1 1 PM&R RESIDENCY PROGRAM DEPARTMENT of PHYSICAL MEDICINE and REHABILITATION (PM&R) at VIRGINIA COMMONWEALTH UNIVERSITY (VCU) Welcome and thanks for your interest in the PM&R residency program at Virginia Commonwealth University (VCU) and the Medical College of Virginia Hospitals along with the McGuire Veterans Affairs Medical Center (VAMC). Our three-year fully-accredited PM&R residency program accepts 6 residents annually (through NRMP) and stresses comprehensive medical rehabilitation for individuals with traumatic brain injury, stroke, musculoskeletal injury, spinal cord injury, pediatric disorders, orthopedic and multi-trauma injuries, along with specialty outpatient clinics for pain management, musculoskeletal, sports medicine, electrodiagnosis and orthotics/prosthetics. Upon completion of our fully accredited PM&R residency, VCU graduates demonstrate the knowledge & skills to be highly competent Physiatrists and leaders in the field of PM&R. A combined approach of extensive clinical exposure, bedside teaching, formal lectures, and research mentoring is provided by an experienced and nationally recognized faculty. An Interdisciplinary team approach is utilized, working closely with physical/occupational/speech therapy, nursing, psychology, social work, along with other medical/surgical specialties: such as neurology, neurosurgery & orthopedics. Residents complete a research project during their residency with a departmental faculty advisor. 5 5 VCU Medical Center Rotations 1. General Rehabilitation: The general rehabilitation rotation at VCU provides care to patients with multiple medical problems, including Stroke, Orthopedic & Medically Complex issues, Polytrauma, Amputations, Cardio-Pulmonary and Cancer rehab. Resident s work closely with PM&R faculty (Patrick McGowan, MD) and the interdisciplinary team to learn medical treatment in the rehabilitation setting, restoring functional independence, team management skills, patient/family education and long-term management issues. 2. Spinal Cord Injury Rehabilitation: The spinal cord injury (SCI) rehabilitation service at VCU provides care to patients with traumatic and non-traumatic SCI. There is a 14-bed specialty SCI inpatients service along with SCI consult & long-term outpatient follow-up services. Residents work closely with PM&R faculty (William McKinley MD) and the interdisciplinary team to learn SCI medical management, anatomy/classification, neurological/functional outcomes, rehabilitation, secondary complications (spine stability, neurogenic bowel/bladder, pain, spasticity, skin care, DVT, sexuality, cardiopulmonary issues), equipment/modifications needs and patient/family education. 3. Brain Injury Rehabilitation: The brain injury (BI) rehabilitation service at VCU provides care to patients with traumatic & acquired brain injury, stroke, aneurysms, brain tumors and Parkinson s disease in a 14-bed specialty BI unit. Residents work closely with PM&R faculty (William Walker MD, Richard Kunz MD) and the interdisciplinary team to learn neuro-rehabilitation and the influence of medical & pharmacologic management on neurorecovery, BI medical management of cognitive deficits, agitation, spasticity, reflex sympathetic dystrophy, depression and seizures. 4. Consults: The PM&R consult service at VCU provides care to patients referred from the VCU Level 1 Trauma Center. Resident s work closely with PM&R faculty (Drs. McGowan, Kunz, McKinley) to assess patients with various traumatic and nontraumatic disorders. Residents provide evaluation and recommendations to guide other services as to the rehabilitation principles and when to recommend inpatient rehabilitation for a broad spectrum of patients in the acute care setting. 5. VCU Ambulatory Care Center (ACC) and VCU Sport and Spine Center: Residents work closely with PM&R (Mike DePalma, Maged Hamza, Yaoming Gu, Karen Steidle) and related faculty in VCU outpatient services to provide care for patients with various neurological and musculoskeletal needs, including: Musculoskeletal, Pain and industrial rehabilitation Injection techniques (spinal, ultrasound-guided, pump placement & refills) Prosthetics & orthotics (Physiatrists, prosthetists/orthotists). Specialty clinics (traumatic brain injury, spinal cord injury, spasticity) Rheumatology (Rheumatology & PM&R) Sports Medicine (Orthopedic & PM&R) Neuromuscular (Neurology & PM&R) Electrodiagnosis (PM&R & Neurology) 6. Pain Medicine and VCU Spine Center: The pain rehabilitation rotation at VCU provides care to patients with acute and chronic pain secondary to various etiologies, including: musculoskeletal/soft tissue, neurologic, and cancer. Residents work closely with PM&R faculty (Mike DePalma, Maged Hamza, Yaoming Gu, Karen Steidle Abu Qutubuddin MD, Pain Medicine Fellows) and the interdisciplinary team to learn specialty pain management approach including therapeutic, pharmacological and interventional treatment.

