved HIMSS stage 6 certification Strong regional market position: Mosaic Life Care s market area covers a 23-county, three-state swath. Physician integration: Mosaic Life Care is an integrated delivery system with its owned multispecialty group of 160 physicians.
What we do. Mosaic Life Care at St. Joseph Inpatient Acute Care Emergency Department Specialty Clinics Long Term Acute Care Hospital Mosaic Life Care Centers Family Care Sports Medicine Outpatient Care Shoal Creek Life Center Mosaic Life Care at Saint Joseph, Missouri
What we do. Mosaic Life Care Business Solutions On-site health clinics featuring integrated population health management services: High quality Mosaic Life Care providers Patient-centered medical home Life-care model Employee-risk stratification Targeted chronic-condition management On-site virtual clinics Mobile clinics On-site telemedicine On-site wellness programs Biometric screenings Health coaching Life coaching Occupational Medicine than health care life care.
Improving population health.
Improving population health. Robust Care-management Navigators m-page Dashboard Medical Informatics Quality Improvement and Patient Safety Community Health Needs Assessment
Accountable care.
A value proposition. Bundled Payments for Care Improvement Model 2 retrospective bundled payment arrangement fee-for-service (FFS) payments reconciled against a bundled payment amount
A multi-state network. Specific initiatives already underway: Expanding telemedicine to underserved areas of Missouri Enhancing health care quality led by physician teams Creating a database for analytical research and evidencebased medicine to support population health management Working with payers to offer innovative network options that are patient-centered and offer high-quality care at a lower cost Exploring the development of a clinically integrated network of employed, independent and private practice physicians focused on improving health care quality and outcomes
Mastering the fundamentals. Process efficiency. Attained by the Baldrige experience as well as training in LEAN and Six Sigma. Best valued provider. As recognized by CMMI as among the highest quality MSSP-ACOs. Established an IT infrastructure. Robust information technology and data warehousing allows analytics that translates to higher quality, greater safety and more efficiency in organizations. Physician alignment. Physician champions who understand the process and implement it. Experience with risk-taking. Community Health Plan, a now defunct insurance company offered many lessons.
Conclusion.
Discussion We invite your questions and comments.
