3 PREFACE The number one complaint among therapists always seems to be that documentation is too time consuming and they would much rather be spending their time one on one with their clients. The following school based therapy forms were developed to offer a thorough, user- friendly format for consistent communication between therapists, families and school personnel. The versatile forms will assist with referrals, prescription acquisition, progress reports, daily notes, evaluations, annual reviews and more. These reporting forms assist in developing and monitoring each student s progress towards their annual goals. They are flexible and provide a checklist for school based practice. Each form works as a guide for school based therapists to assist in all of your documentation and treatment needs. Most importantly, these reports provide an increase in communication between the therapist, family and teachers. Each form is reproducible to use for every one of your students.
