1 INDEPENDENT SCHOOL DISTRICT 833 Community Education 8400 East Point Douglas Road South Cottage Grove, MN Notice is hereby given, that Independent School District 833, South Washington County Schools, MN, seeks proposals from qualified vendors of teen driving education services interested in providing high quality driver training and instruction. 2. Written proposals must be received in the District Program Center, Independent School District 833, Community Education, 8400 E. Pt. Douglas Rd. S., Cottage Grove, MN 55016, no later than August 24th at 2:00 pm, at which time the written proposals will be opened. Following the opening, School District representatives may conduct interviews with one or more service providers supplying a written proposal for purposes of evaluation, clarifying the submitted quotation, and negotiating terms. 3. Introduction South Washington County Schools Community Education services seeks written proposals from qualified vendors of teen driving education services interested in providing high quality driver training and instruction. DISTRICT PROFILE South Washington County Schools provides a high-quality education to students in grades PreK-12 and programs and services to residents from birth through adulthood that live in the cities of Cottage Grove, Newport, St. Paul Park, Woodbury and surrounding areas. South Washington County Schools is home to 14 pre-kindergarten through fifth grade elementary schools, four sixth eighth grade middle schools and three ninth-twelfth grade senior high schools. Approximately 1,000 students reach the eligible age to participate in driver education training each year. Partnerships with local government, businesses, civic and parent organizations continue to enhance the overall learning experiences of students in the district. II. 3 III. General Information and Instructions Written proposals shall be submitted in a plain, sealed envelope and shall be plainly marked- Driver Education Proposal. Written proposals shall be delivered on or before August 24 th at 2:00 PM to the following address: Laurie Brang Office Coordinator South Washington County Schools Community Education 8400 E. Pt. Douglas Rd. S. Cottage Grove, MN IV. Program Information Included in this section are details, terms and responsibilities on which to base your business proposal. These details, terms and responsibilities are not exhaustive. Either Community Education or the driving school has the right to terminate any agreement with ninety days written notice. This does not absolve the driving school from any contract between the driving school and prior students. Community Education reserves the right to cancel any class without advance notice. Community Education reserves the right to visit or inspect the business office, classroom and vehicles during regular operating hours. The fee structure that is proposed in the below answers will be fixed for two years. Annual increases after the first two years must be pre-approved by Community Education. All additional charges/rentals/deposits/surcharges etc. must be disclosed below. Increases in these charges must be pre-approved by Community Education. At minimum, Community Education would like to offer ten summer classroom sessions, and nine school year classroom sessions per calendar year. Classroom sessions could be held at East Ridge High School, Park High School and Woodbury High School. For the sake of this proposal, assume; o 10 summer sessions in our three high schools o 9 school year sessions in our three high schools Additional Community Education Responsibilities Community Education will schedule and reserve a room for the driver education program. Provision of specific classroom equipment will be agreed upon by Community Education and the driving school at a future time. Community Education will process the initial payment and general registration for the student. Community Education will distribute driver education flyers for this program to the offices at each of our high schools and middle schools and will provide a listing in its department 3