Electrodiagnosis (EMG): EMG services at VCU (under supervision of both PM&R and Neurology faculty) provides for a comprehensive overview and in-depth training/experience in all aspects of electrodiagnostics. Resident s work closely with faculty (William McKinley MD, Karen Steidle, MD) to receive graduated levels of responsibility and receive hands-on experience to over 200 EMG s. Formal weekly didactic lectures and competency worksheet/testing are provided to review neuromuscular anatomy/physiology, instrumentation and interpretation of EMG. 8. Elective Rotation: The elective rotation (2 months) for VCU PM&R residents provides an opportunity for expanded and in-depth experience with a variety of clinical or research endeavors, depending on their individual interests. Veteran s Administration Medical Center (VAMC) 9. VAMC General/Polytrauma Inpatient Service: The 15-bed inpatient polytrauma and general rehabilitation service at McGuire VA hospital provides care to patients with Polytrauma and general rehabilitation (including stroke, orthopedic and amputee). Resident s work closely with PM&R faculty (Shane McNamee, David Cifu, Gary Goldberg) and the interdisciplinary team to learn medical treatment in the rehabilitation setting, restoring functional independence, appropriate team management skills, patient/family education and long-term management issues. 10. SCI Medicine Service: The spinal cord injury (SCI) medicine service at VAMC provides care to patients with acute & chronic SCI on a 50-bed specialty SCI inpatients service along with SCI consult & long-term outpatient follow-up services. Residents work closely with the faculty (David Gater, Tim Lavis, Ted Castillo) and the interdisciplinary team to learn SCI medical management, anatomy/classification, neurological/functional outcomes, rehabilitation, secondary complications (spine stability, neurogenic bowel/bladder, pain, spasticity, skin care, DVT, sexuality, cardiopulmonary issues), equipment/modifications needs and patient/family education. 11. VAMC Outpatient Clinics: Residents work closely with PM&R (and related specialty) faculty (Jeff Ericksen, Shane McNamee, Tripti Jena, Douglas Murphy) in VAMC outpatient services to provide care for patients with various neurological and musculoskeletal needs, including: Polytrauma Prosthetics & orthotics (Physiatrists, prosthetists/orthotists). Specialty clinics (traumatic brain injury, spinal cord injury, spasticity) Musculoskeletal, Pain and industrial rehabilitation Specialized Pain Medicine & injection clinic Rheumatology (Rheumatology & PM&R) Cardiac Rehabilitation Electrodiagnosis (PM&R & Neurology)

7 7 Richmond Children s Hospital 14. Pediatric Rehabilitation: The pediatric rehabilitation rotation at VCU and Richmond Children s Hospital provides care to patients with cerebral palsy, muscular dystrophy/neuromuscular disorders, TBI, SCI, spina bifida, neurodevelopmental delays, JRA, spasticity, burns and scoliosis. Residents work closely with PM&R faculty (Eugene Monasterio, Chad Dillard) and the interdisciplinary team to experience a variety of inpatient and outpatient pediatric-rehab issues. Additionally, pediatric consultation services are available at VCU. Community Rehabilitation 15. Community inpatient rehabilitation: This rotation is based at Sheltering Arms Hospital (SAH), a non-profit rehab hospital in Richmond (with additional satellite-clinics throughout the city) that provides care to patients with general rehabilitation disorders (predominantly orthopedic, stroke and medically complex). Resident s work closely with PM&R physicians and the interdisciplinary team to experience interaction and referral patterns unique to the community based Physiatry, acute inpatient consultations, managing patients on the rehabilitation unit, optional EMG s and outpatient follow-up evaluations 16. In addition to clinical activities, students will be exposed to the didactic lectures and grand rounds with the PM&R residents and faculty MI - Intro to PM&R (Jan-Feb) MII - Intro to PM&R & Advanced PM&R (Oct-Dec) M-IV (4th year): Physical Medicine and Rehabilitation "Acting Intern" (Course 6000): Inpatient PM&R Service (spinal cord, polytrauma, orthopedic, stroke, neuromuscular disorders). Brain Injury "AI" (Course 6001): Inpatient PM&R Brain Injury Rehabilitation Services. PM&R Rotating Elective (Course 9500): Exposure to PM&R Consult Team, Inpatient Rehab and Outpatient PM&R Clinics Pediatric Rehabilitation (Course 9600): PM&R exposure at VCU and Children's Hospital.
