1 UTILITY ACTIVATION AUTHORIZATION & DE-WINTERIZATION/RE-WINTERIZATION INSTRUCTIONS FOR HOME BUYER INSPECTION/APPRAISAL (Servicing HUD Properties in AR, KS, LA, MO, OK, & S. TX) After the sales contract is executed, the buyer/agent is responsible for activating the utilities in order to complete a home inspection. The buyer/agent assumes all utility activation fees and pays those fees directly to the utility provider(s). This also includes the purchase of fuel oil or propane, if applicable. TO BEGIN THE PROCESS, follow the steps listed below. Please note that regardless of the time of year, A2Z properties may be winterized and must be de-winterized in order to complete the home buyer inspection. Failure to notify A2Z within 36 hours of any schedule changes may result in additional fees. STEP 1: Eligibility Determination A2Z Field Services 2DUAD@a2zfs.com to confirm the property can be de-winterized. Your subject line should include the HUD case number and complete property address. An ed response can be expected within 1 business day. STEP 2: Utility Activation Once authorization has been received, the buyer/realtor may call the utility companies and have the utilities activated to the OUTSIDE meters only. NOTE: A2Z DOES NOT AUTHORIZE WATER TO BE TURNED ON INSIDE THE HOUSE PRIOR TO DE-WINTERIZATION. STEP 3: Form Completion and Payment Remission Once you have the utility activation dates, forms and payment should be sent to A2Z using one of the following methods: 1. Check: a. Complete the Utility Activation Form (Page 3) circle payment by money order/cashier s check & sign and date the form. b. If this request is being made April 1 st September 30 th, you must also complete the No Re- Winterization Acknowledgement (page 5). c. Make check payable to A2Z Field Services using the following guidelines for the amount: April 1 st -September 30 th when re-winterization is not required: $90.00* October 1- March 31 st : $190.00* d. Reference the HUD Case Number in the memo line of your check or money order. e. a copy of the money order/cashier s check with your completed forms to 2DUAD@a2zfs.com. f. Mail the check with your completed forms within 2 business days to: A2Z Field Services ATTN: HUD Accounts Receivable 5747 Perimeter Drive, Suite 250 Dublin, OH We recommend overnight/next day mail to expedite your funds during your due diligence period. 2 2. Credit Card/PayPal: a. Complete the Utility Activation Form (Page 3) circle payment by credit card & sign and date the form. (DO NOT INCLUDE CREDIT CARD INFORMATION) b. If this request is being made April 1 st September 30 th, you must also complete the No Re- Winterization Acknowledgement (page 5). c. your completed forms to 2DUAD@a2zfs.com. d. You will receive an invoice from PayPal via using the following guidelines for the amount: April 1 st -September 30 th when re-winterization is not required: $90.00* October 1- March 31 st : $190.00* A 2.9% + $0.30 PayPal Processing Fee will be added to your total bill. e. Follow the instructions on the ed invoice to complete your credit card transaction. f. A2Z will be automatically notified that the payment is complete. STEP 4: Confirmation & Completion of De-Winterization After receiving the forms and payment confirmation via , A2Z will notify you via that the dewinterization has been scheduled. You will also be notified once completed. Please note that the dewinterization will NOT be scheduled until we have received a copy of all forms, AND confirmation of receipt of funds from either our corporate office or from PayPal. STEP 5: Re-Winterization Request If this request is being made October 1- March 31 st, you must notify A2Z once the inspection is complete and the signed Utility Deactivation Form (page 4) to 2DUAD@a2zfs.com. The property will automatically be rewinterized 7 days after the de-winterization even if the Deactivation Form is not submitted in a timely fashion. Disclaimers: If the property has a sump pump present, it is your responsibility to ensure that the electric is not shut off to the pump when your inspection is complete. The breaker for the sump pump and the main breaker are the only breakers that should be left on. Otherwise, all breakers must be left in the off position once the inspection is complete. If the property s electric, gas, and/or water meter is missing it is the BUYER S responsibility to replace them if electrical/gas/plumbing functionality is necessary for financing. A2Z and HUD will not be held liable for those costs to replace the meters. The selling agent, or their designated representative, must be present when all utilities are activated at the property AND when the inspection is taking place. If utilities are activated to the inside of the home, it is considered to be an unauthorized de-winterization and will be reported to the asset manager. Additional fees may be assessed.