4 catalog as well as on the Community Education website. Periodic list-serve s may also be used for marketing purposes. Community Education will provide the class roster. Additional Driving School Responsibilities Provide 30 hours of classroom instruction time to approximately 30 students (at least) nineteen times per year (beginning as soon as you are able ISD 833 would consider transferring current registrations to your program if necessary {negotiable}) plus 6 additional hours of Behind the Wheel instruction to each registered individual student (beginning as soon as you are able ISD 833 would consider transferring current registrations to your program if necessary {negotiable}), per Minnesota State Guidelines. The driving school will maintain adequate vehicles to meet the needs of all registrants. The driving school will provide, on an annual basis, a copy of their liability insurance certificates specifically naming South Washington County Schools ISD 833 as additional insured. Liability coverage must be in an amount no less than $2,000,000 in general liability, $2,000,000 in automobile liability per occurrence, as well as a minimum of $2,000,000 in additional umbrella coverage. The driving school will be responsible for collecting any additional charges/rentals/deposits/ surcharges required beyond the initial payment. The driving school will communicate with Community Education any discrepancies between the official roster and the students that attend class. This needs to be completed prior to invoicing Community Education. The driving school will submit invoices, including a roster of the students being invoiced for, after each session has been completed. The driving school must notify Community Education in writing within thirty days of receiving any citation from the state regarding any aspect of business operations. How much insurance will your business carry if it is selected for this agreement (include all coverages)? 11. How much is your business annual surety bond? Does the surety bond extend to cover classroom instruction and behind the wheel instruction when programs are offered in partnership with South Washington County Schools? If no, how will a satisfaction guarantee to our students be upheld? 12. How much will your business annual surety bond be if it is selected for this agreement? 13. What is the total number of fault or partial-fault crashes and moving driver convictions your business has experienced in 2013 and 2014 (include all instructors, owners and officers)? 14. What is the total number of driver or instructor license revocations your business has experienced in 2013 and 2014 (include all instructors, owners and officers)? 15. What is the total number of fault or partial-fault crashes your business student drivers experienced in 2013 and 2014? Personnel 16. How many full- and part-time classroom instructors do you employ? How many full-and part-time BTW instructors do you employ? Do you have both male and female instructors? 17. What is the number of instructors who currently hold teaching licenses through the State of Minnesota? 18. Specifically, what background checks are completed on each type of employee? How often are these background checks completed? Provide a brief statement about the background and credentials for each instructor that would teach in the classroom or in behind-the-wheel lessons. 5

6 19. Please describe your requirements for training, observations and evaluation of teachers. Classroom 20. If your business currently has fixed classroom locations in ISD 833, where are they located and in what type of facilities? Will you continue to use these spaces if selected as the RFP winner with South Washington County Schools? 21. Please list the schools and/or school districts currently using your Driver Education services; please include contact information. 22. How many students completed the classroom portion of your business program in 2013 and 2014? 23. If your business is selected for this agreement, what is the minimum and maximum number of students you would require to run a class? 24. Does your business have any minimum or maximum age limitations? 25. Do you provide each student a Minnesota Driver s Manual? Is there a charge? If you do not provide a driver s manual, please describe how students study the manual. 26. Please provide examples of study guides, worksheets, etc. that are provided to the student. 27. Provide copies of the lesson plans that are used in the classroom. 28. Are there any out of classroom activities? If so, please describe the activities and their value to the student. 29. How would a student who misses a classroom lesson, arrange to make up that lesson? 30. What experience does your business have in making class accommodations to meet the special needs of students or students who have disabilities? 31. What is your business classroom discipline policy? 32. How is classroom behavior managed? Parent Involvement 33. What type of parent class/orientation does your business currently offer? Does it meet the state requirements? 34. If a parent is unable to attend this class/orientation, what other methods will be available to convey the information? Behind-The-Wheel (BTW) 35. How many students completed the BTW portion of your business program in 2013 and 2014? 36. Provide copies of the lesson plans that are used in BTW lessons. 37. Provide copies of the written evaluations that are given to students in BTW lessons. 38. When students schedule their BTW lesson, is there someone assigned to answer the telephone during office hours or do they leave a voic ? When voic s are left, what is your business average response time? 39. Can BTW lessons be scheduled online? If yes, please describe. 6

7 40. What is the average wait time it takes from the time a student calls to schedule a BTW lesson and when that lesson occurs? 41. Do students have the ability to schedule multiple BTW lessons at the same time or to do this online? 42. What is your business general policy for missed BTW lessons? 43. Is there a time limit on how long a student has to complete BTW lessons? If yes, please describe. 44. Will your business pick students up and drop them off at their home? 45. What experience does your business have in making BTW accommodations to meet the special needs of students or students who have disabilities? Vehicles 46. What is the number of vehicles that your business owns? 47. What is the number of vehicles that your business leases? 48. List the year, make, model and transmission type of each vehicle. 49. Are there formal maintenance schedules and records for each vehicle? 50. Do any of the BTW instructors use their own vehicles during BTW lessons? Price 51. Provide a copy of the contract that parents or guardians sign. 56. What is the price, per student, that your business would invoice Community Education for classroom and BTW? (Community Education would establish the overall cost to the client to cover operational and facility costs associated with the program). 57.
